BACKGROUND: Amyloid PET allows for the assessment of amyloid β status in the brain, distinguishing true Alzheimer's disease from Alzheimer's disease-mimicking conditions. Around 15-20% of patients with clinically probable Alzheimer's disease have been found to have no significant Alzheimer's pathology on amyloid PET. However, a limited number of studies had been conducted on this subpopulation in terms of clinical progression.
OBJECTIVE: We investigated the risk factors that could affect the progression to dementia in patients with amyloid-negative amnestic mild cognitive impairment (MCI).

OBJECTIVE: People with Parkinson disease (PD) frequently develop dementia, which is associated with neocortical deposition of alpha-synuclein (α-syn) in Lewy bodies and Lewy neurites. In addition, neuronal loss and deposition of aggregated α-syn also occur in multiple subcortical nuclei that project to neocortical, limbic, and basal ganglia regions. Therefore, we quantified regional deficits in innervation from these PD-affected subcortical nuclei, by measuring the neurotransmitters and neurotransmitter transporter proteins originating from projections of dopaminergic neurons in substantia nig…

Microglia are the primary immune cells of the brain, but their role in Parkinson's disease is not fully understood. Chronic microglial activation is toxic to neurons, but in the early stages of pathology, microglia also exert beneficial functions. We used induced pluripotent stem cell (iPSC)-derived coculture models to investigate how human microglia respond to α-synuclein aggregates that form de novo inside human dopaminergic neurons with α-synuclein gene triplication or are triggered by fibrils. We found that microglia cleared Ser129-phosphorylated α-synuclein aggregates through a contact-de

α-Synuclein (αSyn) is a presynaptic protein of unestablished physiological function that plays a central role in Parkinson's disease neuropathology. To date, the reported effects of αSyn expression on the kinetics of axonal synaptic vesicle exocytosis and membrane cycling have been relatively small. In contrast, we report that αSyn is the major modulator of substantia nigra somatodendritic dopamine release, a little-understood form of neurotransmission that is central to sensorimotor and basal ganglia circuitry. This regulation is due to a previously unknown role for αSyn in controlling the ac

Parkinson’s disease (PD) is a progressive neurodegenerative disorder characterized by oxidative stress, neuroinflammation, and α-synuclein accumulation. Activation of the angiotensin II type 1 (AT1) receptor contributes to these pathogenic mechanisms. This study aimed to evaluate the neuroprotective effects of telmisartan, an AT1 receptor antagonist, in a cellular model of PD. An in vitro PD model was established by exposing SH-SY5Y neuroblastoma cells to 200 µM 6-hydroxydopamine (6-OHDA). Cells were pre-treated with varying concentrations of telmisartan prior to toxin exposure. Cell viability

Human midbrain organoids (hMOs) derived from induced pluripotent stem cells provide a powerful system to model disorders involving dopamine (DA) dysfunction, including Parkinson's disease (PD) and neuropsychiatric conditions. However, current differentiation protocols still fall short in recapitulating early specification, substantia nigra pars compacta (SNpc)-like identity, and the functional maturation of vulnerable DA neurons. Here, we established a differentiation strategy that combines tri-phasic WNT modulation with dynamic bioreactor culture to generate hMOs enriched in SNpc-like DA neur

Parkinson's disease (PD) is a complex neurodegenerative disorder characterized by the progressive loss of dopaminergic neurons. This study aimed to elucidate the dynamic interplay between these pathological pathways in a rotenone-induced rat model of PD. Adult male Wistar rats were chronically treated with rotenone. Behavioral assessments included Rotarod, Open Field, Rotating Pole, and Cylinder tests. On days 7, 14, and 21, gene expression levels of key markers including α-synuclein, NLRP3 inflammasome components (NLRP3, ASC), inflammatory cytokines (IL-1β, TNF-α, IL-6), upstream signaling mo

Cognitive impairment is a common but poorly understood non-motor aspect of Parkinson's disease, negatively affecting the patient's functional capacity and quality of life. The mechanisms underlying cognitive impairment in Parkinson's disease remain elusive, limiting treatment and prevention strategies. This study investigates the molecular and cellular basis of cognitive impairment associated with heterozygous mutations in GBA1, the strongest risk gene for Parkinson's disease, which encodes glucocerebrosidase, a lysosome enzyme that degrades the glycosphingolipid glucosylceramide into glucose

Polypathology is a major driver of heterogeneity in the clinical presentation and extent of neurodegeneration (N) in patients with Alzheimer's disease (AD). Beyond amyloid (A) and tau (T) pathologies, over half of patients with AD have concomitant pathology such as α-synuclein (S) in mixed AD with Lewy body disease (LBD). Patients with multiple aetiology dementia such as AD + LBD have faster progression and potentially differential responses to targeted treatments, although the diagnosis of AD + LBD can be challenging given the overlapping clinical and imaging features. The metabolic disorder of α-synuclein in the central nervous system (CNS) is a key factor leading to excessive accumulation of aggregated α-synuclein and ultimately to the formation of Lewy bodies, which is one of the main pathological features of Parkinson's disease (PD). Although the mechanism underlying this process remains elusive, systemic metabolic abnormalities have been demonstrated to affect the progression of neurodegenerative diseases. The recent discovery of meningeal lymphatics provides a specific route for connection between the CNS and the periphery. As draining lymph nodes for

Progressive supranuclear palsy (PSP) is a neurodegenerative disorder characterized by four-repeat (4R) tau protein deposition. The substantia nigra (SN) and midbrain tegmentum nuclei (MBT) are consistently affected. Lymphocyte infiltrates are scarce in the brains of patients with neurodegenerative diseases, although a few reports have described their presence in the α-synucleinopathy Parkinson's disease (PD). To evaluate the cytotoxic T-cell response, serial sections spanning 120 μm of the SN were immunostained consecutively for phosphorylated tau (p-tau, AT8) or α-synuclein, cytotoxic T-cell

BACKGROUND: Timely and accurate diagnosis of progressive supranuclear palsy (PSP) remains challenging.
OBJECTIVE: To assess diagnostic certainty and progression biomarkers in the PSP spectrum from "suggestive of" (so-PSP) category as proxy of early disease, to definite (neuropathologically confirmed) cases.
METHODS: Multicenter, prospective, longitudinal study of 131 participants (so-PSP, n = 23; definite, n = 5) with oculometric (n = 47) and cerebrospinal fluid (CSF) biomarkers (n = 75), compared with control (n = 18) and Parkinson's disease (PD) subjects (n = 12).
RESULTS: Anti-saccade ve

Cerebrospinal fluid (CSF) α-synuclein (α-syn) seed amplification assays (SAAs) can detect Lewy body pathology (LBP) with high accuracy but are invasive and costly. To address these challenges, this study evaluated a two-step workflow combining prescreening via smell-function testing with confirmatory CSF α-syn SAA testing only in individuals with reduced smell, for predicting postmortem LBP status. Among 358 autopsied participants, the two-step workflow predicted brain LBP with high accuracy overall (94%), and within clinical subgroups (clinical parkinsonism=95%; clinical Alzheimer's disease [

The clinical heterogeneity of multiple system atrophy (MSA) may, along with the primary aggregation of alpha-synuclein (α-syn), partly be shaped by co-pathologies, such as amyloid-beta (Aβ), phosphorylated (p)-tau, and pTDP-43, though their relevance remains unclear. Here, we aimed to characterize the prevalence, morphology, regional patterns, and clinical relevance of co-pathologies in a well-characterized MSA autopsy cohort. Regional load (%area) and morphological characterization of α-syn (KM51), Aβ (6F/3D), p-tau (AT8), and pTDP-43 (pSer409) pathology were assessed in limbic regions of MSA…

INTRODUCTION: Most septic patients have severe sleep disorders that are closely related to poor prognoses, and some patients with sepsis-associated encephalopathy (SAE) experience severe cerebral inflammatory reactions similar to those in neurodegenerative disease. α-synuclein, a widely expressed synaptic protein whose abnormal deposition can lead to neurodegenerative diseases, was also observed to be deposited abnormally in an SAE mouse model.
OBJECTIVES: Sleep disorders are common in SAE patients and are strongly associated with prognoses. Our aim is to clarify sleep architecture changes, i

A slow decline in the autophagy-lysosomal pathway is a hallmark of the normal aging brain. Yet, an acceleration of this cellular function may propel neurodegenerative events. In fact, mutations in genes associated with the autophagy-lysosomal pathway can lead to Parkinson's disease. Also, amyloidogenic protein deposition is observed in lysosomal storage disorders, which are caused by genetic mutations representing risk factors for Parkinson's disease. For example, Gaucher's disease GBA1 mutations leading to defects in lysosomal sphingolipid metabolism cause α-synuclein accumulation. We observe

BACKGROUND: Neurological disorders are the second leading cause of death and the leading cause of disability in the world. Thus, the development of novel disease-modifying strategies is clearly warranted. We have previously developed a therapeutic approach using mouse targeted rabies virus glycoprotein (RVG) extracellular vesicles (EVs) to deliver minicircles (MCs) expressing shRNA (shRNA-MCs) to induce long-term α-synuclein down-regulation. Although the previous therapy successfully reduced the pathology, the clinical translation was extremely unlikely since they were mouse extracellular vesi

The blood-brain barrier (BBB) serves as an interface between the bloodstream and the central nervous system. It limits the movement of molecules and immune cells, regulates the entry of nutrients, and removes waste products from the brain. The dysfunction of the BBB has been identified in Parkinson's disease (PD) but the role of the BBB and endothelial cells (ECs) has not been well studied. LRRK2 G2019S mutation is the most common PD causing mutation with similar pathophysiology than in sporadic cases. How the mutation affects EC function has not been investigated previously in patient cells.
