Treatment of unilateral coronal synostosis by endoscopic strip craniectomy or fronto-orbital advancement: Ophthalmologic findings.

BACKGROUND: Unilateral coronal synostosis results in ipsilateral retrusion of the forehead and superior orbital rim, shortening the elevation of the orbital roof and contralateral frontal bossing and orbital roof depression. This craniosynostosis is associated with the development of V-pattern strabismus and aniso-astigmatism. Since 2004 endoscopic strip craniectomy performed in patients by 3 months of age has been offered as an alternative to fronto-orbital advancement at 9 to 11 months of age. We compare the incidence and severity of V-pattern strabismus and aniso-astigmatism in children tre

Angular photogrammetric analysis of the soft tissue facial profile of Anatolian Turkish adults.

The purpose of this study was to establish norms of photogrammetric soft tissue profile analysis for Anatolian Turkish adults. Standardized right lateral facial photographs of 111 Anatolian Turks (64 females, 47 males) with class I skeletal pattern were taken. Age range was 21 to 40 years. Descriptive statistics for 8 angular measurements were computed. Male and female values were compared with Mann-Whitney U test. Intraclass correlation coefficients (ICCs) were calculated for repeated measurements. Intraobserver reliability of the photogrammetric measurements was quite high with 6 measurement

Preliminary observations on the association between simple metopic ridging in children without trigonocephaly and the Chiari I malformation.

Historically, there have been no accounts of an association between simple metopic ridging and the Chiari I malformation. We prospectively evaluated 50 children with simple metopic ridges and found that 30% of these patients had a Chiari I malformation. During the period in which this screening for simple metopic ridges was performed, only 9% of the total patient population (324) was diagnosed as having a Chiari I malformation. We hypothesize that small decreases in anterior fossa volume in children with a simple metopic ridge and no other clinical findings of trigonocephaly may increase their

Intracranial fibrosarcoma arising 5 years after chemotherapy alone for glioblastoma multiforme in a child.

We present a child diagnosed with glioblastoma multiforme during infancy and successfully treated with the 'eight-in-one' chemotherapy regimen, who developed an intracranial fibrosarcoma 5 years later. After resection of the fibrosarcoma, she received cranial radiation therapy and high dose chemotherapy with bone marrow transplant. She remains alive and recurrence-free 7 years following the diagnosis of her second intracranial malignancy.

Cranioplasty for traumatic deformities of the frontoorbital area.

Cranioplasties of the frontoorbital area pose a major challenge to the surgeon because of the proximity to the globe, the sinuses, and the eyebrows and eyelids. The choice of material for reconstruction includes autogenous and alloplastic materials. A modification of the fixation technique of split rib graft to avoid depression deformities at the periphery of the defect is described. Sixteen patients were reconstructed with different treatment modalities, including the use of alloplastic materials or autogenous bone or a combination of both. The results are reported as are the complications. L

[Intracranial lipomas--report of six cases].

Intracranial lipoma is a very rare tumor, and infantile lipoma has been reported quite scarcely. The authors recently have encountered six cases of intracranial lipoma, five are infants and one is adult, on which the results of neuroradiological examinations, especially CT findings are reported. Two of six cases had lipoma of the corpus callosum associated with cranium bifidum at the midline of frontal region. Two of six cases had lipoma occurred at the quadrigeminal cistern. To one of them showing advanced enlargement in circumference of the head in the postnatal period, the V-P shunt was con

Posttraumatic optochiasmatic arachnoiditis.

Seven cases of adhesive arachnoiditis of the optochiasmatic cistern, verified surgically, with a history of head injury are presented. Neuroophthalmologic findings consisted of impairment of visual acuity and funduscopic atrophy rather than papilledema. The visual fields showed a damage of the periaxial rather than the axial type. Neuroradiologic examinations did not consistently give the findings considered classical of optochiasmatic arachnoiditis. Surgical lysis of the adhesions has given an improvement of the visual function, or at least an arrest in its impairment, in almost all cases. It

MR Imaging in Covid-19-Associated Invasive Fungal Sinusitis.

BACKGROUND AND AIM: We witnessed a sharp peak in the incidence of invasive fungal sinusitis, particularly mucormycosis, in patients with history of coronavirus disease 2019 (COVID-19) infection in India. Rhino-orbito-cerebral mucormycosis (ROCM) is a fulminant rapidly progressive disease. Early diagnosis significantly improves patient survival and outcomes. Hence, neuroimaging plays a very important role. We studied the magnetic resonance (MR) imaging manifestations of invasive fungal sinusitis and established an imaging protocol, which helps in early diagnosis of the disease per se as well as

Lumbosacral arthrodesis for neuromuscular scoliosis using a simplified Jackson technique.

UNLABELLED: Treating patients with severe neuromuscular scoliosis by long spinal fusion improves their quality of life and provides significant comfort for the patient and caregivers. But lumbosacral (L5-S1) fusion is challenging in these patients because of the significant deformities that result in poor bone anchoring quality and a risk of impingement between the skin and implants. In 1993, Jackson described a L5-S1 fusion technique using S1 pedicle screws and intrasacral rods (implanted under X-ray guidance) that are linked to the construct above with connectors. The goal of this study was

[Clinical and image features, and identification of pathogenic gene mutation of two cleidocranial dysplasia families].

OBJECTIVE: Cleidocranial dysplasia (CCD) is a dominantly inherited skeletal dysplasia caused by mutations in the osteoblast-specific transcription factor-encoding gene, core binding factor α1 (CBFA1). Over 90 mutations in CBFA1 gene have been published to date in 500 independent cases of CCD, including missense mutations, deletions, insertions, frameshift, and splice mutations. However, mutational screening of the CBFA1 gene is still far from saturation, and more novel mutations will be identified to enrich the insights into the molecular basis for the pathogenesis of CCD. The aim of this stud

Computed tomography assessment of pelvic bone density: Associations with age and pelvic fracture in motor vehicle crashes.

Motor vehicle crash (MVC) occupants routinely get a computed tomography (CT) scan to screen for internal injury, and this CT can be leveraged to opportunistically derive bone mineral density (BMD). This study aimed to develop and validate a method to measure pelvic BMD in CT scans without a phantom, and examine associations of pelvic BMD with age and pelvic fracture incidence in seriously injured MVC occupants from the Crash Injury Research and Engineering Network (CIREN) study. A phantom-less muscle-fat calibration technique to measure pelvic BMD was validated using 45 quantitative CT scans w

Accurate, firm stabilization using external pins: a proposal for closed reduction of unfavorable nasal bone fractures and their simple classification.

Nasal bone fractures are the most common among facial fractures and are the third most common fractures in the human frame. Although many forms of treatment have been introduced, controversy regarding the optimal treatment still remains. Nasal bone fractures are complex, with significantly varying types that are often undermanaged in closed reduction procedures. The authors' experiences with nasal bone fractures have shown that the baseline for surgical intervention depends on the type of fracture and the method of maintenance after reduction, both of which have considerable impact on the fina

Analysis of relationship between loading condition and cranial cracking pattern using a three-dimensional finite element model.

BACKGROUND: A hairline crack on the cranium can occur even under a small external load or impact and are thus often observed in patients who have experienced an accidental fall or collision. Typical finite element analysis is useful to analyze the stress concentration or the propagation of stress waves. However, a stress propagation model does not accurately reproduce the features of hairline cracks on the cranium. The objective in this study was to reproduce cranial hairline cracks. METHODS: A three-dimensional finite element model of the cranial bone was developed from a patient CT images.

Perioperative Outcomes of Secondary Frontal Orbital Advancement After Posterior Vault Distraction Osteogenesis.

BACKGROUND: Posterior cranial vault distraction osteogenesis (PVDO) has gained popularity as the initial intervention in patients with syndromic craniosynostosis. Patients may require secondary frontal orbital advancement (FOA) following PVDO, but little is known about the perioperative risks associated with this staged management. The purpose of this study is to compare the perioperative morbidity profile of secondary FOA (study) to that of primary FOA (control). METHODS: A retrospective review was conducted for patients with syndromic or complex craniosynostosis undergoing FOA between 2004

Supraorbital vs Endo-Orbital Routes to the Lateral Skull Base: A Quantitative and Qualitative Anatomic Study.

BACKGROUND: Various extensions of the supraorbital approach reach the lateral and parasellar middle cranial fossa regions by removing the orbital rim and greater/lesser sphenoid wings. Recent proposals of a purely endoscopic ventral transorbital pathway to these regions heighten the need to compare these surgical windows. OBJECTIVE: To detail the lateral and parasellar middle cranial fossa regions and quantify exposures by 2 surgical windows (transcranial and transorbital) through anatomic study. METHODS: In 5 cadaveric specimens (10 sides), dissections consisted of 3 stages: stage 1 began w

Comparing two fronto-orbital advancement strategies to treat trigonocephaly in metopic synostosis.

BACKGROUND: Trigonocephalic treatment entails frontoorbital reshaping of the forehead, increasing bitemporal dimensions, and advancing lateral orbits. Various techniques can achieve this, but no consensus exists regarding effects on long-term skull growth. Overcorrecting forehead dimensions is one strategy though preserving a vascularized fronto-orbital bar can influence future growth. We therefore seek to craniomorphologically compare fronto-orbital advancement (FOA), using bandeau widening and advancement, to a pedicled "tilt" procedure to assess whether adequate 3D remodeling is achieved.
