Bacterial meningitis in the pediatric population: paradigm shifts and ramifications for otolaryngology-head and neck surgery.

Various population-based studies have suggested that the incidence and epidemiology of bacterial meningitis is changing. No studies have been published which examine a sample population of the United States at large. Records of pediatric patients age 5 and under who were treated for bacterial meningitis (n = 470) at all US Army medical facilities between 1986 and 1991 were reviewed. The incidence of bacterial meningitis declined by 75% in the study group during this period. The largest decrease occurred in infants less than 1 year of age. The bacterial organisms isolated most commonly, in decr

Functional and Structural Imaging Features of Cognitive Profiles in Multiple Sclerosis.

BACKGROUND AND OBJECTIVES: Cognitive impairment affects 40%-70% of patients with multiple sclerosis (MS). Although recent hypothesis-driven and data-driven efforts have identified cognitive phenotypes, functional substrates underlying impairment in such phenotypes are still not clear. Our objective was to identify distinct cognitive phenotypes using clustering in a large cohort of patients with MS and characterize their associated clinical, structural MRI, and resting-state (RS) functional connectivity (FC) features. METHODS: We conducted an observational, cross-sectional study with right-han

Prevalence and serotype distribution of nasopharyngeal carriage of Streptococcus pneumoniae in Vietnam: a systematic review and meta-analysis.

BACKGROUND: Streptococcus pneumoniae (S. pneumoniae) is a leading cause of childhood morbidity and mortality worldwide. While pneumococcal conjugate vaccines (PCVs) have significantly reduced the global burden of pneumococcal disease, Vietnam has yet to introduce PCV into their National Immunisation Program. Better understanding of pneumococcal disease in Vietnamese children is key to informing vaccination policy, including PCV product selection. The aim of this study was to assess the prevalence, serotype distribution, and antimicrobial susceptibility patterns of nasopharyngeal carriage of S.

Turkish Validation of the Oxford Depression Questionnaire: Psychometric and Clinical Evidence Across Healthy, Drug-Naïve, and Remitted Medicated Depression Groups.

Emotional blunting is a clinically significant yet often under-recognized alteration in emotional experience, characterized by reduced intensity of both positive and negative emotions, diminished emotional responsiveness, and impaired interpersonal engagement. First described through patients' reports of antidepressant treatment, it is now recognized as associated with both depressive illness and pharmacological treatment, underscoring the need for its direct assessment. However, no validated Turkish-language instrument specifically designed to assess emotional blunting is currently available.

LLM-powered socially assistive robot-delivered cognitive behavioral therapy exercises: an exploratory study with university students.

Mental health is a significant healthcare challenge, and cognitive behavioral therapy (CBT) is a widely used therapeutic method for treating anxiety and depression. However, traditional CBT often requires access to trained clinicians and can be cost-prohibitive or logistically difficult for many individuals. To address these barriers, we developed a low-cost socially assistive robot (SAR) that uses a large language model (LLM) to guide the user through interactive at-home CBT exercises. In this exploratory study, 38 university students completed CBT exercises across a 15-day period using one o

Paradoxical Hyperexcitability in Disorders of Neurodevelopment.

Autism Spectrum Disorder (ASD), Rett syndrome (RTT) and Angelman Syndrome (AS) are neurodevelopmental disorders (NDDs) that share several clinical characteristics, including displays of repetitive movements, developmental delays, language deficits, intellectual disability, and increased susceptibility to epilepsy. While several reviews address the biological basis of non-seizure-related ASD phenotypes, here, I highlight some shared biological mechanisms that may contribute to increased seizure susceptibility. I focus on genetic studies identifying the anatomical origin of the seizure phenotype

Can LLMs reliably perform epidemiological extraction in elite football?

Anterior cruciate ligament (ACL) injuries remain a critical burden in professional football, resulting in prolonged absence and high economic costs. While physical demands have increased, traditional epidemiological surveillance relies on manual data extraction, which is resource-intensive and subjective. Large Language Models (LLMs) offer a potential solution for automated screening, but their reliability in sports medicine remains largely unexplored. This study aimed to: (1) update the epidemiology of ACL injuries in the Spanish First Division (LaLiga) over six consecutive seasons (2019/2020…

A Neural Network Approach to Identify Left-Right Orientation of Anatomical Brain MRI.

PURPOSE: This study presents a novel application of deep learning to enhance the accuracy of left-right orientation identification in anatomical brain MRI scans. Left-right orientation misidentification in brain MRIs presents significant challenges due to several factors, including metadata loss or ambiguity, which often occurs during the de-identification of medical images for research, conversion between image formats, software operations that strip or overwrite metadata, and the use of older imaging systems that stored orientation differently. METHOD: A three-dimensional convolutional neur
