[Pulmonary nocardiasis with abscesses spreading to cerebrum, cerebellum and orbits].

HISTORY AND ADMISSION FINDINGS: A 71-year-old woman presented with suspected tuberculosis. She reported having productive coughs, unwanted weight loss and subfebrile temperature in the preceding 3 months. She was known to have chronic obstructive pulmonary disease treated with corticoids given systemically and by inhalation. She was a heavy smoker. INVESTIGATIONS: Computed tomography revealed a left apical lung abscess. In the further course of the disease magnetic resonance imaging of the head demonstrated multiple abscesses in both cerebral hemispheres and an abscess, 3.4 cm in diameter, in

CT and MRI Findings in Cerebral Aspergilloma.

PURPOSE: Invasive aspergillosis usually affects immunocompromised patients. It carries a high risk of morbidity and mortality and usually has a nonspecific clinical presentation. Early diagnosis is essential in order to start effective treatment and improve clinical outcome. MATERIALS AND METHODS: In a retrospective search of the PACS databases from two medical centers, we identified 9 patients with histologically proven cerebral aspergilloma. We systematically analyzed CT and MRI imaging findings to identify typical imaging appearances of cerebral aspergilloma. RESULTS: CT did not show a ty

Community acquired Staphylococcus aureus meningitis and cerebral abscesses in a patient with a hyper-IgE and a Dubowitz-like syndrome.

The Hyper-IgE syndrome (HIES) is a rare primary immunodeficiency which recently has been associated with heterozygous dominant-negative mutations in the signal transducer and activator of transcription 3 (STAT3). Although HIES is characterized by recurrent staphylococcal infections, the microbial invasion of the central nervous system (CNS) is definitively uncommon. We here report on Staphylococcus aureus meningitis and cerebral abscesses acquired in the community in a 31-year-old female patient with a de novo heterozygous mutation of STAT3 and a Dubowitz-like syndrome characterized by growth

Cerebellar abscesses caused by dog bite: a case report.

INTRODUCTION: Bites by house pets can be lethal or cause a series of catastrophic events with severe sequels, such as the loss of a limb or a systemic infection which may be life-endangering, especially in the case of children being bitten. PRESENTATION: A 2-year-old girl was attacked by a dog, causing lesions at the occipital region. This was treated initially as a superficial wound that became further complicated with two cerebellar abscesses. These abscesses required neurosurgical and antimicrobial treatment, with a satisfactory outcome. CONCLUSION: The precise and diligent evaluation of

Meningioma associated with abscess formation--a case report.

A rare case of meningioma associated with both intratumoral and peritumoral abscess formation occurred in a 38-year-old man presenting with signs and symptoms of elevated intracranial pressure, intracranial infection, and right temporal pole mass lesion. The mass lesion was totally removed, revealing a meningioma. Group B streptococcus and peptostreptococcus were cultured from both the tumor and peritumoral white matter. Hematogenous spread of the organisms related to recent dental work was the likely mechanism.

[MRI and Listeria monocytogenes rhombencephalitis].

Rhombencephalitidis is a serious form of brainstem inflammation, difficult to diagnose on the basis of clinical and laboratory findings alone. We describe the MR appearance of 3 cases of Listeria monocytogenes rhombencephalitidis and correlate the findings with clinical information. MR imaging is crucial for early diagnosis of this illness: patchy signal hyperintensity throughout the medulla and cerebellar peduncles on T2 weighted images, always in association with a hypointense dot; numerous gadolinium-enhanced microabcesses in the rhombencephalon. MR imaging is very useful for follow-up exam

Infectious intracranial complications of sinusitis, other than meningitis, in children: 12-year review.

Sinusitis is usually a mild illness in children, but intracranial complications can be life-threatening. We retrospectively reviewed nine cases of intracranial infections secondary to paranasal sinusitis that occurred over a 12-year period, excluding patients with orbital infection only. Cases were highly age- and sex-associated: the median age was 14 years, 89% of patients were > 9 years of age, and seven (78%) of the nine patients were male. Symptoms included fever (67%), headache (67%), eye swelling (56%), and seizure (33%). Rhinorrhea was uncommon (22%). Only two patients (22%) had had pre

Temporal bone cholesteatoma.

Clinical features of temporal bone cholesteatoma are miscellaneous, and sometimes misleading: signs of middle ear cholesteatoma, progressive or sudden facial palsy, sensorineural deafness as in acoustic neuroma, conductive deafness as in otosclerosis, secretory otitis media, or intracranial complications. Polytomography is the only way to pinpoint topography and extension. CT scanning is very useful in determining extension. Topography and severity of deafness are the guidelines for surgical approach. Among sixteen cases, total deafness was produced by the lesion itself in ten cases, and six h

Intracranial complications of mucormycosis: an experimental model and clinical review.

The clinical course of patients with mucormycosis of the paranasal sinuses can be unpredictable and is often determined by intrinsic host factors. The degree to and mechanism(s) by which these factors influence a patient's ability to survive the disease are poorly understood. Extensions to orbital and intracranial structures occur in some patients with paranasal sinus mucormycosis despite aggressive treatment. Controversies persist over adequate antifungal regimen, the precise role of hyperbaric oxygenation, and the appropriate extent of surgical debridement. We have developed an alloxan-induc

[Diagnostic possibilities and prospects for the use of thermovision in neurosurgical clinical practice].

On the basis of examination of 130 patients, the authors describe for the first time the thermovision syndromes of cerebral tumors, intracranial hematomas, posttraumatic cranial defects, tumors of the spinal cord, Schmorl' body, injuries to the peripheral nerves, and some other neurosurgical pathologic conditions. The mechanisms of the origin of thermoasymmetry are discussed. The diagnostic possibilities and the prospects of thermovision in neurosurgery are disclosed.

[Difficulties of diagnostic and therapeutic policy in association of purulent intracranial processes with inflammatory ENT diseases].

The paper analyses treatment given to 125 patients with purulent intracranial processes combining with inflammatory diseases of the paranasal sinuses, ear and other diseases. Otorhinogenic genesis was diagnosed in 45.6% cases. Often, affections of the paranasal sinuses and ear mask the true reason of the intracranial pathology, making problems for diagnosis and choice of therapeutic policy. Differential-diagnostic features of the diseases which may complicate with purulent intracranial processes as well as urgent therapeutic measures are described.

JAM3 methylation status as a biomarker for diagnosis of preneoplastic and neoplastic lesions of the cervix.

BACKGROUND: Brain abscesses (BA) are a medical emergency in all age groups and require early diagnosis and treatment. Childhood BA constitute 25% of all abscesses and may have a different clinical course than adults. This article aims to investigate patients followed up for adult and child BA over a 10-year period. MATERIALS AND METHODS: A retrospective analysis was performed on adult and pediatric patients diagnosed with BA. Demographic, clinical, predisposing factors, imaging, treatment modalities, and prognosis of the patients, and clinical differences were examined. RESULTS: Of a total o…
