Outcome after maternal varicella infection in the first 20 weeks of pregnancy.

BACKGROUND: Infection with the varicella-zoster virus during pregnancy can produce an embryopathy characterized by limb hypoplasia, eye and brain damage, and skin lesions. The risk is greatest when infection occurs during the first 20 weeks of pregnancy, but the magnitude of the risk is uncertain. METHODS: We studied 106 women with clinically diagnosed varicella infection in the first 20 weeks of pregnancy and compared the outcomes with those in 106 age-matched, nonexposed controls. RESULTS: Among the women with varicella, there was a trend toward more elective terminations of pregnancy (14

Viral reactivation as a cause of unexplained fever in patients with progressive metastatic breast cancer.

Patients suffering from metastatic breast cancer and recurrent fever were investigated for viral reactivation or new viral infection as a possible cause of these febrile episodes. Three groups of patients were included in the study: (a) patients under adjuvant chemotherapy with cyclophosphamide, methotrexate and fluoruracil, (b) patients with stable metastatic disease treated with cyclophosphamide, fluoruracil and Adriamycin or mitoxantrone and (c) patients with progressive metastatic disease who also received the latter treatment. During the time of observation, patients under adjuvant chemot

Infection of the fetus and the newborn: prevention, treatment and related handicap.

Congenital infection is uncommon and the cause of only a small proportion of handicap seen in children but some infections may be preventable or even treatable. As an example, the congenital rubella syndrome first described in the 1940s is preventable by use of the vaccine but cases still occur. It is hoped that with the introduction of the measles, mumps, rubella immunization for young children, rubella will become as rare in the UK as it is in the USA. Cytomegalovirus is now a more common cause of handicap than rubella but no vaccine has been developed. Although antiviral drugs are available

Viral infections in childhood leukemia.

The importance of viral infections in 150 children receiving therapy for leukemia was studied prospectively by application of comprehensive viral diagnostic procedures. One hundred five viral infections were identified, with herpes simplex virus and varicella-zoster virus being the most common agents. The spectrum of viruses associated with serious illness was wider than that in previous studies, and adenoviruses, parainfluenza viruses, rhinoviruses, and enteroviruses were important causes of morbidity. Viral isolation was the most sensitive diagnostic procedure used because complement fixatio

Deaths from varicella in infants.

Onset of maternal varicella up to 5 days before delivery is associated with in utero exposure to varicella and may result in severe infection in the newborn 5 to 10 days after delivery. Since up to 31% of these newborns may die, routine administration of varicella-zoster immunoglobulin to these infants is recommended. Little is known, however, about the risk of death in other infants with postnatal infection. Available epidemiologic data indicate an estimated death/case ratio for children less than 1 year of age 4 times that for 1- to 14-year-olds (8 in 100,000 vs. 2 in 100,000). Since the act

Preparation of varicella-herpes zoster immunoglobulin.

The efficacy of zoster immunoglobulin in prophylaxis of varicella and herpes zoster is reported by many authors. The screening for zoster's antibodies is performed in the blood donor population and in persons convalescing from herpes zoster. The results show that 25 per cent of blood donors and 83.5 per cent of convalescents, respectively, have CF antibody titer of l6. Varicella zoster immunoglobulin prepared by alcohol fractionation has a fluorescent antibody titer of 1,024 and a CF antibody titer of 512. These results are discussed and compared with those of the literature.

Acute facial palsy. Some clinical and virological observations.

A prospective clinical and virological study on 44 patients with acute, peripheral facial paralysis was carried out in consecutive cases during one year. In 9 cases varicella-zoster infections were serologically established. In 5 additional patients an associated varicella-zoster, or herpes simplex, infection was possible. Of the 9 confirmed cases, 6 were clinically diagnosed as zoster oticus, whereas on clinical grounds, 3 were regarded as Bell's palsy. No evidence was obtained of associated enterovirus, mumps, measles, cytomegalovirus, tick-borne encephalitis virus, para-influenza virus, mon

Nonbacterial pus-forming diseases of the skin.

The formation of pus as a result of an inflammatory response to a bacterial infection is well known. Not so well appreciated, however, is the fact that many other nonbacterial agents such as certain fungi, viruses and parasites may provoke pus formation in the skin. Also heat, topical applications, systemically administered drugs and some injected materials can do likewise. Numerous skin diseases of unknown etiology such as pustular acne vulgaris, pustular psoriasis and pustular dermatitis herpetiformis can have bacteriologically sterile pustules. The importance of considering nonbacterial cau

Measles, mumps, rubella and varicella among service members and other beneficiaries of the Military Health System, 2019-2024.

Measles, mumps, rubella, and varicella (MMR/V) cases have decreased in the U.S. Military Health System (MHS) overall, but in recent years, increasing numbers of MMR/V outbreaks in the U.S. have led to a rise in reported cases among the civilian population. Data were queried from the Defense Medical Surveillance System to identify total number of confirmed and possible MMR/V cases among all MHS beneficiaries from 2019 through 2024. The total numbers of confirmed and possible cases among MHS beneficiaries included 8 confirmed and 71 possible cases of measles, 18 confirmed and 193 possible cases

Vaccination coverage and influencing factors of four non‑National Immunization Program vaccines: A cross‑sectional survey among children aged 1‒6 years in Beijing, China.

The pneumococcal conjugate vaccine (PCV), Haemophilus influenzae type b (Hib) vaccine, rotavirus vaccine (RV), and varicella vaccine (VarV) have been recommended by WHO for inclusion in National Immunization Program (NIP). However, their vaccination rate is low in China as self-funded vaccines. This study aimed to investigate the coverage of these four vaccines and to identify their influencing factors. Selected via multi-stage stratified sampling, 4,182 guardians participated in a February-March 2025 study. Guardians completed an online questionnaire capturing sociodemographic characteristics

Live Attenuated Measles-Mumps-Rubella and Varicella Vaccinations and Multiple Sclerosis Activity.

IMPORTANCE: Live attenuated measles-mumps-rubella (MMR) and varicella vaccines are recommended for immunocompetent people with multiple sclerosis (MS) lacking immunity. However, concerns about postvaccination relapses may contribute to vaccine hesitancy, and robust data addressing this risk remain limited. OBJECTIVE: To assess whether the risk of MS relapse within 1 year after MMR or varicella vaccination is not unacceptably worse than risk after no such vaccination based on a predefined noninferiority margin. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used prospectively collected

High Prevalence of Varicella Zoster Virus Infection among Persons with Suspect Mpox Cases during an Mpox Outbreak in Kenya, 2024.

Kenya is experiencing a clade Ib mpox outbreak, with 28 confirmed cases reported by the Ministry of Health from July to December 2024. Polymerase chain reaction testing was performed for varicella zoster virus (VZV). Testing was also part of the differential diagnosis for mpox, and a subset of cases was genetically sequenced. Of 277 specimens tested for mpox, 170 (61%) tested positive for VZV, including instances of coinfection with both viruses in four cases. Genome analysis was used to identify clade 5 human alphaherpesvirus 3 in eight of the genetically sequenced cases. Expanding laboratory
