Liver transplantation for the treatment of urea cycle disorders.

The principal goal of therapy when liver transplantation is used for the treatment of metabolic disease is to correct the metabolic error. By doing so, liver transplantation eliminates the hepatic and peripheral consequences of the error. Inborn errors involving the urea cycle appear on theoretical grounds to be amenable to treatment using liver transplantation and, indeed, published data demonstrate that this approach to therapy can be successful. The purpose of this study is to examine the outcome of liver transplantation done for the indication of urea cycle defects in a large group of pati

Potent and selective inhibition of human nitric oxide synthases. Selective inhibition of neuronal nitric oxide synthase by S-methyl-L-thiocitrulline and S-ethyl-L-thiocitrulline.

Potent and selective inhibition of neuronal nitric oxide synthase (nNOS) compared to endothelial NOS (eNOS) and inducible NOS (iNOS) may be useful to treat cerebral ischemia (stroke) and other neurodegenerative diseases. S-Methyl-L-thiocitrulline (Me-TC) and S-ethyl-L-thiocitrulline (Et-TC) inhibited the oxidation of L-arginine and the L-arginine-independent oxidation of NADPH by nNOS from human brain. Me-TC and Et-TC were slow, tight binding inhibitors of nNOS with second-order association rate constants (kon) of 2.6 x 10(5) M-1 s-1 and 1.3 x 10(5) M-1 s-1, respectively. The respective dissoc

Chlorpromazine inhibits both the constitutive nitric oxide synthase and the induction of nitric oxide synthase after LPS challenge.

The effects of chlorpromazine on either the activity of mouse brain nitric oxide synthase or the induction of lung nitric oxide synthase in mice and rats were studied. Chlorpromazine inhibited the nitric oxide synthase activity in mouse brain cytosol. This effect could be reversed by adding an excess of calmodulin. In addition, chlorpromazine was able to inhibit the induction of lung nitric oxide synthase, in both species, after LPS administration. Furthermore, chlorpromazine also inhibited arginase activity in mouse lung cytosol.

The effect of occupational lead toxicity on testosterone secretion and the L-arginine nitric oxide pathway.

Endocrine-disrupting chemicals (EDCs), including heavy metals such as lead (Pb), interfere with hormonal homeostasis, particularly in the hypothalamic-pituitary-gonadal (HPG) axis. Occupational lead exposure is linked to male reproductive dysfunction and cardiovascular risk via oxidative stress and endothelial impairment. This study investigated the effects of chronic lead exposure on testosterone levels and the L-arginine-nitric oxide (NO) pathway, a key regulator of endothelial function. This case-control study compared 120 male workers with occupational lead exposure (in battery manufacturi

CSF2 drives radiation-induced lung injury by eliciting PADI4-high pathogenic neutrophils.

BACKGROUND: Radiation-induced lung injury (RILI) represents a severe, therapy-refractory complication of thoracic radiotherapy. Although neutrophils have been implicated in RILI pathogenesis, the specific pathogenic subsets and the molecular mechanisms governing their recruitment and functional specialization remain elusive. This study aims to decode the crosstalk between damaged epithelial cells and neutrophils in the RILI microenvironment. METHODS: We established a murine RILI model using 17 Gy thoracic irradiation and performed integrated single-cell RNA sequencing (scRNA-seq) and bulk tra

Development and evaluation of dried blood spot quality control materials in newborn screening for inborn errors of metabolism.

The application of tandem mass spectrometry (MS/MS) for detecting inborn errors of metabolism (IEM) is an important achievement in newborn screening (NBS). A single blood spot provides information on about 60 metabolites and related ratios that enable the screening of approximately 50 different diseases. A major limitation of newborn screening for IEM is the lack of matrix-matched dried blood spot (DBS) quality control (QC) materials in clinical laboratories. Herein, we developed DBS QC materials for IEM and investigated their recovery, homogeneity, and stability. Four concentration levels of

Bone metabolism and inflammation drive structural damage in rheumatoid arthritis: clustering and multivariable analysis.

BACKGROUND: Rheumatoid arthritis (RA) is characterized by synovial inflammation leading to joint damage, periarticular bone loss, and systemic osteoporosis. While inflammation is a primary driver of structural damage, dysregulation of the Wnt signaling pathway, particularly through inhibitors such as Dickkopf-1 (Dkk1) and sclerostin, has been implicated in RA-associated bone loss. OBJECTIVES: Our study investigated factors associated with erosive RA, focusing on bone turnover markers and modulators of the Wnt system. DESIGN: We performed a cross-sectional study of stable conventional synthet

The Role of IgM Anti-acetylated Protein Antibodies and B Cells in the Origin of Antimodified Protein Autoimmunity in Rheumatoid Arthritis.

OBJECTIVE: Rheumatoid arthritis (RA) is characterized by anti-modified protein antibodies (AMPAs), including anti-citrullinated protein antibodies (ACPA), anti-carbamylated protein antibodies (anti-CarP), and anti-acetylated protein antibodies (AAPA). In contrast to other AMPAs, AAPA IgM is found in healthy individuals, raising questions about its role in early immune responses. We investigated whether AAPA IgM serves as a precursor for other AMPAs, marking the initial breach of tolerance in RA. METHODS: AAPA IgM levels were measured in cord blood and serum of children up to three years old t

An interpretable metabolomic-driven machine learning model for early prediction of knee structural OA progression.

OBJECTIVES: Osteoarthritis (OA) is the most prevalent chronic musculoskeletal disease. Early identification of individuals at risk of knee structural progression is essential for targeted interventions. This study aimed to develop and validate a machine/deep learning (ML/DL)-based prognostic model for knee OA progression using serum metabolomics. METHODS: Baseline serum metabolomic factors from two independent cohorts were analysed: Tasmanian Older Adult Cohort (n = 180) for model development and Licofelone trial (n = 137) for external validation. Participants were categorized by likelihood o

A Curious Case of Emphysema and Fibrosis.

Rheumatoid arthritis (RA) is a systemic autoimmune disease with frequent extra-articular manifestations, of which pulmonary involvement is the most common and clinically significant. Combined pulmonary fibrosis and emphysema (CPFE) is a distinct clinic-radiological syndrome characterized by the coexistence of upper-lobe emphysema and lower-lobe fibrosis and is increasingly recognized in patients with connective tissue diseases, including RA. We describe the case of a 50-year-old nonsmoking woman who presented with a 3-year history of progressive exertional dyspnea and chronic dry cough, preced

Anlotinib ameliorates myositis-associated interstitial lung disease (MAILD) via suppression of the NETs-PI3K/Akt-driven epithelial-mesenchymal transition.

BACKGROUND: Myositis-associated interstitial lung disease (MAILD) is one of the most severe complications of idiopathic inflammatory myopathy, characterised by rapidly progressive pulmonary fibrosis and high mortality. Treatment options are limited, and mechanisms driving epithelial-mesenchymal transition (EMT) in MAILD are incompletely understood. Anlotinib, a multitarget tyrosine kinase inhibitor, shows potential in fibrotic diseases; however, its role and mechanism in MAILD need clarification. METHODS: To assess anlotinib's therapeutic effects, we established a MAILD mouse model and a neut

Cyclodextrin Inhibits Caveolin-1 to Regulate CD4+ T Cell Subset Differentiation and Ameliorate Rheumatoid Arthritis Progression.

OBJECTIVE: Rheumatoid arthritis (RA) is a systemic autoimmune disease characterized by dynamically recurring inflammatory episodes, and targeting T cells has been shown to effectively alleviate RA. Caveolin-1 (CAV-1) is widely distributed in the membrane structures of immune cells and participates in the differentiation and homeostasis of CD4+ T cell subsets through signal transduction; however, its role in RA has not yet been investigated. This study aimed to examine the expression pattern and clinical relevance of CAV-1 in RA, and to elucidate its regulatory role in CD4+ T cell-mediated immu

Does Anemia Independently Influence the Erythrocyte Sedimentation Rate in Rheumatoid Arthritis? A Multivariate Analysis From North India.

BACKGROUND: In regions such as North India, where anemia is prevalent, interpretation of the erythrocyte sedimentation rate (ESR) in rheumatoid arthritis (RA) may be challenging. We evaluated the independent associations of inflammation, autoantibody status, and anemia with ESR elevation and examined morphologic anemia patterns in this cohort. METHODS: We conducted a retrospective cross-sectional analysis of 152 RA patients attending a tertiary care center. Multivariate logistic regression identified independent predictors of elevated ESR and of anemia after adjustment for covariates. Seropos

Metabolic alterations associated with immune dysfunction in lysinuric protein intolerance.

Lysinuric protein intolerance (LPI) is a rare inherited metabolic disorder characterized by multisystem involvement, including failure to thrive, protein intolerance, hepatosplenomegaly, osteoporosis, pulmonary and renal disease, neurological impairment, and immune-dysregulation. Although immune-abnormalities are increasingly recognized in LPI, the immunological consequences of altered amino acid homeostasis remain incompletely understood. According to this, we aimed to characterize the immunologic-phenotype of patients with LPI. Patients with LPI and age-matched healthy controls were enrolled
