[Parenteral nutrition in treatment of short stature in adolescents with Crohn disease].

Growth retardation and delayed puberty occur in 20-35% of children and adolescents with Crohn's disease. Alternate day corticosteroid treatment, use of azathioprine, enteral or parenteral hyperalimentation and surgery have been advocated to reverse growth failure. Because of nonacceptance of elemental diet 7 patients with Crohn's disease and growth retardation received parenteral nutrition for 2-3 months (maximal for more than 30 months in one patient). All of them exhibited a mean weight gain of 10 kg and a mean increase of their height velocity from 2.4 to 7.1 cm/year. Main problems were bac

Peripheral nerve damage during limb lengthening. Neurophysiology in five cases of bilateral tibial lengthening.

Limb lengthening is used to correct leg length discrepancy and to increase stature. The reported frequency of peripheral nerve complications varies from 5% to 30%, but is probably underestimated. Damage may be direct or be caused by overstretching of the nerves. We have used electrophysiological tests to evaluate five patients during bilateral tibial lengthening by the Ilizarov method. Results after 24 to 107 days of lengthening showed electromyographic evidence of partial muscle denervation in all 10 limbs, with reduced motor conduction velocities in two tibial nerves and three common peronea

Unusual cause of short stature.

A 24-year-old man evaluated for paresthesias and short stature was found to be hypocalcemic on initial presentation. Further evaluation showed that he had a low-normal parathormone level by amino-terminal assay, medullary stenosis of the long bones, and multiple ophthalmologic abnormalities. The remainder of his pituitary function, including growth hormone response to insulin-induced hypoglycemia, was normal. As no family history of similar findings was evident, a sporadic case of Kenny's or Kenny-Caffey syndrome was diagnosed. He became normocalcemic in response to vitamin D and calcium carbo

[Growth in puberty].

During puberty growth velocity increases within 3 years from a prepubertal nadir to a maximum, in male to 9-10 cm/year, in female to 7-9 cm/year. This spurt is triggered by the androgenic steroids, in male mainly by testicular testosterone. In patients with anorchia the pubertal growth spurt is missing. The androgens have a dual effect. 1. As comes forth from observations in hypopituitary dwarfs being treated with constant replacement doses of hGH, these steroids potentiate the growth-stimulatory effect of hGH. In order to continue growing at all, these dwarfs need the additional medication of

Effect of pituitary hollow fiber units and thyroid supplementation on growth in the little mouse.

Hollow fiber units containing allogeneic pituitary cells were implanted intracranially into heterozygous (lit/+) and homozygous, mutant (lit/lit) C57BL/6J "little" weanling mice. Over the 48 days of the experiment, heterozygous mice with pituitary cell implants had a lower percentage weight gain than control mice. Homozygous, mutant mice with cell implants, however, made significant weight gains over mutant controls. Long bone lengths were lower, and organ and carcass weights were higher, in heterozygous mice receiving pituitary cell implants than in control mice, but corresponding measurement

Cognitive and emotional functioning in hypopituitary short-statured children.

Eleven children with documented growth hormone deficiency were studied to assess their cognitive and emotional functioning and their academic achievement before and after 1 year of human growth hormone replacement therapy. Standardized personality and intelligence measures were used, and records of school achievement, as well as developmental and family history from parents, were obtained. Although some subjects in this group were coping with problems other than their short stature (e.g., other medical and psychosocial problems), their intelligence and academic achievement were found to be pos

[Hypothalamic endocrinopathy in histiocytosis X (author's transl)].

Histiocytosis X in a 2-year-old boy with mainly skull manifestations was treated with radiotherapy and cytostatic drugs. Diabetes insipidus and growth retardation followed. Secretory capacity of GH was variable, periods of normal and insufficient secretion alternating during the 15-year observation period; there was a satisfactory response to GH therapy for one year. Thyrotropin and ACTH deficiency were diagnosed 3 1/2, and again 9 years after termination of histiocytosis therapy. Since there was no relapse of histiocytosis, this hypothalamic endocrinopathy is interpreted as being a late seque

Zinc and copper deficiency, with particular reference to parenteral nutrition.

Trace mineral metabolism has reached a new importance and received a new stimulus with the development of total intravenous feeding. A syndrome of acute zinc deficiency has been described in patients receiving intravenous feeding with pure amino acid infusates. Although the rapid response to zinc therapy makes it tempting to assume that the syndrome we have recognized during total parenteral nutrition is one of pure zinc deficiency, it is very likely that this is an oversimplication. The subtle relationships between zinc and other metals, such as calcium, copper, cadmium, and selenium, need fu

[Results of the exercise (ergometer) test as a diagnostic screening procedure in children with short stature].

The so-called Exercise Test of Lacey and coworkers (1973) is based on the fact that a functioning pituitary can be stimulated by physical exercise. The test was performed in 28 children of short stature as an out-patient screening test. The results were satisfactory since a growth-hormone deficiency could be excluded in 70% of the cases. The convenience of this test, which entails the investigation of only two blood samples, is stressed in respect to paediatric practice.

A case of leprechaunism and an analysis of some clinical manifestations of this syndrome.

A case of Leprechaunismm is reported. The parents were not consanguineous. The patient had two female and one male sibling. The patient was female and one sister was normal but the brother was also affected by Leprechaunismm. Another sister was possibly affected by Leprechaunis. The laboratory and autopsy examination showed various abnormal results, but few correlation was noted between the findings of the investigations and the clinical manifestations. The abnormality of the carbohydrate metabolism in Leprechauism may be due to the delay of insulin-release from pancreatic islets, from the abn

[Technical notes and clinical use of radioimmunologic determination of plasma ACTH].

Particular aspects of plasma ACTH radioimmunoassay are examinated. The results obtained with this method in adrenocortical diseases (Addison's disease, Cushing's syndrome, congenital adrenal hyperplasia) are reported and clinical value in etiological diagnosis of Cushing's syndrome is outlined. Moreover the results obtained in other syndromes of endocrinologic interest (massive obesity, Turner's and Klinefelter's syndromes, Laurence-Moon-Biedl syndrome, true precocious puberty, primary hypothyroidism, hypopituitary dwarphism) are reported and discussed.
