Schwannoma in the parotid gland. Experience at our institute and review of the literature.

Schwannoma of the parotid gland is an uncommon tumor. When the facial nerve is involved by the tumor at the time of the operation, the surgical approach requires careful evaluation. The purpose of this study is to provide indications which can support the decision, taking into account the different clinical presentations of parotid schwannoma. In a retrospective study, eight cases of parotid schwannoma were collected between 1975 and 2006. In three cases the tumor affected the facial nerve itself and in the remaining ones it originated from intraparotid nonfacial peripheral nerves. When the fa

Acinic cell carcinoma of salivary gland with massive deposits of globular amyloid.

We present the case of a 23-year-old woman with a parotid gland tumor, the fine-needle aspiration biopsy smears of which showed epithelial cells with wide cytoplasm, isolated or arranged in micropapillary groups together with psammoma bodies. The surgical specimen contained a 5-cm tumor with the histologic features of an acinic cell carcinoma (ACC) with papillary areas. Notably, the cells of the tumor seemed to follow a sequence from large cells with rounded nuclei with open chromatin and prominent nucleoli to vacuolated cells with granular material, and finally to cells undergoing apoptosis.

Papillary cystadenocarcinomas of salivary glands with oncocytic epithelial lining: report of 2 cases.

Papillary cystadenocarcinoma involving the salivary glands is rare, and there are only about 13 case reports and a single large series of 57 cases in the literature. We report 2 cases of this rare entity with emphasis on the diagnostic histomorphological features. The cases are also distinctive, in that they have an epithelial lining, which is oncocytic throughout. One of our cases was detected in an 8-year-old male child, which is itself a rarity.

Comparison of sonographic appearance of normal and postradiotherapy parotid glands: a preliminary study.

This study was undertaken to evaluate and compare the grey-scale and Doppler sonographic features of postradiotherapy (RT) and normal parotid glands. A total of 10 patients with previous head and neck RT and with different degrees of xerostomia were included. Another 10 healthy subjects, who are age and gender-matched with the 10 patients, were also recruited. Grey-scale and Doppler ultrasound examinations of parotid glands were performed on both the patients and healthy subjects. The parotid glands were assessed for their size, echogenicity, echotexture, conspicuity of intraparotid ducts, blo

Basal cell adenoma versus pleomorphic adenoma of the parotid gland: CT findings.

OBJECTIVE: Basal cell adenoma is a rare benign epithelial tumor of the salivary gland. The objective of this study is to present the CT findings of parotid basal cell adenoma. We also compare CT findings of basal cell adenoma with those of pleomorphic adenoma, the most common parotid tumor, to determine whether any features on CT can help differentiate these two entities. CONCLUSION: Basal cell adenomas of the parotid gland are located chiefly in the superficial lobe. They are generally round, well-circumscribed tumors that show heterogeneous enhancement on CT. The age of the patient and the

Helical tomotherapy for parotid gland tumors.

PURPOSE: To investigate helical tomotherapy (HT) intensity-modulated radiotherapy (IMRT) as a postoperative treatment for parotid gland tumors. METHODS AND MATERIALS: Helical tomotherapy plans were developed for 4 patients previously treated with segmental multileaf collimator (SMLC) IMRT. A primary planning target volume (PTV64) and two secondary PTVs (PTV60, PTV54) were defined. The clinical goals from the SMLC plans were applied as closely as possible to the HT planning. The SMLC plans included bolus, whereas HT plans did not. RESULTS: In general, the HT plans showed better target coverag

Sonographically guided core biopsy of a parotid mass.

OBJECTIVE: The purpose of this study was to evaluate the accuracy of sonographically guided core biopsy in the evaluation of parotid masses. SUBJECTS AND METHODS: Between 1998 and 2004, 135 patients consecutively presenting with a parotid mass were prospectively enrolled into this study. A single operator performed initial diagnostic sonography and then sonographically guided core biopsy using local anesthesia. Biopsy was performed with an 18- or 20-gauge needle and a spring-loaded biopsy gun with a mean of two passes per patient. Outcome measures were accuracy, sensitivity, specificity, and

Epstein-Barr virus associated lymphoepithelial carcinoma of the parotid gland; a clinicopathological report of three cases.

Lymphoepithelial carcinoma is a relatively uncommon malignant tumor of the salivary gland demonstrating malignant epithelial cells with dense lymphoid stroma. The authors report three cases of lymphoepithelial carcinoma associated with Epstein-Barr virus of the right parotid gland with clinically presenting as painless, gradual enlargement of the preauricular mass. The histopathologic examination of the parotid gland is characterized by malignant epithelial cells with dense lymphoid stroma. Immunohistochemical stains show positive reactivity to cytokeratin and p53 in malignant epithelial cells

Changing clinical manifestations of a T-peripheral lymphoma: from hypereosinophilic syndrome to questionable Kimura's disease resulting in parotid mass.

The diagnosis of low-grade lymphoproliferative disorders during a long clinical course sometimes represents a great clinical challenge. The idiopathic hypereosinophilic syndrome (HES) represents a pre-malignant state in some patients and close follow-up is necessary to detect early signs of malignant transformation. Kimura's disease (KD) mimicking HES is an immune mediated inflammatory disorder that usually involves the head and neck region, primarily affecting the salivary glands, adjacent muscle and regional lymph nodes. Clinically, it is very difficult to differentiate KD from salivary glan

Gene expression screening of salivary gland neoplasms: molecular markers of potential histogenetic and clinical significance.

Salivary gland neoplasms comprise phenotypically and biologically diverse lesions of uncertain histogenesis. The molecular events associated with their development and clinicopathological heterogeneity remain unknown. To reveal these events, we performed microarray expression analysis using a nylon-filter membrane platform on 18 primary lesions representing the most common benign and malignant types. Our study identified a small set of genes that are differentially altered between normal salivary gland tissues and benign and malignant tumors. Of the 5000 genes arrayed, 136 genes were different

First bite syndrome: a complication of surgery involving the parapharyngeal space.

INTRODUCTION: First bite syndrome (FBS) is the development of pain in the parotid region after the first bite of each meal and can be seen after surgery of the parapharyngeal space. The cause is not clear but has been proposed to involve a loss of sympathetic nerve function to the parotid, causing a denervation supersensitivity of salivary gland myoepithelial cells. The purpose of this study was to review the records of 12 patients with FBS to determine any common features of the operations performed that would support this theory of parotid "sympathectomy" as an etiologic factor of FBS. Vascularized medial sural cutaneous nerve based on the superficial sural artery: a reliable nerve graft.

The sural nerve is nourished by the single superficial sural artery proximally, but distally receives multiple contributions from musculocutaneous and fasciocutaneous perforators of the posterior tibial and peroneal (fibular) arteries. The prevalence of this proximal arterial supply is largely unknown. The authors report a large anatomic study (n = 56), together with two clinical cases, to assess the incidence and potential suitability of the sural nerve as a vascularized nerve graft. Dissections were performed on 6 fresh cadavers injected with Microfil dye and on 22 preserved cadavers. The su

A simple reconstructive procedure for radiation-induced necrosis of the external auditory canal.

Localized necrosis of the bone, cartilage, and soft tissue of the external auditory canal is an uncommon side effect of radiotherapy to the parotid region. Five patients developed late onset skin necrosis of a quadrant of the ear canal secondary to an underlying osteoradionecrosis of the tympanic ring. We report a one-stage procedure to excise the necrotic tissue and replace it with a local rotational flap derived from the post-auricular skin. Otological side effects of radiotherapy are discussed.

A prospective longitudinal study on radiation-induced hearing loss.

Recent clinical reports indicate that patients receiving radiotherapy that includes the auditory system in the treatment volume are likely to develop an irradiation-induced hearing loss. Although the early, presumed reversible, conductive hearing impairment due to secretory otitis media following radiotherapy is a sequela well known by radiation oncologists, permanent hearing loss, both conductive and sensorineural, is believed to be rare. A prospective study was performed enrolling patients receiving postoperative radiotherapy for unilateral parotid tumors. Audiometric results prior to irradi

Histology of the major salivary glands.

Major salivary glands are characterized by the presence of numerous excretory units that consist of acini and a peculiar duct system. Under normal conditions, sebaceous glands are annexed to the duct system in the parotid and the submaxillary glands. The excretion of the saliva is assisted by the myoepithelial cells, which play an important role in the morphology of several kinds of salivary gland tumors. Interstitial lymphocytes give rise to the formation of lymph nodes in the parotid gland. Heterotopic salivary tissue is found in many locations throughout the head and neck. Age-induced varia

[Measurement by means of 99mTc-pertechnetate of the function of salivary glands before and after stimulation with pilocarpine in cases of sicca-syndrome of the parotidic glands (author's transl)].

Judgement on xerostomia in systemic diseases of the salivary glands and on the sicca-syndrome as a side-effect of radiation therapy or of a treatment with psychopharmaca has been improved by the function test of the parotidic glands for each side individually with 99mTc-pertechnetate, particularly in view of the detection of lateral differences. Measurements of the salivation volume and activity are completive of the function test, yielding knowledge of the total excretory power of all the salivary glands. Functional remainders and reserves can be visualized objectively by additional utilizati

Stereotactic radiosurgery for recurrent pleomorphic adenoma invading the skull base--case report--.

A 38-year-old man presented with a recurrent pleomorphic adenoma in the parapharyngeal space invading the skull base 19 years after the first operation for a parotid gland tumor. Stereotactic radiotherapy was performed to control the tumor growth using a marginal dose of 8 Gy and maximum dose of 18 Gy with care taken to minimize the dose to nearby structures. The symptoms were reduced within a few months. Magnetic resonance imaging over 5 years showed that the tumor was controlled with no regrowth. Stereotactic radiotherapy is a therapeutic option for the treatment of pleomorphic adenomas.
