OBJECTIVE: Tumours in the central nervous system are the second most common malignant diseases in children. With improved treatment, the number of survivors is increasing. Therefore, better knowledge of the long-term effects of the disease and the therapy is needed. The aim of the current study was to find the incidence of central nervous system tumours in Iceland, evaluate the treatment results and study the long-term effects on the individuals.
MATERIAL AND METHODS: Data on diagnosis and treatment as well as demographic data were gathered from hospital records from the Reykjavik City Hospit

BACKGROUND: In low-resource settings, challenges in hospitalisation stay for sick young infants younger than 2 months persist. Early discharge of young infants with moderate-mortality-risk possible serious bacterial infection (PSBI) signs might provide a safe and effective alternative. We compared the clinical outcomes of switching parenteral antibiotics to oral antibiotics along with hospital discharge after 48 h of admission with those who continued hospitalisation for 7 days.
METHODS: An open-label, multicountry, multicentre, individually randomised controlled trial was done in Bangladesh,

BACKGROUND: Enuresis (bedwetting) affects up to 20% of five-year-old children and 2% of adults. Although spontaneous remission often occurs, the social, emotional and psychological costs can be great. Desmopressin has been used to treat bedwetting since the 1970s. This is an update of a Cochrane review first published in 2000 and last updated in 2006.
OBJECTIVES: To assess the effects of desmopressin on treating nocturnal enuresis in children.
SEARCH METHODS: We searched Cochrane Incontinence Specialised Register, which contains trials identified from the Cochrane Central Register of Control

RATIONALE: Localized tetanus is a rare subtype characterized by muscle spasms in the vicinity of necrotic wounds and is often misdiagnosed due to overlapping symptoms. Tetanus has a high mortality rate and is tricky to diagnose and treat clinically. Our case report is about a 61-year-old male patient who was admitted to a local hospital with an open fracture of the right foot and underwent emergency debridement of the open fracture site, internal fixation, and closed negative pressure drainage of the wound, and developed a localized Mycobacterium tetani infection in the postoperative period. BACKGROUND: Cilengitide is a selective αvβ3 and αvβ5 integrin inhibitor. Data from phase 2 trials suggest that it has antitumour activity as a single agent in recurrent glioblastoma and in combination with standard temozolomide chemoradiotherapy in newly diagnosed glioblastoma (particularly in tumours with methylated MGMT promoter). We aimed to assess cilengitide combined with temozolomide chemoradiotherapy in patients with newly diagnosed glioblastoma with methylated MGMT promoter.
METHODS: In this multicentre, open-label, phase 3 study, we investigated the efficacy of cilengitide in patient

PURPOSE: Febrile seizures (FS) are the most common type of convulsive events in children. FS are suggested to result from a combination of genetic and environmental factors. However, the pathophysiologic mechanisms underlying FS remain unclear. Using an animal model of experimental FS, it was demonstrated that hyperthermia causes respiratory alkalosis with consequent brain alkalosis and seizures. Here we examine the acid-base status of children who were admitted to the hospital for FS. Children who were admitted because of gastroenteritis (GE), a condition known to promote acidosis, were exami

Most patients with a pregnancy-induced hypertensive disorder have no clinical symptoms. So it can only be reliably detected by repetitive searches (screening) for the early signs and symptoms in the 2nd half of pregnancy. Adequate and proper prenatal care is the most important part of management of preeclampsia. Maternal antenatal monitoring includes identifying women at increased risk, early detection of preeclampsia by recognizing clinical signs and symptoms, and to observe progression of the condition to the severe state. As the etiology of preeclampsia remains in question, the only effecti

ETHNOPHARMACOLOGICAL RELEVANCE: Fever is a prevalent clinical symptom and is usually caused by inflammation or infection. Persistent high fever can lead to delirium, coma and convulsions, causing brain damage. Angong Niuhuang Pill (ANP), a traditional Chinese emergency medicine, has been employed in clinical practice for centuries, with well-documented antipyretic effects. However, the molecular mechanisms underlying its temperature-regulating effects remain to be fully elucidated.
AIM OF THE STUDY: This research aimed to analyze the effective components of ANP and examine its therapeutic eff

Takotsubo cardiomyopathy associated with alcohol withdrawal is rare in Japan, and its management in such cases using percutaneous left ventricular assist devices (Impella; Abiomed, Japan) is not common. This report describes the treatment of a patient with cardiogenic shock resulting from alcohol withdrawal-induced Takotsubo cardiomyopathy using Impella support. A female patient in her 60s with a history of alcohol dependence presented to the emergency room with fever and convulsions. Upon arrival, she developed status epilepticus requiring intubation and mechanical ventilation. Subsequently,

Epilepsy is a disease which affects between 1 and 2% of the population, and a large proportion of these people do not react to currently available anticonvulsant medications, indicating the need for further research into novel pharmacological therapies. Numerous studies have demonstrated that oxidative stress and inflammation occur during epilepsy and may contribute to its development and progression, indicating higher levels of oxidative and inflammatory parameters in experimental models and clinical patients. This research aimed to assess the impact of diclofenac sodium, a nonsteroidal anti-

BACKGROUND: Almost a third of patients with epilepsy have a treatment-resistant form, which is associated with severe morbidity and increased mortality. Cannabis-based treatments for epilepsy have generated much interest, but scientific data are scarce. We aimed to establish whether addition of cannabidiol to existing anti-epileptic regimens would be safe, tolerated, and efficacious in children and young adults with treatment-resistant epilepsy.
METHODS: In this open-label trial, patients (aged 1-30 years) with severe, intractable, childhood-onset, treatment-resistant epilepsy, who were recei

Urea Cycle Disorders (UCD) is an inborn error of urea synthesis in which ammonium and other nitrogenous precursors of urea accumulate leading to episodic coma and a high mortality rate. Therapy with peritoneal dialysis, essential amino acids or their nitrogen-free analogues has increased survival. The authors report 5 cases of urea cycle disorders, all of whom developed and were rescued from hyperammonemic coma. However, the eventual outcome was quite variable. BACKGROUND: Mood disorders are common, disabling and tend to be recurrent. They carry a high risk of suicide. Maintenance treatment, aimed at the prevention of relapse, is therefore of vital importance. Lithium has been used for some years as the mainstay of maintenance treatment in bipolar affective disorder, and to a lesser extent in unipolar disorder. However, the efficacy and effectiveness of prophylactic lithium therapy has been disputed. Low suicide rates in lithium-treated patients have led to claims that lithium has a specific anti-suicidal effect. If so, this is of considerable import

OBJECTIVE Traumatic brain injury (TBI) is a leading cause of death and severe morbidity for otherwise healthy full-term infants around the world. Currently, the primary treatment for infant TBI is supportive, as no targeted therapies exist to actively promote recovery. The developing infant brain, in particular, has a unique response to injury and the potential for repair, both of which vary with maturation. Targeted interventions and objective measures of therapeutic efficacy are needed in this special population. The authors hypothesized that MRI and serum biomarkers can be used to quantify

INTRODUCTION: Ondansetron is more effective than a placebo in treating postoperative nausea and vomiting (PONV), but it has not been proved to be superior to established antiemetics for prophylaxis or therapy. We compared ondansetron vs droperidol for the treatment of PONV.
METHODS: Our prospective, randomized double-blind study was performed between 15 October 1992 and July 1993; it included 271 gynaecological ASA I-III inpatients who had been operated on under general anaesthesia with intubation. Patients were excluded if: there was no informed consent; it was an ambulatory or emergency ope

OBJECTIVE: Wernicke's encephalopathy (WE) is an acute neuropsychiatric syndrome resulting from thiamine deficiency, which is associated with significant morbidity and mortality. The disorder is still greatly underdiagnosed in children because of either a relatively non-specific clinical presentation in some cases or unrecognized clinical setting. The aim of this literature review was to provide knowledge of pediatric WE in an effort to assist in early diagnosis, thereby reducing the morbidity and mortality.
METHODS: The clinical manifestations, characteristic magnetic resonance imaging (MRI),
