Not a firearm: A case report of a fatal penetrating pseudo-ballistic neck injury from defective airbag deployment.

Fatal injuries caused by defective airbag inflators are increasingly recognized as a distinct forensic and medicolegal issue. We describe the case of a 51-year-old male driver who was transported by emergency medical services following a low-speed frontal collision with a palm tree and was declared dead on arrival at the hospital. Review of surveillance footage showed that the driver abruptly swerved in an attempt to avoid a motorcyclist immediately before impact. Emergency services were alerted by witnesses, and police investigation documented limited frontal damage localized to the front bum

The complex dynamics of maxillofacial trauma in blast and fragmentation injuries: Current understanding and gaps in scientific evidence.

Recent epidemiological data demonstrate a significant rise in blast-induced maxillofacial trauma incidences in both military and civilian settings. This study aimed to comprehensively analyze maxillofacial trauma caused by heavy weaponry, examining blast dynamics, ballistic forces, and fragmentation effects. A comprehensive literature review was conducted using PubMed, SciSpace, and Google Scholar, incorporating studies on blast injuries, ballistic trauma, and war-related maxillofacial damage. Keywords included "maxillofacial injuries", "blast injuries", "ballistic trauma", "high-energy trauma

Free hemi-hamate arthroplasty: A review of donor site outcomes.

INTRODUCTION: The use of the dorsal hamate as a free osteochondral bone graft or vascularized bone flap has become the mainstay for large, comminuted middle phalanx volar lip fractures. To date, few studies have been conducted in the assessment of donor site morbidity for the hemi-hamate graft or flap, and none have discussed modes of repair or reconstruction of this donor site. METHODS: A retrospective analysis of 14 hemi-hamate arthroplasty (HHA) procedures, including 6 vascularized and 8 non-vascularized grafts, from two surgeons was performed. Four hamate defect reconstruction techniques

Immediate Proximal Row Carpectomy for Severe Perilunate Dislocation Injuries: A Minimum 5-Year Follow-Up.

Background: In patients with perilunate injuries (PLI) with multiple ligamentous and bony injuries involving the proximal carpal row, open reduction and internal fixation (ORIF) can be difficult and lead to poor functional outcomes. Proximal row carpectomy (PRC) is an alternative procedure that has been used for severely comminuted fractures. The aim of our study is to evaluate the long-term functional outcome (minimum 5 years) of patients that underwent an emergency PRC for PLI. Methods: We conducted a retrospective study of patients who underwent PRC at our centre between 2001 and 2016. Only

Pre-operative templating in total elbow arthroplasty: not useful.

PURPOSE: Total elbow arthroplasty (TEA) is a definitive surgical procedure for treating rheumatoid arthritis and (posttraumatic) osteoarthritis of the elbow and is also useful in comminuted elbow fractures. Pre-operative digital templating may theoretically improve the surgical implantation of TEA, but reliability and predictive values of templating are unknown. The aim of this study was to determine the intra- and inter-observer reliability and the validity of pre-operative digital templating for TEA. MATERIALS AND METHODS: All pre-operative plain anterior-posterior and lateral calibrated el

The Effect of Bridge Plate Duration on Range of Motion: A Breakpoint Modelling Approach for Distal Radius Fractures Treated With Dorsal Wrist Spanning Bridge Plate.

BACKGROUND: Dorsal wrist spanning plate (DWSP) fixation is a technique used to treat distal radius fractures with severe metaphyseal comminution, diaphyseal extension, and other complex fracture patterns. One concern of this technique is the potential risk of stiffness due to prolonged immobilization. The purpose of our study was to examine the association of DWSP duration on range of motion (ROM) outcomes after plate removal. METHODS: Patients who underwent DWSP fixation for distal radius fracture with greater than 6 months of follow-up after DWSP removal at a single institution were retrosp

Radiographic and functional outcomes after displaced intra-articular calcaneal fractures: a comparative cohort study among the traditional open technique (ORIF) and percutaneous surgical procedures (PS).

BACKGROUND: Open reduction with internal fixation (ORIF) and percutaneous surgery (PS) are the most common surgical procedures for the treatment of displaced intra-articular calcaneal fractures. The purpose of this retrospective study was to compare the clinical and radiological results of these techniques and to verify the prognostic value of the radiographic measurement tools proposed in the literature. METHODS: A consecutive series of 104 calcaneal fractures was included in this analysis. Essex-Lopresti and Sanders classifications were used to evaluate the injuries, and their prognostic co

Is arthroplasty preferable to internal fixation for the treatment of extracapsular fracture of the upper femur in the elderly?

INTRODUCTION: Although internal fixation is the reference treatment for extracapsular fracture of the upper femur, indications for arthroplasty are broadening, especially in unstable comminutive fracture in fragile bone. The present study hypothesis was that arthroplasty reduces early mortality and morbidity and provides better recovery of autonomy in over-80 year-old patients than does internal fixation. MATERIAL AND METHODS: A prospective multicenter study was conducted on 8 sites. Internal fixation was systematically used in 5 centers; arthroplasty was used systematically in 1 center, and

Thoracolumbar burst fractures associated with incomplete neurological deficit in patients under the age of 40: Is the posterior approach enough? Surgical treatment and results in a case series of 10 patients with a minimum follow-up of 2 years.

INTRODUCTION: Surgical management of thoracolumbar burst fractures is controversial. While the goals of surgical treatment are well accepted (i.e., fracture reduction and stabilization, neural elements decompression, and segmental angular deformity correction), the choice of the best surgical approach (i.e., posterior vs. anterior vs. combined approach) remains controversial. Several studies have debated the advantages of each surgical approach but there is no definitive evidence available to date, particularly in young adult patients. The aim of this study was to assess whether posterior appr

Immediate socket restoration after orbital trauma with globe loss: principles, timing, and our experience.

The literature-reported incidence of ophthalmic injuries occurring with facial fracture ranges widely from 0.8% to 30%. Ocular trauma necessitating enucleation or evisceration is less common, but it is not rare. The trauma and physical disability related to removal of the eye are extreme. Moreover, the loss of an eye causes severe changes to the anatomy and physiology of the orbit, resulting in deformities that affect the relationship between the socket and the prosthesis. Here, the authors present their own experience of 8 consecutive cases of trauma injuries with globe loss and emphasize the

Does fracture pattern predict death with lateral compression type 1 pelvic fractures?

BACKGROUND: Our goal was to analyze whether radiographic fracture pattern correlates with mortality of patients with lateral compression type 1 (LC1) fractures. METHODS: We conducted a retrospective case-controlled study at a Level I trauma center. Radiographs and outcome data were obtained for 52 patients with LC1 fractures who died and 63 who lived. LC1 fractures were classified by Denis zone of sacral injury and presence of fracture displacement. Our main outcome measurement was mortality during index hospital admission. RESULTS: No difference was observed in frequency of higher energy De

Dislocation of the elbow with fractures of the coronoid process and radial head.

INTRODUCTION: The aim of the study was to evaluate the factors relevant to prognosis after operative treatment of an elbow dislocation/fracture involving the coronoid process and the radial head. In 30-50% of cases, elbow dislocations are accompanied by concomitant bony injuries. Here, the ulnar coronoid process and the radial head are particularly crucial to the stability of the elbow joint. MATERIALS AND METHODS: In a retrospective study, 27 out of 37 patients who were treated surgically in our clinic between 1990 and 1999 for elbow dislocation with involvement of the coronoid process and t

Increasing complexity and plateauing outcomes in AO type C distal humerus fractures: a 50-year institutional case series.

BACKGROUND: Distal humerus fractures in adults are rare but complex injuries, particularly AO type C3 fractures, often involving comminution, instability, and osteoporotic bone. Demographic trends indicate rising incidence, especially among elderly patients, with increasing functional demands challenging modern osteosynthesis. Open reduction and internal fixation (ORIF) with bicolumnar plating remains the preferred reconstructive approach, although complications remain substantial. This study aimed to evaluate trends in fracture complexity, patient demographics, and functional outcomes over fi

Clinical and radiological results 6 years after treatment of traumatic thoracolumbar burst fractures with pedicle screw instrumentation and balloon assisted endplate reduction.

BACKGROUND CONTEXT: When used to fixate traumatic thoracolumbar burst fractures, pedicle screw constructs may fail in the presence of severe vertebral body comminution as the intervertebral disc can creep through the fractured endplates leading to insufficient anterior column support. Balloon-assisted endplate reduction (BAER) and subsequent calcium phosphate cement augmentation may prevent this event by restoring the disc space boundaries. The results of the first studies using BAER after pedicle screw fixation are encouraging, showing good fracture reduction, few complications, and minimal l

[Tibia nonunion after intramedullar nailing for fracture: decortication and osteosynthesis by medial plating].

PURPOSE OF THE STUDY: Intramedullar nailing is now widely used for fractures of the tibia. Extension of nailing indications to proximal, distal, and comminuted fractures has led to a significant rate of complications. The purpose of this study was to analyze decortication and medial plating for the treatment of tibial nonunion after intramedullary nailing. MATERIAL AND METHODS: Eighteen cases of aseptic tibial nonunion after nailing for fracture were treated in three women and fifteen men, mean age 39 years (19-57). The initial classification was open (n = 8), segmental (n = 3), single-focus

The medial collapse sign is associated with cubitus varus after pediatric supracondylar humeral fracture fixation: a retrospective cohort study.

BACKGROUND: Cubitus varus deformity is a common complication following pediatric supracondylar humeral fractures and rarely remodels spontaneously. However, surgeons lack a reliable intraoperative radiographic sign that directly reflects inadequate medial column reduction. This study aimed to evaluate whether the medial collapse sign (MCS) on immediate postoperative anteroposterior radiographs is associated with postoperative cubitus varus. METHODS: A retrospective cohort study included pediatric patients who underwent surgical treatment for supracondylar humeral fractures consecutively betwe

Reverse Total Shoulder Arthroplasty for Failed Osteosynthesis in Proximal Humerus Fractures: Outcomes and Challenges.

Background Proximal humerus fractures (PHF) are among the most common humeral fractures, particularly affecting elderly individuals, often following low-energy trauma. While conservative treatment is often preferred in geriatric patients, surgical management remains indicated for displaced fractures. Reverse total shoulder arthroplasty (RTSA) has increasingly been used as a primary treatment option for complex PHF, especially Neer three and four-part patterns. However, in the setting of failed osteosynthesis, there is a lack of evidence regarding the role and outcomes of RTSA. Methods A retros
