Access to fertility care for LGBTIQ+ and unpartnered persons: a systematic review protocol.

INTRODUCTION: Infertility is a major public health concern, and many individuals face substantial barriers to accessing fertility care. Across jurisdictions, regulations, eligibility criteria and funding frameworks for fertility care vary considerably, particularly in relation to lesbian, gay, bisexual, transgender, intersex and queer/questioning people (LGBTIQ+), as well as unpartnered individuals. Moreover, existing definitions and prevalence estimates of infertility are largely derived from studies of heterosexual couples which may underestimate the need for fertility care among other popul

Examining Site-Level and Structural Factors Influencing the Recruitment and Engagement of Participants of Color in Perinatal Substance Use Research: Insights From the Medication Treatment for Opioid Use Disorder in Expectant Mothers (MOMs) Trial.

BACKGROUND: Racial and ethnic inequities persist in perinatal opioid use disorder (OUD) care, with pregnant and postpartum people of color facing structural barriers to using medications to treat OUD (MOUD). Because access to clinical research is often mediated through treatment settings, these structural barriers may also shape who is reached, recruited, and retained in perinatal OUD trials. The Medication Treatment for OUD in Expectant Mothers trial (NCT03918850) provided an opportunity to assess how site-level and policy contexts influenced equitable recruitment, including gaps between proj

Attitudes of healthcare providers towards fertility preservation in Portugal: perceived support across indications and their ethical implications.

BACKGROUND: Healthcare providers (HCPs) play a pivotal role in shaping access to and ethical deliberation around fertility preservation. While fertility preservation due to oncological and non-oncological diseases is widely accepted, its use for other reasons, such as career development, gender transition, or military deployment, raises ethical and professional questions. This study aimed to explore the attitudes of HCPs in Portugal towards fertility preservation across different indications and to identify professional factors associated with variations in perceived support. METHODS: We cond

The Genetic and Epigenetic Architecture of Keratoconus: Emerging Pathways and Clinical Implications.

BACKGROUND: Keratoconus (KC) is a progressive corneal ectasia and a leading cause of corneal transplantation in young adults. Once regarded as a biomechanical disorder, KC is now recognized as a complex disease driven by genetic predisposition, epigenetic modulation, and environmental triggers. Advances in genomics and transcriptomics have begun to elucidate the molecular mechanisms underlying corneal thinning and ectasia. OBJECTIVES: This review synthesizes two decades of evidence on the genetic and epigenetic architecture of keratoconus, highlights key molecular pathways implicated by these

An investigation of the association between Juvenile rheumatic disease and growth: a retrospective matched cohort study.

Childhood-onset rheumatic diseases including Juvenile Idiopathic Arthritis (JIA), Juvenile Systemic Lupus Erythematosus (jSLE), and Juvenile Dermatomyositis (JDM) could impact growth, but this is not well described within the UK population, particularly over the long term. This study aims to compare the growth patterns of patients with JIA, jSLE, and JDM to those of their peers. This population-based cohort study used primary care data from the Clinical Practice Research Datalink (CPRD) with follow up through to March 2023. Patients diagnosed with JIA, JDM and jSLE, were matched with up to fiv

3DCT reconstruction-does 3DCT improve anatomical lung resection?-a narrative review of the literature.

BACKGROUND AND OBJECTIVE: Minimally invasive and sublobar anatomical lung resection has become the gold standard for stage I non-small cell lung cancer (NSCLC) under 2 cm. Preoperative surgical planning with three-dimensional computed tomography (3DCT) reconstruction has become more common to improve safety and accuracy of resection. The aim of this review is to evaluate the evidence for 3DCT reconstruction in improving perioperative outcomes in patients undergoing anatomical lung resection. METHODS: A targeted literature review of evidence for 3DCT reconstruction in the perioperative period,

Health care experiences of individuals accessing or undergoing in vitro fertilization (IVF) in the U.S.: a narrative review of qualitative studies.

BACKGROUND: In vitro fertilization (IVF) is an increasingly common method of assisted reproduction given the high rates of infertility in the United States (U.S.). However, despite growing utilization of IVF technologies, there is little known about the experiences of those accessing or undergoing IVF, particularly among adults in the U.S. The aims of this review are to (1) explore how economic, emotional and physical health, and interpersonal relationships impact and are impacted by accessing or undergoing IVF, and (2) understand the role of healthcare providers and the healthcare system. Automated Assessment of Word- and Sentence-Level Speech Intelligibility in Developmental Motor Speech Disorders: A Cross-Linguistic Investigation.

Background/Objectives: Accurate assessment of speech intelligibility is necessary for individuals with motor speech disorders. Transcription or scaled rating methods by naïve listeners are the most reliable tasks for these purposes; however, they are often resource-intensive and time-consuming within clinical contexts. Automatic speech recognition (ASR) systems, which transcribe speech into text, have been increasingly utilized for assessing speech intelligibility. This study investigates the feasibility of using an open-source ASR system to assess speech intelligibility in Hebrew and English

Determinants of exclusive breastfeeding in Tanzania: a systematic review and meta-analysis.

BACKGROUND: Exclusive breastfeeding (EBF) is a critical intervention for reducing childhood morbidity and mortality, yet global EBF rates remain below targets. In Tanzania, EBF rates vary widely across regions, with reported disparities influenced by individual, familial, and health system factors. Despite increasing national prevalence rates of EBF from 41% in 2005 to 64% in 2022, localized studies show lower prevalence ranging from 20 to 30%, highlighting the need for a comprehensive synthesis of evidence on the determinants of EBF to inform policy in Tanzania. METHODS: We searched PubMed, S

Evaluating the efficacy of lower-level transversus abdominis plane block for postoperative analgesia following lower-uterine-segment caesarean delivery: study protocol for an observer-blinded, randomised controlled trial at a tertiary hospital in China.

INTRODUCTION: Both transversus abdominis plane block (TAPB) and transversalis fascia plane block (TFPB) have been shown to provide effective analgesia for caesarean section (CS) pain. However, the targeting of distinct spinal nerve branches by each technique suggests that neither approach offers complete analgesia. Moreover, the targeting of these branches by TAPB is influenced by puncture level, which has seldom been clearly defined in previous studies. This study aims to evaluate the efficacy of lower-level TAPB (L-TAPB), performed at the midpoint between the umbilicus and the anterior super

Functional connectivity alterations in high-functioning preschool boys with autism spectrum disorder.

Information processing impairments are associated with the social and sensory symptoms of autism spectrum disorder (ASD). Functional connectivity forms the basis of information exchange and integration. Previous research has shown atypical functional connectivity patterns in school-aged children, adolescents, and adults with ASD. However, connectivity alterations in children with ASD in preschool years, a critical period for brain and cognition function development, are yet to be explored. Here, we compared resting-state EEG functional connectivity between high-functioning preschool boys with

The contribution of childhood trauma to irritability symptoms.

BACKGROUND: Irritability is a transdiagnostic psychiatric phenotype defined as an increased proneness to anger relative to peers. Trauma is defined as actual or threatened death, serious injury, or sexual violence while adversity more broadly describes difficult or challenging situations including abuse, neglect, and household dysfunction. Irritability [or aggression] is symptom of posttraumatic stress disorder (PTSD) and may arise in response to trauma or traumatic events. Responses to negative early life experiences may differ based on the type of exposure, for example, threat (abuse) versus
