Towards a Quantitative Understanding of Aficamten Clinical Pharmacology: Pharmacokinetic-Cardiodynamic Modeling to Support Safety and Efficacy.

In the phase 3 study SEQUOIA-HCM (NCT05186818), aficamten, a next-in-class cardiac myosin inhibitor, was safe and efficacious in participants with obstructive hypertrophic cardiomyopathy (oHCM). Using pharmacokinetics/pharmacodynamics (PKPD) modeling, we quantified the relationship between aficamten exposure and cardiodynamic measures of safety (left ventricular ejection fraction [LVEF]) and efficacy (post-Valsalva left ventricular outflow tract gradient [LVOT-G]), and used Clinical Trial Simulations (CTS) to predict cardiodynamics for a flexible dose regimen in a post-approval setting. Association of twinning and maternal age with major structural birth defects in Utah, 1999 to 2008.

BACKGROUND: Twinning is a risk factor for birth defects. Using population-based data from the Utah Birth Defect Network and the Utah Department of Health Vital Records, we investigated whether twinning increases the risk of major structural birth defects in live-born opposite-sex (OS) and same-sex (SS) twins as proxies for monozygotic and dizygotic twins, respectively. We also assessed whether maternal age modified the association between twinning and birth defects. METHODS: All resident live-born singleton (n = 492,143) and twin (n = 13,596) infants issued a Utah birth certificate and delive

Proximal pulmonary emboli modify right ventricular ejection pattern.

Analysis of the systolic flow velocity curve (SFVC) in the right ventricular outflow tract is considered as an alternative to the tricuspid valve pressure gradient (TVPG) method for echo-Doppler assessment of pulmonary arterial pressure (P(pa)). The present study checked whether or not SFVC is affected by the cause of pulmonary hypertension. Doppler recordings of 86 patients (39 female, aged 55.5+/-15.2 yrs) with acute (AP-PE) or chronic (CP-PE) proximal pulmonary embolism, chronic obstructive pulmonary disase (COPD) or primary pulmonary hypertension (PPH) were retrospectively analysed by two

Minimally invasive cardiac surgery for complex procedures: extensive septal myectomy and double valve replacement.

In this video tutorial, we demonstrate that minimally invasive cardiac surgery and all its benefits can be applied even to complex, multiple cardiac procedures. We present a 71-year-old patient with severely obstructive hypertrophic cardiomyopathy, moderate mitral regurgitation for systolic anterior motion of the mitral valve, moderate aortic stenosis and regurgitation and atrial fibrillation. We performed a mitroaortic valve replacement, transmitral and transaortic septal myectomy and left atrial appendage closure through a minimally invasive approach (right anterolateral minithoracotomy). Patient selection for transcatheter mitral valve implantation: why is it so hard to find patients?

Both transcatheter mitral valve repair and replacement have been introduced in patients with severe mitral regurgitation (MR). However, while transcatheter mitral valve repair is rapidly evolving, transcatheter mitral valve implantation (TMVI) has had a slow development path. One of the main reasons for this is the challenge to find good candidates for this therapy. Although scarce data exist for patient inclusion and/or exclusion criteria and patient screening, the current rejection rate for TMVI is reported to be around 60%. The rejection could be due to: 1) the restriction of the technique

Survival after alcohol septal ablation for obstructive hypertrophic cardiomyopathy.

BACKGROUND: The clinical efficacy of alcohol septal ablation for obstructive hypertrophic cardiomyopathy (HCM) has been demonstrated, but the long-term effects of the procedure remain uncertain. This study examined the survival of patients after septal ablation performed in a tertiary HCM referral center. METHODS AND RESULTS: We examined 177 patients (mean age, 64 years; 68% women) who underwent septal ablation at our institution. Over a follow-up of 5.7 years, survival free of all mortality was no different than the expected survival for a comparable general population, and similar to that o

Dobutamine stress echocardiography in hypertrophic cardiomyopathy.

AIMS: Myocardial ischemia in the absence of coronary artery disease is common in patients with hypertrophic cardiomyopathy (HCM). Dobutamine stress echocardiography (DSE) induces left ventricular (LV) new wall motion abnormalities (NWMA) in some patients with HCM. We evaluated the effects of dobutamine on LV performance and hemodynamics in HCM. METHODS AND RESULTS: Eighteen patients with non-obstructive HCM underwent DSE. Dobutamine was administered at dosages of 5, 10, 20, 30 and 40 microg/kg/min with increments at intervals of 3 min. Seven patients developed NWMA, whereas the other 11 did n

Spectrum and influence of hypoplasia of the left heart in neonatal aortic coarctation.

Obstruction of the left ventricular outflow tract may be associated with hypoplasia of the left heart, which importantly influences the options for treatment. Although the influence of the size of the left heart on the outcome for critical aortic stenosis has been described, less is known about the spectrum of such hypoplasia seen with neonatal aortic coarctation, and how this influences outcome. To determine, first, the spectrum and influence of hypoplasia of the left heart in neonatal coarctation, second, if the previously described critical values for adequacy of the left heart in neonates

Magnetic resonance imaging of congenital heart disease: sensitivity and specificity using receiver operating characteristic curve analysis.

MRI has shown promise for the evaluation of various congenital heart lesions. The current study was designed to assess the sensitivity and s … op (100%). Three of five anomalies of the pulmonary and systemic veins were diagnosed by MRI. Right ventricular outflow obstructions (95%) were detected with a much higher sensitivity than lesions of the other valves (aortic valve 52%, mitral valve 62%, tricuspid valve 76%). Spin-echo MRI is a sensitive and specific method for the noninvasive assessment of congenital heart disea …

The surgical anatomy of the left ventricular outflow tract in atrioventricular septal defect.

The left ventricular (LV) outflow tract (OT) in atrioventricular (AV) septal defect is an important structure that paradoxically is hardly ever seen by a surgeon. The LVOT is prone to develop obstruction following surgical procedures, such as left AV valve replacement, that seemingly do not affect the LVOT itself. We examined 15 hearts with AV septal defects and noted the anatomical boundaries of the LVOT. Additionally, the LVOT was examined microscopically, and it was sectioned to replicate echocardiographic images. A sham operation was performed to show the extent of the proposed resection f

The presentation of symptomatic heart disease in infancy based on 10 years' experience (1973-82). Implications for the provision of services.

The diagnoses and age at presentation of 1665 infants with symptomatic heart disease, who were admitted to the Brompton Hospital, London, during the period 1973-82 were reviewed. The frequency of certain conditions had changed during the period of the study. Complete transposition of the great arteries and critical aortic stenosis had become less common, whereas the frequency of right ventricular outflow tract obstruction, and of critical pulmonary stenosis in particular, had increased. Several conditions, including coarctation of the aorta, hearts with univentricular-atrioventricular connexio

Exercise performance after septal myotomy and myectomy in patients with obstructive hypertrophic cardiomyopathy.

The effect of left ventriculomyotomy and myectomy on exercise capacity and cardiac function in patients with obstructive hypertrophic cardiomyopathy has not previously been determined. In this study, 29 patients were evaluated during graded treadmill exercise before and after operation. Postoperatively, 27 of 29 patients reported symptomatic improvement and had greatly reduced left ventricular outflow gradient. Twenty-five of 28 patients (89 percent) attained higher exercise levels after operation, and this was accompanied by an increase in total body oxygen consumption from 16 to 21 ml/min pe

Case Report: Newly discovered ELN gene mutation in congenital heart disease: case analysis and review.

INTRODUCTION: Supravalvular aortic stenosis (SVAS) is a rare left ventricular outflow tract obstruction, most commonly caused by pathogenic variants in ELN. Truncating variants in exons 1-29 typically produce non-syndromic SVAS through elastin haploinsufficiency, whereas C-terminal variants are linked to autosomal dominant cutis laxa. However, clinically and mechanistically well-characterized variants in the distal part of this "stenotic zone," such as exon 28, remain uncommon. METHODS: We conducted a retrospective family-based case report with standardized clinical evaluation, serial echocar

Right ventricular function assessment by transesophageal echocardiography in lung transplantation: Preoperative, intraoperative, and postoperative considerations.

BACKGROUND: Right ventricular (RV) function is a key determinant of outcomes in lung transplantation, particularly in patients with pre-existing pulmonary hypertension. The RV is uniquely sensitive to perioperative hemodynamic stressors, including ischemia-reperfusion injury, abrupt afterload changes, and allograft implantation. PURPOSE: This review aims to summarize the perioperative echocardiographic assessment of RV function in lung transplantation, emphasizing the role of transesophageal echocardiography (TEE) in evaluating RV performance and guiding intraoperative management. METHODS: A

The AltaValve system for transcatheter mitral valve replacement: A narrative review of early safety and efficacy data.

Mitral regurgitation (MR) is a prevalent and progressive condition associated with poor outcomes in high-risk patients who are often ineligible for surgery or transcatheter repair. The AltaValve system (4C Medical Technologies, Maple Grove, MN), a novel transcatheter mitral valve replacement (TMVR) device, employs a unique supra-annular, atrial-only fixation mechanism designed to minimize left ventricular outflow tract (LVOT) obstruction and paravalvular leak. This narrative review summarizes current clinical evidence on AltaValve, including case reports, single-center, and multicenter experie

Predictors of successful pulmonary valve-sparing repair in pediatric humanitarian patients with Tetralogy of Fallot.

INTRODUCTION: The optimal surgical approach for right ventricular outflow tract obstruction in Tetralogy of Fallot aims to preserve the native pulmonary valve annulus, as this is associated with better long-term outcomes. Pediatric humanitarian patients often present with a delayed diagnosis and lack access to preoperative palliative treatments, reducing the likelihood of pulmonary valve annulus preservation and potentially compromising long-term outcomes. This study aims to identify independent predictors of successful pulmonary valve-sparing repair in pediatric humanitarian patients undergoi

Cerebral Hypoperfusion Caused by Brachiocephalic Artery Stenosis.

INTRODUCTION: Extracranial cerebrovascular disease can cause cerebral ischaemia through embolism or hypoperfusion. Managing cerebral ischaemia in patients with hypoperfusion and multivessel cerebrovascular disease can pose challenges owing to the risks of embolisation and haemodynamic instability, especially when normal embolisation protection techniques and cross clamping are hazardous. REPORT: This article presents the case of a 74 year old woman who experienced a peri-operative cardiac arrest during femoropopliteal bypass surgery, secondary to undiagnosed severe left ventricular hypertroph
