Trauma during pregnancy.

Trauma is the leading nonobstetrical cause of maternal death. The effect of trauma on the pregnant woman and unborn fetus can be devastating. The major causes of maternal injury are blunt trauma, penetrating trauma, burns, falls, and assaults. There are specific changes associated with pregnancy that are important for the clinician to consider when providing care to these patients. Initial management of traumatic injuries during pregnancy is essential for maternal and fetal well-being. This review outlines common causes of maternal trauma, the initial assessment of the pregnant trauma patient,

Active responses decrease impact forces at the hip and shoulder in falls to the side.

Active responses, such as using the arm to break the fall, may be an effective means of decreasing likelihood of injury in a fall and may help explain why only a small percentage of falls result in a fracture. We quantified the impact force at the hip and shoulder in falls to the side from a kneeling position under three conditions: (1) attempting to break the fall by using an arm; (2) falling with the body relaxed; and (3) falling with the body tensed. Subjects fell from a kneeling position onto a force platform array covered with foam padding and impact force data were recorded. The ground r

A longitudinal analysis of alcohol consumption and the risk of posttraumatic symptoms.

BACKGROUND: Previous studies investigating the impact of alcohol ingestion on the emergence of posttraumatic psychological symptoms have generated contradictory findings. METHODS: One thousand forty-five patients, admitted to hospital following traumatic injury were assessed during hospitalisation for patterns of alcohol consumption prior to the injury and also during the month prior to reassessment at 3 months. Anxiety, depression and posttraumatic stress disorder (PTSD) were assessed post accident and at 3 months. In a sub sample (n=167), blood alcohol levels were measured at the time of ad

Outcomes of indeterminate fractures identified on skeletal surveys obtained during non-accidental trauma evaluations.

BACKGROUND: Skeletal surveys (SS), a critical component of non-accidental trauma (NAT) evaluations, may have indeterminate findings concerning for a possible fracture. The frequency and clinical impact of these indeterminate fractures is unknown. OBJECTIVES: In children with SS obtained as part of NAT evaluations, 1) determine the prevalence and clinical characteristics of indeterminate fractures, 2) describe frequency and type of follow-up imaging, and 3) ascertain the potential impact of indeterminate fractures. PARTICIPANTS AND SETTING: A single-center, retrospective chart review of child

Infant, maternal and pregnancy associated risk factors for offspring fractures during infancy - A population-based study.

BACKGROUND: The etiology of fractures during infancy includes birth trauma, accidents, and uncommon medical conditions predisposing to bone fragility. A 2018 study from Sweden suggested several hypothesized metabolic bone disease (MBD) risk factors elevated maternal BMI, smoking, multiple gestation birth, and small-for-gestational age may predispose infants to fractures. OBJECTIVE: To evaluate the incidence, etiology, and associated infant and maternal factors for non-birth-related fractures during infancy. PARTICIPANTS AND SETTING: Infants born between 2009 and 2018 in Olmsted County, Minne

Supervision and guardianship during drowning fatalities among Canadian youth: An 11-year review of a preventable paediatric public health crisis.

Drowning is a leading cause of injury-related deaths among youth worldwide, often linked to inadequate supervision and ineffective guardianship. While paediatric studies have examined demographics and risk factors, little is known about how these variables interact to influence rescue likelihood. This study addresses that gap using a multivariate binary logistic regression model to assess the odds of not receiving a rescue attempt, using fatality records (n = 638) from coroners and medical examiners across Canada (2006-2016). This approach captures all known accidental fatal drownings but find

Effect of Air Exposure Time Under Room-Temperature Conditions on the Performance of Chemical Heat Blankets Intended for Use in Prehospital Accidental Hypothermia.

IntroductionAccidental hypothermia increases mortality in patients with traumatic injury, making hypothermia management essential in prehospital trauma care. Chemical heat blankets are commonly used for this purpose. These blankets require time to get warm, with agitation often used to accelerate the exothermic reaction. Many search and rescue teams use chemical heat blankets, but optimal activation of the blankets remains uncertain. This study investigated how varying oxygen exposure durations affect the thermal performance of these blankets to guide their optimal use in search and rescue sce

Sex differences in the acute effects of cannabis on human cognition: A systematic review.

Acute cannabis exposure can transiently impair cognitive performance, increasing the risk of accidental injury and potentially disrupting activities of daily living. Although sex differences in responses to cannabis have been reported, no systematic review has examined whether these extend to acute cognitive effects. Our primary aim was to examine sex differences in the acute effects of cannabis (including isolated delta-9-tetrahydrocannabinol [THC]) on cognition in humans. Our secondary aim was to determine if sex differences in the acute effects of cannabis vary by cognitive domain, route of

Clearing the airway? A pilot cadaveric study of the LifeVac™ device.

BACKGROUND: Foreign body aspiration (FBA) is a leading cause of accidental death, especially among children and older adults. Suction-based airway clearance devices, such as the LifeVac™, an FDA-registered Class II medical device, have garnered attention as potential adjunctive tools in FBA management; however, data on their efficacy remain limited. This study evaluated the performance of the LifeVac™ in a cadaveric model simulating complete upper airway obstruction by commonly aspirated food items. METHODS: Fresh cadavers with intact upper airway anatomy were used. Grapes, hot dog slices, an

Renal-clearable organic NIR-II dye cluster for non-invasive ureteral imaging.

Intraoperative ureter visualization is essential in colorectal cancer (CRC) surgery, but current modalities are limited by invasiveness, poor photostability, and insufficient spatial resolution. Second near-infrared (NIR-II) fluorescence imaging provides a non-invasive approach with improved tissue penetration and spatial resolution. By modulating the aggregation behavior of NIR-II dyes, intravenous administration followed by renal metabolism enables real-time, high-resolution ureteral visualization. In this study, we prepared a series of donor-acceptor-donor-based NIR-II probes through a one-

Hand Sanitizer: Stopping the Spread of Infection at a Cost.

The recent rise in hand sanitizer use due to the COVID-19 pandemic has had a beneficial impact on stopping the spread of disease, but the potential negative implications of its overuse on the body and the microbiome have yet to be thoroughly reviewed. Epidermal layers absorb hand sanitizer from direct application to the skin, making them some of the most susceptible cells to the adverse effects of overuse. The increased usage of hand sanitizer can affect the variation, quantity, and diversity of the skin microflora, leading to conditions such as eczema, atopic dermatitis, and even systemic tox

From suicide to accident - Case report - The significance of the medico-legal autopsies.

INTRODUCTION: Suicide is the intentional and voluntary act of destroying one's own life, while an accident is an unintended event that involuntarily causes injury to one's health or destruction of life. CASE PRESENTATION: We report a case of a 54-year-old male who was found in a forest in late November, approximately 500 m away from his car. He was positioned bent over a trunk of a tree with his head beneath the rest of his body. His pants were down to his knees, and there were soiled blades of grass and leaves on his body. Investigation of the case circumstances revealed that he had attempte

Skull Fracture Healing in Children Up to 36 Months - A Cohort Analysis.

During medicolegal proceedings in cases of suspected child abuse it is sometimes argued that skull fractures could be sequelae from complications at birth or resulted from a prior witnessed accidental trauma that may have preceded the suspected abusive event. There is paucity of scientific evidence indicating timing for skull fracture healing in children up to 36 months old. Objective of this study was to assess the average time to imaging documentation of skull fracture healing in children up to 36 months old. We performed retrospective chart review and image analysis in children with documen

Spinal neuroschistosomiasis caused by Schistoma mansoni: cases reported in two brothers.

BACKGROUND: Spinal neuroschistosomiasis (SN) is one of the most severe clinical presentations of schistosomiasis infection and an ectopic form of the disease caused by any species of Schistosoma. In Brazil, all cases of this clinical manifestation are related to Schistosoma mansoni, the only species present in the country. Although many cases have been reported in various endemic areas in Brazil, this is the first time in the literature that SN is described in two brothers. CASE PRESENTATION: Two cases of SN were accidentally diagnosed during an epidemiological survey in an urban area endemic

Accessing Medical Care After a Needlestick Injury: First Responders' Perception of HIV Risk and Attitudes Toward Syringe Service Programs.

First responders have an increased risk of occupational exposure to HIV as the result of a needlestick injury (NSI) because of the chaotic prehospital environment in which they provide care. Approximately 2.3 of every 1000 first responders (0.23%) who are exposed to HIV via a NSI risk seroconversion if left untreated. Participants completed a 28-question online survey examining level of concern about HIV, thoughts about injection drug use, number of accidental NSIs, and medical services received after a needlestick. First, all data were analyzed descriptively. Second, a multiple linear regress
