Minimal Margin Extracapsular Dissection: A Viable Alternative Technique for Benign Parotid Lesions?

OBJECTIVE: Extracapsular dissection (ECD) has become an accepted, less invasive alternative for the removal of select benign parotid lesions that may reduce complications. Minimal margin extracapsular dissection (MECD) with dissection on or closer to the tumor capsule may be a reasonable alternative to ECD. The objective of this study is to review the complications and safety of the MECD technique at a single institution. SUBJECTS AND METHODS: Medical records for patients who underwent MECD for suspected benign parotid lesions were reviewed. Outcome measurements included intraoperative findin

Parotid gland tumors: a retrospective study of 154 patients.

INTRODUCTION: Benign tumors of the parotid gland comprise the majority of salivary gland tumors. OBJECTIVE: To review the clinical characteristics of parotid gland tumors submitted to surgical treatment by the same surgeon. METHODS: Retrospective study with 154 patients who had parotid gland tumors. Clinical and histological data, type of surgery, and complications were assessed and described. RESULTS: The main manifestation was a mass with a median evolution of 12 months for benign tumors and five months for malignant tumors. Ultrasonography was the most frequent complementary exam. Pleomo

Patterns-of-failure after helical tomotherapy-based chemoradiotherapy for head and neck cancer: implications for CTV margin, elective nodal dose and bilateral parotid sparing.

OBJECTIVES: There is debate about the optimal clinical target volume (CTV) expansion and prophylactic nodal dose (PND) in head and neck IMRT. We evaluated our patterns-of-failure (POF) after helical tomotherapy-based concurrent chemoradiotherapy (CCRT) to assess the oncologic safety of reducing the CTV, PND, and bilateral parotid sparing (BPS). MATERIALS AND METHODS: All patients with locally advanced squamous cell carcinoma of the head and neck treated with curative intent CCRT between January 2007 and April 2013 at a single institution were included in this retrospective study. Locoregional

Kuttner tumor.

OBJECTIVE: Kuttner tumor or chronic sclerosing sialadenitis is a benign inflammatory condition of the salivary gland. Clinically, it produces a firm swelling of the gland and may be difficult to distinguish from neoplasia. This is an under-recognized entity in the surgical pathology and cytology literature. We describe our experience with Kuttner tumor. METHODS: Retrospectively, we analyzed a group of 7 patients with chronic sclerosing sialadenitis treated from January 1999 to March 2010 at the Department of Otorhinolaryngology, FD Roosevelt Faculty Hospital in Banska Bystrica, Slovakia. The

[Effectiveness of pedicled sternocleidomastoid muscle flap in repairing defect after parotidectomy].

OBJECTIVE: To investigate the effectiveness of pedicled sternocleidomastoid muscle flap in repairing defect after parotidectomy by comparing with direct suture. METHODS: The clinical data were retrospectively analyzed, which were from 73 patients with parotid tumor undergoing parotidectomy between January 2002 and April 2010. After parotidectomy, defects were repaired with pedicled sternocleidomastoid muscle flap in 38 cases (flap group) and with direct suture in 35 cases (control group). There was no significant difference in gender, age, disease duration, tumor location and size between 2 g

Parotid tumor presenting with hemifacial spasm.

A 47-year-old man presented with right parotid swelling and a history of frequent attacks of hemifacial spasm. MRI of the brain and neck showed a mass in the right parotid gland. Fine needle aspiration biopsy of the mass revealed a pleomorphic adenoma of the parotid gland, which was confirmed after total right parotidectomy. His attacks of hemifacial spasm did not improve after surgery and 8 months postoperatively, he received botulinum toxin-A injections, which improved his symptoms. Clinicians need to be aware that patients with occult parotid tumors can present like patients with classic he

Skin type spiradenoma of the parotid gland with malignant transformation: report of a case with analysis of the CYLD gene.

A distinctive variant of basal cell adenoma called membranous basal cell adenoma is well recognized in salivary glands. It appears identical to cutaneous cylindroma, hence the synonym dermal analogue tumor. We present a case of a tumor involving the parotid gland that appeared identical to cutaneous spiradenoma. In addition, the lesion had a malignant component identical to basal cell adenocarcinoma. The patient had no features suggestive of Brooke-Spiegler syndrome. Analysis of the CYLD gene revealed polymorphisms. This is the first report of a "dermal analogue tumor" of the salivary gland th

Surgical treatment of a giant lipoma in the parotid gland.

Recently, a case of deep-lobe lipoma with enucleation was reported, but frozen-section biopsy for the confirmation of the malignancy was not done. It has been suggested that lipoma in the deeper tissues should be regarded as a well-differentiated liposarcoma and be treated with wide excision. Our experience is that of a 75-year-old woman who had a mass with fat density in the deep lobe of the right parotid gland, which extended through the parapharyngeal and the buccal spaces. Lumpectomy with frozen-section biopsy was performed, not only preserving branches of facial nerve but also ruling out

Assessment of parotid gland dose changes during head and neck cancer radiotherapy using daily megavoltage computed tomography and deformable image registration.

PURPOSE: To analyze changes in parotid gland dose resulting from anatomic changes throughout a course of radiotherapy in a cohort of head-and-neck cancer patients. METHODS AND MATERIALS: The study population consisted of 10 head-and-neck cancer patients treated definitively with intensity-modulated radiotherapy on a helical tomotherapy unit. A total of 330 daily megavoltage computed tomography images were retrospectively processed through a deformable image registration algorithm to be registered to the planning kilovoltage computed tomography images. The process resulted in deformed parotid

Management of a total parotidectomy defect with a gastrocnemius muscle transfer and vascularized sural nerve grafting.

Immediate facial nerve reconstruction is very demanding after total parotidectomy. Under such conditions, we reconstructed facial nerves using vascularized sural nerve with free lateral gastrocnemius muscle flap. The patient was a 14-year-old male diagnosed with mucoepidermoid carcinoma of the right parotid gland. We reconstructed zygomatic, buccal, and mandibular branches of facial nerve using 2 vascularized sural nerves, medial sural cutaneous nerve, and the lateral sural cutaneous nerve. The postoperative course was good, and there was no flap trouble. The reinnervated nerve recovered from

[Frey syndrome secondary to submaxillectomy and botulinic treatment].

A case of Frey syndrome (FS) secondary to submaxillar gland exeresis is presented and the results of the treatment with botulinum toxin (BTX) type A. FS is a condition of sweating cheek and preauricular area during realtime as a sequel detected in about 20-60% of patients after parotidectomy. The clinical symptoms include swelling, flushing and hyperhidrosis. The treatment choice for this condition is intracutaneous injection of BTX type A which blocks acetylcholine release at the sweat glands. A 30-year-old man, with thyroid medullar carcinoma diagnosed in 2002 received 6 cicles of cisplatin

Sclerosing mucoepidermoid carcinoma of the parotid gland.

Mucoepidermoid carcinoma is the most common malignant tumor of the parotid gland. Its clinical behavior is determined by the relative ratio of mucous and epidermoid cells with pathologic high-grade tumors containing a greater proportion of epidermoid cells. Sclerosing mucoepidermoid carcinoma is a rarely reported variant with unclear long-term clinical behavior. A 23-year-old female was found to have a deep lobe parotid mass on imaging studies. Preoperative evaluation was non-diagnostic. Intraoperatively, the tumor was found to be adherent to the lower division of the facial nerve and portions

Complications of parotidectomy.

Parotidectomy, whether subtotal or total, is a surgical procedure associated with certain possible complications--namely, facial nerve injury, hemorrhage, infection, salivary fistula, seroma, keloid formation, greater auricular nerve anesthesia, gustatory sweating, and recurrent tumor. We review these complications to help the surgeon with preoperative patient counseling, as well as postoperative reassurance, and discuss those that are amenable to further treatment, either medical or surgical or both.

Recurrent pleomorphic adenoma of the head and neck.

We reviewed the clinical and imaging files of 15 patients with pathologically confirmed recurrent pleomorphic adenomas of the head and neck. The primary tumours were in the lacrimal gland (1 case), cheek (2), palate (5), parotid gland (4), and submandibular gland (3). Recurrence was generally late, with an average of 14.9 years between initial surgery and recurrence. Multiple recurrent tumours, measuring 5-50 mm were found in eight patients. In all palatal cases recurrence was large and solitary. Most recurrent pleomorphic adenomas were well delineated with smooth margins, like most primary tu

12q Amplification Characterizes a Distinctive Salivary Gland Tumor with Bizarre Myoepithelial Atypia.

BACKGROUND: Over the past two decades, an increased understanding of molecular alterations has greatly refined salivary gland tumor classification. Many tumors that were previously difficult or impossible to classify have been recognized to represent emerging entities based on shared histologic, immunophenotypic and molecular characteristics. While initial attention was given to carcinomas, more recently molecular discoveries have shed light on salivary gland adenomas as well. We present a series of biphasic salivary gland tumors characterized by striking bizarre myoepithelial atypia, unified

Lymphoid cell rich fine-needle aspirations of the salivary gland: What is the risk of malignancy?

OBJECTIVES: Lymphoid cell rich fine-needle aspirations (FNAs) of the salivary glands pose a diagnostic dilemma, with a wide range of differential diagnoses that include several benign and malignant entities. There is limited literature regarding the entities that are commonly encountered in this situation. Our goal was to characterize the surgical outcome in these cases and to evaluate the risk of malignancy. MATERIAL AND METHODS: This is a retrospective study at a tertiary care institution. Our database was queried over a 10-year period. FNAs yielding a prominent population of well-visualize
