Why low-dose aspirin persists in recurrent pregnancy loss without APS: cognitive bias and system design.

Low-dose aspirin (LDA) is widely prescribed for recurrent pregnancy loss (RPL) even when antiphospholipid syndrome (APS) is absent, despite high-level evidence showing no improvement in live-birth rate in such cases. In Japan and elsewhere, this empirical use persists across fertility and obstetric settings, suggesting a behavioral and systemic rather than purely evidentiary problem. This commentary interprets the persistence of LDA through the lens of cognitive bias and institutional design. At the cognitive level, action bias-the instinct to intervene when outcomes are uncertain-provides a s

Tolerance of uncertainty and reflection ability among medical students in Germany.

BACKGROUND: With growing recognition of uncertainty tolerance (UT) and reflection ability (RA) as core competencies for healthcare professionals, targeted promotion of these skills has become increasingly important in medical education. However, research on the associations between UT and RA and students' individual characteristics, such as age and gender, remains limited, and existing findings are inconsistent. Addressing this gap, the present study assesses UT and RA among medical students at entry to medical school, examines their associations with age and gender, and explores implications

From andragogy to heutagogy: a scaffolded pathway for self-directed neurosurgical learning.

BACKGROUND: Neurosurgical practice demands continual adaptation to complex pathologies, novel technologies, and evolving service models. Traditional teacher-centred training may be insufficient to cultivate the autonomous, reflective, and adaptive experts required for modern neurosurgical care. METHODS: We conducted a targeted narrative review of literature on pedagogy, andragogy, heutagogy, self-directed learning, and the Master Adaptive Learner (MAL) framework, and mapped key principles onto the UK Intercollegiate Surgical Curriculum Programme (ISCP) for neurosurgery and the European neuros

Impostor syndrome, associated factors and impact on well-being across medical undergraduates and postgraduate medical professionals: a scoping review.

OBJECTIVES: Impostor syndrome (IS) is a psychological state whereby individuals doubt their abilities despite evidence of competence. Though IS has been studied in specific medical groups, no review to date compares findings across groups. This study aimed to: (1) determine the range of IS rates among medical undergraduates versus postgraduates and (2) examine associated factors across both groups. DESIGN: This scoping review used the Joanna Briggs Institute methodology for scoping reviews, using a five-step framework. DATA SOURCES: PubMed, Scopus and PsycINFO databases were searched from in

What is the global perspective on advanced practice physiotherapy: A qualitative study across five countries.

BACKGROUND: Despite the growth of advanced practice physiotherapy (APP) and the evidence of its effectiveness, challenges persist in implementing and sustaining this model of care. The main challenge is the lack of a universally accepted definition. The lack of a clear understanding of what APP is, who can become advanced practice physiotherapists, and the required educational qualifications leads to widespread confusion among patients, employers, and healthcare providers. AIM: Our study aims to explore the global perspectives of advanced practice physiotherapy. OBJECTIVES: 1.To understand h

French national diagnosis and care protocol (Protocole National De Diagnostic et de Soins; PNDS): Gaucher disease.

Gaucher disease (GD) is a rare autosomal recessive lysosomal disorder caused by glucocerebrosidase deficiency, with a prevalence in France of around 1/130,000 people. The clinical picture of GD is very heterogeneous, ranging from lifelong asymptomatic forms to severe forms with onset during childhood, such as GD type 2 (< 1% of cases). GD type 1, the most common form (95% of cases), manifests with varying degrees of organomegaly, cytopenia and bone manifestations. Progressive encephalopathy of varying severity is also observed in GD type 3. Symptoms may result in acute and/or chronic pain and

ETHICS IN RADIOLOGICAL PROTECTION FOR MEDICAL DIAGNOSIS AND TREATMENT.

Publication 138 defines the ethical foundations of the ICRP System of Radiological Protection based on core values (beneficence and non-maleficence, dignity, justice, and prudence) and procedural values (accountability, transparency, and inclusiveness). The purpose of the present publication is to propose a practical application of values for medical radiological protection professions. As medicine has a long history and strong culture of ethics, this publication starts by identifying the shared values, and defines a common language between biomedical ethics and radiological protection. Ruxolitinib Inhibits IFNγ Licensing of Human Bone Marrow Derived Mesenchymal Stromal Cells.

Ruxolitinib is a JAK2/JAK1 inhibitor that blocks the inflammatory JAK-STAT signaling pathway. Ruxolitinib has been demonstrated to be effective in the treatment of steroid-resistant acute graft-versus-host disease (GVHD). Ruxolitinib's effect on inflammatory cells of hematopoietic origin is known. However, its effect on nonhematopoietic cell types with immune-modulating and antigen-presenting cell competency plausibly involved in pathogenesis of GVHD has not been explored. Mesenchymal stromal cells (MSCs) are CD45- nonhematopoietic cells of the bone marrow with immune modulatory functions in v
