Orbital tuberculosis with involvement of the eyelid: An unusual presentation.

Although cases of ocular tuberculosis (TB) are increasing, involvement of the eyelid and orbit are unusual. These cases occur secondary to the presence of TB elsewhere in the body, usually pulmonary TB. The primary infection of orbit or eyelid is a rare occurrence. We report a 4-year-old child with primary orbital TB and involvement of the eyelid. The diagnosis of TB should not be missed in patients with ocular symptoms (especially in India) as it is a treatable condition and delays in diagnosis or incorrect diagnosis can lead to serious sequelae.

Mycobacterium gordonae in Patient with Facial Ulcers, Nosebleeds, and Positive T-SPOT.TB Test, China.

Mycobacterium gordonae is often regarded as a weak pathogen that only occasionally causes overt disease. We report a case of M. gordonae infection in the facial skin, nasal mucosa, and paranasal sinus in an immunocompetent patient and review previous cases. The T-SPOT.TB test might be useful in diagnosing such cases.

Associated Risk Factors for Latent Tuberculosis Infection in Subjects with Diabetes.

BACKGROUND AND AIMS: Type 2 diabetes mellitus (DM2) confers a higher risk for active tuberculosis (TB). However, information on associated risk factors for latent tuberculosis infection (LTBI) inpatients with DM2 is limited. We conducted a cross-sectional study to elucidate the prevalence of LTBI and its associated factors on Mexican adults with DM2 receiving medical care at the Mexican Social Security Institute (IMSS). METHODS: Six hundred patients with DM2 without a prior history of TB from outpatient diabetes clinics were enrolled in the study. The tuberculin-skin-test (TST) was performed.

Decreasing household contribution to TB transmission with age: a retrospective geographic analysis of young people in a South African township.

BACKGROUND: Tuberculosis (TB) transmission rates are exceptionally high in endemic TB settings. Adolescence represents a period of increasing TB infection and disease but little is known as to where adolescents acquire TB infection. We explored the relationship between residential exposure to adult TB cases and infection in children and adolescents in a South African community with high burdens of TB and HIV. METHODS: TB infection data were obtained from community, school-based tuberculin skin test (TST) surveys performed in 2006, 2007 and 2009. A subset of 2007 participants received a repeat

Tuberculosis in HIV-infected persons in British Columbia during the HAART era.

OBJECTIVE: Prior to the introduction of highly active antiretroviral therapy (HAART), active tuberculosis (TB) was a major contributor to HIV-related morbidity and mortality in Canada and other low-incidence regions. We performed this study to examine TB incidence, clinical manifestations and screening uptake in HIV-infected TB patients during the era of HAART therapy. METHODS: We performed a retrospective study on all HIV-infected TB patients in British Columbia over a 10-year period (2003-2012). Demographic and clinical characteristics were extracted along with screening and treatment outco

Bacille Calmette-Guérin vaccination is associated with lower prevalence of allergic diseases in Indian children.

BACKGROUND: Immunization with bacille Calmette-Guérin (BCG) has been shown to augment T helper (Th) 1 biased response and hence gives protection against developing allergies. However, results across the studies have been inconclusive and there is no Indian study verifying this association. We explored the link between BCG vaccination and prevalence of allergic diseases among school children of Chandigarh, North India. METHODS: In a large cross-sectional study, children aged 7-14 years, with and without documentation and/or scar consistent with BCG vaccination, were examined over a period of 2

The utility of an interferon gamma release assay for diagnosis of latent tuberculosis infection and disease in children: a systematic review and meta-analysis.

BACKGROUND: The utility of interferon gamma release assays (IGRAs) has been assessed in adults, but remains unclear in children. We reviewed the literature on the use of a commercial IGRA in immunocompetent children for the diagnosis of both latent tuberculosis infection (LTBI) and TB disease. METHODS: We searched PubMed for studies published before January 2010 on the diagnosis of TB in children using an IGRA. We compared the specificity and sensitivity of the tuberculin skin test (TST) and the IGRA for LTBI and conducted a random effects meta-analysis on sensitivity of the IGRA for TB disea

Role of the mantoux test and treatment with antitubercular therapy in a South Indian patient population of presumed intraocular tuberculosis.

AIM: To assess the role of mantoux test and ATT in presumed intraocular tuberculosis in South India. MATERIALS & METHODS: Retrospective study of 51 records of patients with recurrent uveitis with a documented positive mantoux test and a minimum of 1 year followup from the time of initiation of ATT between January 1997 and May 2008. The main outcome measure was the recurrence of inflammation after 1 year of starting ATT. RESULTS: There was a significant reduction in the number of recurrences of uveitis post ATT (.000). CONCLUSIONS: There is a definite role of ATT in latent TB with uveitis.

Prevalence of latent tuberculosis infection among gold miners in South Africa.

SETTING: South African gold mines. OBJECTIVE: To determine the prevalence of latent tuberculosis infection (LTBI) and risk factors for a positive tuberculin skin test (TST) among gold miners. DESIGN: Cross-sectional survey. Human immunodeficiency virus (HIV) status was determined by self-report and medical records. TST positivity was defined by the mirror method to estimate the prevalence of LTBI, and by the US Centers for Disease Control and Prevention definitions to explore risk factors at the individual level. RESULTS: Among 429 participants (105/130 subjects aged <30 years, 324/390 > or

Tuberculosis treatment effect on T-cell interferon-gamma responses to Mycobacterium tuberculosis-specific antigens.

The hypothesis that T-cell interferon-gamma responses to Mycobacterium tuberculosis-specific antigens decline as disease activity diminishes with tuberculosis (TB) treatment has generated interest in the interferon-gamma release assays (IGRAs) as treatment-monitoring tools. We studied the effect of TB treatment on these responses as measured by the QuantiFERON-TB Gold In-tube (QFT-IT) and T-SPOT.TB assays. 275 sputum culture-positive, HIV-uninfected pulmonary TB patients were tested with QFT-IT and T-SPOT.TB at baseline, treatment completion and 6 months thereafter. The QFT-IT was also perform

Enhanced protection against bovine tuberculosis after coadministration of Mycobacterium bovis BCG with a Mycobacterial protein vaccine-adjuvant combination but not after coadministration of adjuvant alone.

Current efforts are aimed at optimizing the protective efficacy of Mycobacterium bovis BCG by the use of vaccine combinations. We have recently demonstrated that the protection afforded by BCG alone is enhanced by vaccinating cattle with a combination of vaccines comprising BCG and a protein tuberculosis vaccine, namely, culture filtrate proteins (CFPs) from M. bovis plus an adjuvant. In the current study, three different adjuvant systems were compared. The CFP was formulated with a depot adjuvant, dimethyldioctadecyl ammonium bromide (DDA), together with one of three different immunostimulant

[Accuracy of inpatient and outpatient gastric lavage in the diagnosis of pulmonary tuberculosis in children].

OBJECTIVE: To compare gastric lavage (GL) performed in inpatients with GL performed in outpatients in terms of its accuracy in diagnosing pulmonary tuberculosis (TB) in children. METHODS: A prospective study was carried out in the state of Espírito Santo, Brazil, from 1999 to 2003. A total of 230 children suspected of having TB (103 inpatients and 127 outpatients) were selected to undergo GL. Those thus diagnosed with TB (n = 53) were divided into two groups: inpatient GL (n = 30) and outpatient GL (n = 23). All 53 children were monitored for 6 months in order to evaluate the accuracy of the

Potential challenges to the Stop TB Plan for humans in China; cattle maintain M. bovis and M. tuberculosis.

Thirty-eight cows in a herd were determined to be positive for bovine tuberculosis (TB). The bacterial isolation and characterization with multiplex PCR identified six Mycobacterium tuberculosis isolates. The Mycobacterium bovis and M. tuberculosis infection induced comparable pathology in cattle in both gross pathology and histopathology based on the qualitative assessment of the sampled lung tissues. The spoligotyping demonstrated that cow M. tuberculosis isolates belonged to Beijing family strains. Meanwhile, the isolates from tuberculosis patients hospitalized in the local hospitals were a

Prevalence of abnormal radiological findings in health care workers with latent tuberculosis infection and correlations with T cell immune response.

BACKGROUND: More than half of all health care workers (HCWs) in high TB-incidence, low and middle income countries are latently infected with tuberculosis (TB). We determined radiological lesions in a cohort of HCWs with latent TB infection (LTBI) in India, and determined their association with demographic, occupational and T-cell immune response variables. METHODOLOGY: We obtained chest radiographs of HCWs who had undergone tuberculin skin test (TST) and QuantiFERON-TB Gold In Tube (QFT), an interferon-gamma release assay, in a previous cross-sectional study, and were diagnosed to have LTBI

Brainstem tuberculoma in the immunocompetent: case report and literature review.

Brainstem tuberculoma is an uncommon entity especially in the immunocompetent adult. The authors describe the case of a 32-year-old immunocompetent woman with no systemic signs or symptoms of tuberculosis, presenting with sudden-onset hemiparesis and diplopia. MRI of brain revealed an enhancing lesion in the midbrain and thalamus. Cerebrospinal fluid examination was normal. Chest imaging was consistent with miliary tuberculosis; however initial expectorated sputum was culture negative. Bronchosopy sputum culture was positive for Mycobacterium tuberculosis. Her PPD was negative. Patient became
