Change in Policy Allowing Overlapping Surgery Decreases Length of Stay in an Academic, Safety-Net Hospital.

BACKGROUND: The practice of surgeons running overlapping operating rooms has recently come under scrutiny. OBJECTIVE: To examine the impact of hospital policy allowing overlapping rooms in the case of patients admitted to a tertiary care, safety-net hospital for urgent neurosurgical procedures. METHODS: The neurosurgery service at the hospital being studied transitioned from routinely allowing 1 room per day (period 1) to overlapping rooms (period 2), with the second room being staffed by the same attending surgeon. Patients undergoing neurosurgical intervention in each period were retrospec

Extended Length of Stay After Lumbar Spine Surgery: Sick Patients, Postoperative Complications, or Practice Style Differences Among Hospitals and Physicians?

BACKGROUND: The degree to which extended length of stay (LOS) after surgery represents patient illness or postoperative complications is unknown. The aim of this study was to investigate the influence of postoperative complications and patient comorbidities on variance in extended length of hospital stay after lumbar spine surgery. METHODS: Data from the American College of Surgeons National Surgical Quality Improvement Program were analyzed from January 1, 2008, through December 31, 2014. Study participants were 23,102 patients undergoing a 1-level or 2-level lumbar decompression and fusion

Postoperative speech impairment and surgical approach to posterior fossa tumours in children: a prospective European multicentre cohort study.

BACKGROUND: Brain tumours are the most common solid tumours in childhood. Half of these tumours occur in the posterior fossa, where surgical removal is complicated by the risk of cerebellar mutism syndrome, of which postoperative speech impairment (POSI) is a cardinal symptom, in up to 25% of patients. The surgical approach to midline tumours, mostly undertaken by transvermian or telovelar routes, has been proposed to influence the risk of POSI. We aimed to investigate the risk of developing POSI, the time course of its resolution, and its association with surgical approach and other clinical

Pain and psycho-affective disorders.

The subject of human pain can be subdivided into two broad categories: physical pain and psychological pain. Since the dawn of human consciousness, each of these two forms of pain-one clearly physical, the other having more to deal with the mind-have played a central role in human existence. Psychological pain and suffering add dimensions that go far beyond the boundaries of its physical counterpart. In the past 50 years, one of the more remarkable accomplishments of medical science has been to increasingly enable the clinician to impact, as never before, each of these critical realms of human

The development of intraoperative imaging has made it possible to visualize shifting brain structures during surgery, and may allow greater intraoperative discrimination of normal and abnormal tissue. This may provide greater confidence to the neurosurgeon to proceed with a more extensive resection while decreasing postoperative morbidity. We investigated the intraoperative use of a portable CT scanner in the resection of 4 cases of supratentorial, subcortical astrocytomas of childhood to assess its usefulness in determining the endpoint of the dissection. We operated on 4 patients, ages 3-17,

Implications of overstuffing in cervical total disc replacement: biomechanical assessment of index and adjacent level range of motion.

PURPOSE: We assessed the biomechanical effects of oversizing a cervical total disc replacement (cTDR) device using a cadaver model. We hypothesized that an overstuffed cTDR would significantly reduce flexion-extension range of motion (FE ROM) compared to the native spine and a 2-level cTDR, while still providing more 2-level motion compared to hybrid (ACDF adjacent to a cTDR). METHODS: Eight C2-T1 cadaveric spines were instrumented at C4-C6 by a board-certified neurosurgeon with the following interventions: 2-level cTDR, cTDR with one overstuffed level (+ 1 mm height), and hybrid with ACDF. A

Analysis of the learning curve for neuroendoscopic evacuation of basal ganglia hemorrhage.

BACKGROUND: Neuroendoscopic evacuation has become an important minimally invasive approach for the treatment of basal ganglia hemorrhage. However, its technical complexity requires a well-defined learning curve, and evidence regarding learning curve characteristics, particularly among experienced surgeons, remains limited. OBJECTIVE: To evaluate the learning curve of neuroendoscopic evacuation for basal ganglia hemorrhage and to determine the number of cases required to achieve technical proficiency. METHODS: A retrospective analysis was conducted on 120 consecutive patients with basal gangl

Evaluation of nationwide variation in post-haemorrhagic ventricular dilatation management following national guideline implementation in the Netherlands.

PURPOSE: Post-haemorrhagic ventricular dilatation (PHVD) is a severe complication of germinal matrix-intraventricular haemorrhage in preterm infants and is associated with significant morbidity and adverse neurodevelopmental outcomes. In 2023, national recommendations for PHVD management were revised, but implementation has not been evaluated. We aimed to assess diagnostic and therapeutic practices across Dutch neonatal intensive care units (NICUs), quantify inter-centre variation, and provide a basis for a national PHVD registry. MATERIALS AND METHODS: A nationwide, cross-sectional survey wa

Audiologic Outcomes with Auditory Brainstem Implantation Including Successful Open Set Speech Perception with Bilateral Implantation.

Background/Objectives: For patients with profound deafness resulting from auditory nerve pathology, as in Neurofibromatosis type 2, auditory brainstem implantation (ABI) can restore meaningful acoustic input. The literature reporting real-world results for ABI users is limited, especially regarding patients with bilateral implants. Here, we provide an updated report on the audiologic outcomes among all ABI patients treated at a tertiary institution, including high-performing bilateral ABI users. Methods: In this updated and expanded retrospective case series, audiologic outcomes were reviewed

Altering physician referral practices is challenging, but not impossible: spine assessment clinic quality improvement study.

INTRODUCTION: Access to medical specialists is a persistent challenge, with neurosurgical spine services reporting some of the longest waits across all fields. Inappropriate and incomplete referrals contribute to delayed access to these providers. Referral guidelines and physician education have been shown to decrease such inefficiencies. Therefore, the goal of this study was to address inappropriate referrals directed to the neurosurgical spine assessment clinic via implementation of a quality improvement initiative. We hypothesised that appropriate referrals, which included patients with pot

Spatial attention-based CSR-Unet framework for subdural and epidural hemorrhage segmentation and classification using CT images.

BACKGROUND: Automatic diagnosis and brain hemorrhage segmentation in Computed Tomography (CT) may be helpful in assisting the neurosurgeon in developing treatment plans that improve the patient's chances of survival. Because medical segmentation of images is important and performing operations manually is challenging, many automated algorithms have been developed for this purpose, primarily focusing on certain image modalities. Whenever a blood vessel bursts, a dangerous medical condition known as intracranial hemorrhage (ICH) occurs. For best results, quick action is required. That being said

Computed-tomography evaluation of parietal foramen topography in adults: a retrospective analysis.

BACKGROUND: The parietal foramen (PF) of the skull is a variable anatomic feature with important implications for venous drainage, infection, and injury. Its topography is clinically relevant for neurosurgeons for intracranial navigation and preoperative planning. METHODS: PF topography was investigated in a series of 440 head computed-tomography scans of Omani subjects at Sultan Qaboos University Hospital. The mean age of the patients was 52 ± 17 years and there were 160 males and 280 females. The topography features of the PF, including frequency, diameter, patency, and relative position in…

Application of fluorescein sodium videoangiography in aneurysm surgery.

BACKGROUND: The use of fluorescent technologies in vascular neurosurgery emerged after indocyanine green video angiography (ICG-VA) was first described in 2003. As data supporting the efficiency of ICG in preventing postoperative complications has grown substantially, it has now established itself as the standard of care. However, the predominant literature centers on ICG techniques, leaving the evaluation of cost-effective fluorescein tools pending. We report the results of a prospective study in which we demonstrated the impact of intraoperative fluorescein videoangiography (FL-VA) in aneury

Revision and removal of vagus nerve stimulation systems: twenty-five years' experience.

BACKGROUND: Epilepsy, a disease characterized by recurrent seizures, is a common chronic neurologic condition. Antiepileptic drugs (AED) are the mainstay of treatment for epilepsy. Vagus nerve stimulation (VNS) surgery is an adjuvant therapy for the treatment of drug refractory epilepsy (DRE). VNS revision and implant removal surgeries remain common. METHODS: Using a single neurosurgeon data registry for epilepsy surgery, we retrospectively analyzed a total of 824 VNS surgeries. Patients were referred to two Level IV Comprehensive Epilepsy centers (from 08/1997 to 08/2022) for evaluation. Pat

Neuroendoscopic Lavage and Third Ventriculostomy for the Treatment of Intraventricular Hemorrhage and Hydrocephalus in Neonates. A Prospective Study with 18 Months of Follow-Up.

BACKGROUND: Neonatal intraventricular hemorrhage (IVH) may evolve into posthemorrhagic hydrocephalus and cause neurodevelopmental impairment, becoming a common complication of premature infants, occurring in up to 40% of preterm infants weighing less than 1,500 g at birth. Around 10 to 15% of preterm infants develop severe (grades III-IV) IVH. These infants are at high risk of developing posthemorrhagic hydrocephalus. Neuroendoscopic lavage (NEL) is a suitable alternative for the management of this pathology. In this study, an endoscopic surgical approach directed toward the removal of intrave

Assessment of postoperative pain, dysesthesia, and weather sensitivity after pterional and temporal neurosurgical approaches.

OBJECTIVE: Many neurosurgical approaches require incision of the temporal muscle (TM). Consequently, patients often report reduced opening of the mouth, facial asymmetry, numbness, and pain after lateral craniotomies. A systematic assessment of these postoperative subjective complaints is lacking in the literature. Therefore, in this study, the authors evaluate subjective complaints after pterional, frontolateral-extended pterional, or temporal craniotomy using a 6-item questionnaire. They examine the association of these subjective complaints with the extent of the mobilization of the TM. ME
