[Fatal aspergillosis after cardiac transplantation. About 26 cases].

Complications due to infection by aspergillus and favorized by therapeutic immunodepression are a significant cause of mortality after cardiac transplantation, but are only rarely diagnosed before autopsy. A detailed description of the anatomical lesions and their localisation might help the interpretation of symptoms observed in vivo. In a series of 147 autopsies subsequent to cardiac transplantation, aspergillosis appeared as the cause of death in 26 cases. Three anatomoclinical presentations could be identified: early post-operative septic pyohaemia (14 cases), delayed septic pyohemia (7 ca

Cytokine and inducible nitric oxide synthase mRNA expression during experimental murine cryptococcal meningoencephalitis.

The immune events that take place in the central nervous system (CNS) during cryptococcal infection are incompletely understood. We used competitive reverse transcription-PCR to delineate the time course of the local expression of mRNAs encoding a variety of cytokines and inducible nitric oxide synthase (iNOS) during progressive murine cryptococcal meningoencephalitis and assessed the CNS inflammatory response using immunohistochemistry. Interleukin 18 (IL-18), transforming growth factor beta1, and IL-12p(40) mRNAs were constitutively expressed in the brains of infected and uninfected mice; IL

Low-velocity penetrating craniocerebral injury in childhood.

Fifty-four children with low-velocity penetrating injury of the skull and brain are described. The incidence of septic complications was 43%, which is significantly higher than that seen in adults. Nine percent of children developed vascular complications. Due to the high septic complication rate, a more aggressive management protocol consisting of craniectomy and dural repair under antibiotic cover is suggested.

Management of Middle Third Hypoplasia Using Rigid External Device: Long-Term Analysis of Results and Report of Complications.

OBJECTIVE: The objective of this study was to review and present the experience, with the functional results as well as complications of the individuals who underwent facial middle-third advancement with rigid external device. We report a major complication (cerebral abscess) secondary to pin migration. BACKGROUND: There are functional and esthetics objectives when performing surgical procedures in middle facial third hypoplasia. Many have compared external and internal distraction devices use and results with a lack of evidence on perioperative morbidity; this study was designed to revise co

Usefulness of 3T split acquisition fast spin-echo diffusion-weighted imaging for differentiating pituitary abscess from other sellar cystic lesions: a preliminary study.

PURPOSE: Pituitary abscess is a rare condition that can be challenging to distinguish from other sellar cystic lesions (OSCLs) due to no specific symptoms and overlapping imaging features. Accurate differentiation is crucial for appropriate treatment planning. This study aimed to evaluate the usefulness of 3T split acquisition fast spin-echo diffusion-weighted imaging (SPLICE-DWI) in differentiating pituitary abscesses from OSCLs. METHODS: We retrospectively analyzed magnetic resonance imaging (MRI) data from 32 patients with sellar cystic lesions, including 5 with pathologically confirmed pi

Disseminated cutaneous nocardiosis mimicking cellulitis and erythema nodosum.

Infection with Nocardia asteroides is a rare, life-threatening infection, which is most commonly encountered in immunocompromised patients. Cutaneous involvement is usually seen with disseminated infection but may also occur as primary cutaneous nocardiosis. We present a case of an immunocompromised patient who presented with cellulitis of the right hand and disseminated subcutaneous nodules of the lower extremities resembling erythema nodosum. Cultures from both a skin biopsy of a subcutaneous nodule on the leg as well as a surgical specimen from the debridement of her hand grew Nocardia aste

Orofacial infections in the 21st century.

In the 21st century, most orofacial infections are routinely managed with proper clinical assessment, selective diagnostic imaging, appropriate antibiotic therapy and timely surgical intervention. A thorough understanding of contemporary microbiology, pharmacology and surgical anatomy will enable the clinician to minimize the morbidity and mortality of orofacial infections.

Combined CNS and pituitary involvement as a primary manifestation of Wegener granulomatosis.

Wegener granulomatosis (WG) is a systemic vasculitis of small and medium vessels. It predominantly affects the upper and/or lower respiratory airway and kidneys. Its pathogenesis is not fully understood. WG relatively frequently affects the nervous system (in 30-50% according to the different studies). Most frequently, it manifests as necrotizing vasculitis that leads to the peripheral neuropathies or to the cranial nerves palsy. Impairment of the central nervous system (CNS) is less frequent and occurs in 2-8% of patients. Three major pathogenetic mechanisms were described: CNS vasculitis, sp

Successful therapy combined with surgery for severe post-transplant nocardiosis.

We report a case of a 35-year-old man with nocardiosis infection involving soft tissue and the central nervous system who had received a cadaveric donor kidney. The patient was admitted with fever, malaise and right shoulder pain. Soft tissue abscess was seen on ultrasound examination. It was presumed due to gram (+) microorganisms, so 4 g day (IV) ampicillin/sulbactam was started empirically once the abscess was drained. Nocardia asteroides was found in the pus specimen. On the second day in hospital, severe headache, ataxia and signs of meningeal irritation appeared. The cranial CT showed tw

Pediatric intracranial epidural abscess secondary to an infected scalp vein catheter.

The authors present a case of a 5-week-old infant developing a retrotorcular epidural abscess as a result of an infected scalp vein catheter. The abscess developed in the absence of sinusitis, otitis, trauma, or prior surgery. The diagnosis of epidural abscess was made on the basis of magnetic resonance imaging and ultrasound-guided aspiration of the fluid collection. An identified strain of Staphylococcus epidermidis was cultured from both the intravenous catheter and the abscess. The patient underwent a suboccipital craniectomy with drainage of the abscess and a 6-week total course of intrav
