Loneliness as a Risk Factor for Time to Care Home Entry for Older Adults Receiving Community Care.

BACKGROUND AND OBJECTIVES: International efforts to contain long-term care costs have prioritized personal care. However, reductions in services aimed at addressing loneliness or promoting social participation may affect demand for long-term care facilities. Research on the impact of loneliness on entry to residential or nursing care is based on survey data, which under-represents those with highest needs. Administrative records include such individuals and, unlike surveys, contain continuous data on service receipt, enabling accurate modeling of time to care home entry. RESEARCH DESIGN AND M

Radiomics of pericoronary adipose tissue on computed tomography angiography predicts coronary heart disease in patients with type 2 diabetes mellitus.

BACKGROUND: Diabetes is a common chronic metabolic disease. The progression of the disease promotes vascular inflammation and the formation of atherosclerosis, leading to cardiovascular disease. The coronary artery perivascular adipose tissue attenuation index based on CCTA is a new noninvasive imaging biomarker that reflects the spatial changes in perivascular adipose tissue attenuation in CCTA images and the inflammation around the coronary arteries. In this study, a radiomics approach is proposed to extract a large number of image features from CCTA in a high-throughput manner and combined

Patient Attitudes Towards Ambient Artificial Intelligence in Clinical Consultations.

Background Ambient Artificial Intelligence (AI) technologies are increasingly being integrated into clinical practice to support documentation. These systems employ speech recognition, talk-to-text technology, generative intelligence and large language models (LLMs) to automatically transcribe and summarise clinician-patient consultations into structured clinical notes. While it may reduce documentation burden and enable patient-focused interactions, there remains limited evidence regarding patients' perspectives, experiences and the acceptability of ambient AI use in healthcare settings. Evaluating the oncologic impact of statin use in gynecologic cancers: a critical review of pre-clinical and clinical evidence.

Statins, potent inhibitors of 3-hydroxy-3-methylglutaryl coenzyme A reductase, have demonstrated anti-neoplastic activity in pre-clinical models; however, their clinical efficacy in gynecologic malignancies remains uncertain. This review evaluates the association between statin use and survival outcomes in cervical, endometrial, and ovarian cancers, and synthesizes emerging pre-clinical and clinical data to clarify their translational potential. A PubMed search identified 44 English-language studies published between January 2005 and August 2025 that reported survival outcomes among gynecologi

Systems Thinking: Identifying Leverage Points for Achieving a Continuum of Competency-Based Health Professions Education.

Competency-based education is widely endorsed in health professions education (CBHPE) as an outcomes-focused approach to ensuring that all graduates demonstrate the competencies needed to improve patient and population health. Despite adoption of outcomes frameworks, accreditation requirements, and CBHPE tactics, interventions remain siloed within programs and training levels rather than spanning the education to practice continuum. Consequently, learners and teachers experience fragmented educational journeys that can fall short of addressing each learner's needs for continuous learning and g

Development of an LLM pipeline exceeding physician-documented cardiovascular risk scores under routine clinical conditions.

AIMS: Risk scores are essential to evidence-based cardiovascular care, but manual calculation is labour intensive and error prone. Large language models (LLMs) could automate this process, yet LLMs are limited by their propensity for calculation errors and factual hallucinations. Pipelines separating LLM-based data extraction from deterministic score computation may improve reliability and transparency. METHODS AND RESULTS: We conducted a retrospective diagnostic study at a quaternary heart centre in Germany (January 2020 to July 2023). Patients with atrial fibrillation (n = 179) from an abla…

Understanding the Cellular Spatiotemporal Dogma by Spatial and Single-cell Omics.

The biological complexity of plants arises from highly coordinated cellular activities. We propose that a "cellular spatiotemporal dogma" governs the zygote's programmed development into a complete plant and its adaptation to various environmental stresses, representing the set of principles describing how gene expression, cell identity, and function are coordinated across physical spatial contexts and temporal developmental progressions. Historically, our understanding of this dogma remains limited because traditional bulk tissue sequencing provides only a homogenized average of gene expressi
