Outcomes and re-interventions after one-stage repair of transposition of great arteries and aortic arch obstruction.

OBJECTIVES: One-stage repair of transposition of great arteries (TGA) and aortic arch obstruction (AAO) is currently advocated, but carries formidable surgical challenges. This report presents our experience and re-interventions for residual lesions over the last 10 years. METHODS: Twenty-two patients (19.5 ± 42.4 days; range 2-206; median 10 days, 3.5 ± 0.6 kg) diagnosed with TGA (nine patients) or double outlet right ventricle (DORV) (13 patients) and AAO underwent one-stage repair. Of the nine TGA patients (two with intact ventricular septum), AAO were: two patients hypoplastic arch, one p

Evaluating left ventricular function and myocardial fibrosis in hypertrophic obstructive cardiomyopathy with corrected myocardial work.

BACKGROUND: Echocardiographic Myocardial work (MW) has potential value in hypertrophic obstructive cardiomyopathy (HOCM). This study aimed to utilize corrected MW indices to characterize left ventricular (LV) myocardial mechanical remodeling and evaluate the extent of myocardial fibrosis (MF) in patients with HOCM. METHODS: We prospectively studied 41 patients with HOCM undergoing septal myectomy (SM). 21 patients underwent intraoperative invasive pressure measurement to validate the noninvasive left ventricular systolic pressure (LVSP) estimation and corrected MW analysis methods. Right ventricular to pulmonary arterial coupling in patients with repaired tetralogy of Fallot: a case series.

BACKGROUND: In repaired tetralogy of Fallot (ToF) patients with residual right ventricular (RV) outflow tract obstructions (RVOTO), risk stratification and timing of re-interventions are based on RVOTO gradients. However, this might be insufficient to prevent RV dysfunction. Instead, assessment of RV to pulmonary arterial (RV-PA) coupling allows integrated assessment of RV function in relationship to its afterload and could be of additional value in clinical decision-making. CASE SUMMARY: Two patients with repaired ToF and residual RVOTO without pulmonary regurgitation underwent right heart c

Recognizing Cyanotic Congenital Heart Disease Masked by Infant Pneumonia in a Resource-Limited Setting: A Somali Case Report of Tetralogy of Fallot.

BACKGROUND: In resource-limited settings lacking routine newborn screening, common lower respiratory tract infections like community-acquired pneumonia (CAP) can obscure or delay the diagnosis of underlying cyanotic congenital heart disease (CHD). Tetralogy of Fallot (TOF) is the most common cyanotic CHD, yet its co-occurrence with CAP creates significant diagnostic masking, as acute pulmonary signs often conceal cardiac abnormalities. CASE PRESENTATION: A 5-month-old male presented in Borama, Somalia, with severe respiratory distress, fever, cough, and bilateral crackles, leading to an initi

Single-Barrel, Double-Barrel, and Fenestrated Endografts to Facilitate Transcatheter Pulmonary Valve Replacement in Large RVOT.

OBJECTIVES: The aim of this study was to test the hypothesis that narrowing the landing zone using commercially available endografts would enable transcatheter pulmonary valve replacement (TPVR) using commercially available transcatheter heart valves. BACKGROUND: TPVR is challenging in an outsized native or patch-repaired right ventricular outflow tract (RVOT). Downsizing the RVOT for TPVR is currently possible only using investigational devices. In patients ineligible because of excessive RVOT size, TPVR landing zones were created using commercially available endografts. METHODS: Consecutiv

Review of the role of cardiovascular magnetic resonance in congenital heart disease, with a focus on right ventricle assessment.

Adult patients with congenital heart disease (ACHD) represent a growing population due to progress in management. Surgical procedures generally fall short of restoring entirely normal anatomical and functional relations. Further procedures can be needed and lifelong follow-up is required. The right ventricle (RV) plays an important role in congenital heart disease and cardiac magnetic resonance (CMR) imaging has become the imaging method of choice for its assessment. CMR can provide relatively accurate measurements of RV volume and function, and arterial flow, with additional anatomical inform

Forward Dynamics: Modern Insights into Mitral Systolic Anterior Motion.

Systolic anterior motion (SAM) of the mitral valve can occur either in association with or in the absence of hypertrophic obstructive cardiomyopathy (HOCM). SAM induces dynamic left ventricular outflow tract obstruction (LVOTO) and, in the majority of cases, is associated with a substantial degree of mitral regurgitation (MR) that significantly impacts patient morbidity and mortality. This narrative review explores the contemporary understanding of the pathophysiology, diagnosis, and management of SAM, focusing particularly on surgical strategies and the novel therapeutic class of cardiac myos

Hypoplastic left heart syndrome.

Hypoplastic left heart syndrome (HLHS) refers to the abnormal development of the left-sided cardiac structures, resulting in obstruction to blood flow from the left ventricular outflow tract. In addition, the syndrome includes underdevelopment of the left ventricle, aorta, and aortic arch, as well as mitral atresia or stenosis. HLHS has been reported to occur in approximately 0.016 to 0.036% of all live births. Newborn infants with the condition generally are born at full term and initially appear healthy. As the arterial duct closes, the systemic perfusion becomes decreased, resulting in hypo

Effect of Diffuse Subendocardial Hypoperfusion on Left Ventricular Cavity Size by 13N-Ammonia Perfusion PET in Patients With Hypertrophic Cardiomyopathy.

Vasodilator-induced transient left ventricular (LV) cavity dilation by positron emission tomography (PET) is common in patients with hypertrophic cardiomyopathy (HC). Because most patients with PET-LV cavity dilation lack obstructive epicardial coronary artery disease, we hypothesized that vasodilator-induced subendocardial hypoperfusion resulting from microvascular dysfunction underlies this result. To test this hypothesis, we quantified myocardial blood flow (MBF) (subepicardial, subendocardial, and global MBF) and left ventricular ejection fraction (LVEF) in 104 patients with HC without sig
