Longitudinal systolic strain profiles and outcomes in peripartum cardiomyopathy.

OBJECTIVE: Peripartum cardiomyopathy (PPCM) is a rare cause of heart failure that develops in the last month of pregnancy or within 5 months of delivery. Longitudinal systolic strain has been shown to be impaired in HF patients with both preserved and depressed EF, but has not previously characterized in patients with PPCM. METHODS: The purpose of our study was to investigate the prognosis and recovery in patients with PPCM and use 2D strain imaging to characterize regional and global LV strain in patients with PPCM. Between 2009 and 2014, we identified 47 newly diagnosed patients with PPCM a

Comparative assessment of Patient Health Questionnaire-9 and Edinburgh Postnatal Depression Scale for screening antenatal depression among Chinese pregnant women.

BACKGROUND: The Patient Health Questionnaire-9 (PHQ-9) and Edinburgh Postnatal Depression Scale (EPDS) are commonly used for assessing antenatal depression in China. However, clear guidelines for their application are lacking in China, leading to inconsistencies in prenatal mental health care. This study compares the psychometric properties of these scales to improve early detection and intervention strategies for antenatal depression. METHODS: The cross-sectional study included 44,220 women receiving prenatal care from January 1, 2020, to January 31, 2024. All the participants underwent a ps

Blurring reality with fiction: Exploring the stories of women, madness, and infanticide.

PROBLEM: Often, there is a sense of shock and disbelief when a mother murders her child. BACKGROUND: Yet, literary texts (plays, poems and novels) contain depictions of women experiencing mental illness or feelings of desperation after childbirth who murder their children. AIM: To further understand why a woman may harm her child we examine seven literary texts ranging in time and place from fifth century BCE Greece to twenty-first century Australia. METHODS: A textual analysis approach examined how the author positioned the woman in the text, how other characters in the text reacted to the

Early intervention to protect the mother-infant relationship following postnatal depression: study protocol for a randomised controlled trial.

BACKGROUND: At least 13% of mothers experience depression in the first postnatal year, with accompanying feelings of despair and a range of debilitating symptoms. Serious sequelae include disturbances in the mother-infant relationship and poor long-term cognitive and behavioural outcomes for the child. Surprisingly, treatment of maternal symptoms of postnatal depression does not improve the mother-infant relationship for a majority of women. Targeted interventions to improve the mother-infant relationship following postnatal depression are scarce and, of those that exist, the majority are not

Disparities in Preconception Health Indicators -
Behavioral Risk Factor Surveillance System, 2013-2015, and Pregnancy Risk Assessment Monitoring System, 2013-2014.

PROBLEM/CONDITION: Preconception health is a broad term that encompasses the overall health of nonpregnant women during their reproductive years (defined here as aged 18-44 years). Improvement of both birth outcomes and the woman's health occurs when preconception health is optimized. Improving preconception health before and between pregnancies is critical for reducing maternal and infant mortality and pregnancy-related complications. The National Preconception Health and Health Care Initiative's Surveillance and Research work group suggests ten prioritized indicators that states can use to m

Aspirin (single dose) for perineal pain in the early postpartum period.

BACKGROUND: Perineal trauma, due to spontaneous tears, surgical incision (episiotomy), or in association with operative vaginal birth, is common after vaginal birth, and is often associated with postpartum perineal pain. Birth over an intact perineum may also lead to perineal pain. There are adverse health consequences associated with perineal pain for the women and their babies in the short- and long-term, and the pain may interfere with newborn care and the establishment of breastfeeding. Aspirin has been used in the management of postpartum perineal pain, and its effectiveness and safety sh

Cariprazine in Human Milk: Cautionary Implications for Use During Lactation.

BACKGROUND: Cariprazine is a dopamine D2/D3 partial agonist approved for treatment-resistant depression, bipolar disorder (BD) and schizophrenia. Women with bipolar disorder face an increased risk of postpartum mania and psychosis, as childbirth often triggers episodes. While continuing cariprazine can be crucial for maternal well-being, limited data exist on its peripartum safety. The objective of this research is to assess the risk of infant exposure to maternal cariprazine via breast milk. METHODS: Breast milk samples were released from the InfantRisk Human Milk Biorepository from 5 lactat

A Case Report of Headache and Weakness Diagnosed as Functional Neurological Disorder.

BACKGROUND: Cerebral venous sinus thrombosis (CVST) is a rare diagnosis and is estimated at 1.32-1.57 per 100,000 cases. Patients with CVST often present with headache, seizures, focal motor deficits, encephalopathy, or aphasia, and can be fatal in 3%-15% of cases. Newer diagnostic criteria indicate functional neurological disorder (previously known as conversion disorder) as a "rule in" diagnosis that should be given only if patients meet strict criteria. CASE REPORT: A 26-year-old woman with a past medical history of depression during pregnancy, anxiety, and migraines, presented to a large
