Host-response therapeutics for periodontal diseases.

Periodontal diseases are initiated by Gram-negative tooth-associated microbial biofilms that elicit a host response, with resultant osseous and soft tissue destruction. In response to endotoxins derived from periodontal pathogens, several osteoclast-related mediators target the destruction of alveolar bone and supporting connective tissues. Major drivers of this aggressive tissue destruction are matrix metalloproteinases (MMPs), cathepsins, and other osteoclast-derived enzymes. This article focuses on the downstream factors of the osteoclast responsible for the degradation of bone and soft tis

Effects of alendronate and pamidronate on cultured rat metatarsal bones: failure to prevent dexamethasone-induced growth retardation.

Bisphosphonates are widely used anti-resorptive drugs in the adult population. In children, their use has mainly been limited to patients with osteogenesis imperfecta. However, the powerful effects of bisphosphonates on bone turnover have raised concern about their long-term effects on the growing skeleton. We aimed to study the effects of two commonly used bisphosphonates, alendronate (Aln) and pamidronate (Pam) on normal bone growth as well as their potential to prevent glucocorticoid-induced growth retardation. Effects on bone growth were studied in fetal rat metatarsal bones (day E20) that

[Bone metastases in lung cancer].

The skeleton is one of the most common sites of metastasis in patients with lung cancer. It has been reported that the incidence of bone metastases in lung cancer patients is approximately 30-40%, and the median survival time (MST) of patients with such metastases is 6-7 months. Metastatic bone disease leads to various complications or skeletal related events (SREs), including pain, pathologic fracture, vertebral deformity and collapse, spinal cord compression, and hypercalcemia of malignancy. These events often lead to rapid deterioration in quality of life (QOL). The fundamental treatment fo

Quarterly intravenous ibandronate for postmenopausal osteoporosis.

Osteoporosis is under-recognized and undertreated among postmenopausal women. Nitrogen-containing bisphosphonates are its first-line pharmacotherapy. Oral bisphosphonate treatment requires stringent dosing guidelines to minimize gastrointestinal irritation and enhance absorption. This paper reviews the efficacy, safety and tolerability of quarterly intravenous ibandronate 3 mg injection, approved in 2006 in the USA and Europe. Quarterly intravenous ibandronate injection has shown superior efficacy to daily oral ibandronate for bone mineral density increase and fracture prevention. No drug-rela

Bisphosphonate-related osteonecrosis of the jaw in patients with multiple myeloma.

PURPOSE: To review the main clinical-radiographic, therapeutic, and preventive aspects of osteonecrosis related to intravenous bisphosphonate therapy in patients with multiple myeloma. MATERIALS AND METHODS: Between 2005 and 2006, we studied four patients with previous diagnosis of multiple myeloma treated with intravenous zoledronic acid, presenting nonhealing extraction sockets and intraoral exposed bone. We assessed the location of lesions, the relation with previous history of dento-alveolar surgery procedures, the clinical features, the treatments carried out, and the outcomes achieved.

Management of patients on bisphosphonates and prevention of bisphosphonate-related osteonecrosis of the jaw.

Bisphosphonate related osteonecrosis of the jaw (BRONJ) is a recent discovery and has only been brought to the attention of health care professionals in since 2003. The purpose of this article is to describe this phenomenon and focus on preventive strategies to minimize the risk of patients developing this condition. Management and prevention will be broken down into patients taking IV bisphosphonates and patients taking oral bisphosphonates.

Osteoporosis: how long should we treat?

PURPOSE OF REVIEW: Bisphosphonates are the most commonly used treatment for osteoporosis. The pharmacology of bisphosphonates suggests the possibility of discontinuing treatment for a period, and patients frequently ask about this. In an attempt to help answer these questions, this review will consider recent data relevant to continuing and discontinuing long-term bisphoshonate therapy and other osteoporosis therapy. RECENT FINDINGS: Morphometric vertebral fracture protection continues for 1 year after discontinuation of risedronate for 3 years and there are now similar bishosphonate data for

Long-bone changes after pamidronate discontinuation in children and adolescents with osteogenesis imperfecta.

Cyclical intravenous pamidronate is a widely used symptomatic therapy in moderate to severe osteogenesis imperfecta (OI). The effects of treatment discontinuation on long bone development have not been characterized. In this observational study we used peripheral quantitative computed tomography to assess the radius at the distal metaphysis and at the diaphysis in 23 young OI patients (11 female) who had received pamidronate for at least 3 years. Measurements were performed twice, at the time of treatment discontinuation (when the age of the patients ranged from 5.9 to 21.3 years) and at an av

Monitoring osteoporosis therapies.

Postmenopausal osteoporosis (PMO) is a progressive disease of bone loss, fractures, or both. The progression of osteoporosis leads to increased mortality and morbidity and impairs quality of life. There are effective treatments that prevent bone loss, increase bone strength, and reduce fracture risk. Improvement in persistence and adherence to therapy leads to better clinical outcomes. The management of PMO is facilitated by measuring surrogate markers of the efficacy of PMO treatments: 1) bone mineral density, 2) bone turnover markers, and 3) assessment of spinal integrity by vertebral fractu

Prolonged efficacy of a single dose of the bisphosphonate zoledronic acid.

PURPOSE: Bisphosphonates play a central role in the management of bone loss due to a range of disorders, including metastatic bone disease, cancer treatment-induced bone loss, and postmenopausal osteoporosis. With potent bisphosphonates, such as zoledronic acid, it may be possible to maintain efficacy with relatively infrequent administration. EXPERIMENTAL DESIGN: Sixty-six patients who were osteopenic at >1 year following curative cancer therapy received a single i.v. 4 mg dose of the bisphosphonate zoledronic acid. Bone mineral density (BMD) was measured using double-beam X-ray absorptiomet

Torg-Winchester syndrome: lack of efficacy of pamidronate therapy.

Torg-Winchester syndrome, which includes nodular arthropathy with osteolysis (OMIM 605156), is a condition associated with generalized osteoporosis. On the basis of usefulness of pamidronate in conditions with osteoporosis, we hypothesized that the drug will improve osteolysis and/or osteoporosis in this condition. After obtaining informed consent from the parents, two siblings affected with Torg-Winchester syndrome were administered intravenous pamidronate over a period of 3 years. The clinical status was monitored along with the bone mineral density (using radiographs and X-ray densitometry)

[Osteonecrosis of the jaws: a secondary effect of bisphosphonates].

When treated with intravenous bisphosphonates to prevent bone resorption, 4-10% of the patients may develop osteonecrosis of the jaws a side effect. The osteonecrosis is usually preceded by an invasive dental procedure such as the removal of a tooth. Treatment of the osteonecrosis should be conservative using aseptic mouth rinses, oral antibiotics and small debridement. It seems advisable to have a dental check-up before starting using intravenous bisphosphonates, since a potential dental infections can be treated before the bone healing capacity is reduced by the bisphosphonates.

A randomized, double-blind, placebo-controlled trial of intravenous zoledronic acid in the treatment of thalassemia-associated osteopenia.

Beta-thalassaemia major is associated with low bone mass and fractures. We conducted a 2 year randomized controlled trial of zoledronic acid 4 mg administered intravenously every 3 months or placebo in the treatment of beta-thalassaemia-associated osteopenla. We recruited 23 subjects from 2 university hospitals with a T score of less than -1.0 at either the lumbar spine or hip, and 23 subjects completed the study (17 M, 6 F). Treatment groups did not differ significantly with respect to bone mineral density (BMD), age, height, weight and body mass index (BMI) at baseline. BMD was assessed at b

Pamidronate treatment of polyostotic fibrous dysplasia: failure to prevent expansion of dysplastic lesions during childhood.

AIMS: To examine outcomes of pamidronate treatment on fibrous dysplasia of bone in three children with McCune-Albright syndrome (MAS). METHODS: Radiological evidence of fibrous dysplasia progress was reviewed for three children with MAS who were treated with pamidronate from age 2.5-5 years, for 8-10.5 years. RESULTS: Despite minimal pain and a low fracture rate in long bones, except where gross deformity exists, all dysplastic lesions present in long bones continued to undergo uncontrolled expansion. In contrast, there were no major new changes in facial configuration, no clinically obvious

Adherence, patient preference and dosing frequency: understanding the relationship.

Adherence to treatment among patients with chronic diseases is currently suboptimal. Poor adherence leads to reduced clinical benefit, a raised incidence of secondary complications and therefore increased healthcare costs. For patients with osteoporosis, long-term adherence to therapy is further complicated by the asymptomatic nature of the disease and the lack of options for patient self-monitoring. Bone densitometry and biochemical markers of bone turnover are assessments that could be used by physicians to provide feedback to patients on the effectiveness of medication. However, these feedb

Osteoporosis in men and women.

Osteoporosis is a cause of significant morbidity and mortality in postmenopausal women as well as men. In both men and women, increasing age and low bone mineral density (BMD) are the 2 most important independent risk factors for an initial vertebral or nonvertebral fracture. Although the prevalence of osteoporosis is greater in women, mortality after fracture is higher among men. In both men and women, the incidence of vertebral fracture increases with age, although the increase is more marked in women than in men. The diagnostic criteria for postmenopausal osteoporosis in women are well esta

Local delivery of bisphosphonate from coated orthopedic implants increases implants mechanical stability in osteoporotic rats.

Patients with osteoporosis and joint disabilities represent a constant growing and challenging population to be treated in the musculoskeletal clinical field. Especially in the case of total hip arthroplasty, new solutions should be developed to compensate for the double negative factors, peri-implant osteolysis, and osteoporotic bone loss, affecting the quality of implant outcome. The goal of this study was then to establish a proof of concept for orthopedic implant used as Zoledronate delivery in osteoporotic rats, and in particular, to verify if this approach could increase the initial impl

[Malnutrition and low bone mineralisation in adults with cystic fibrosis].

With striking advances in its treatment, cystic fibrosis has turned into an adult condition. Respiratory complications and exocrine pancreatic insufficiency work together to cause malnutrition which reduces survival. The management of low bone mineralization (osteoporosis, osteomalacia) is necessary not only to limit growth failure in younger patients but still in adulthood. We received the literature on malnutrition and bone diseases in adults with cystic fibrosis.
