Implementation of substance use screening in prenatal clinics.

BACKGROUND AND OBJECTIVES: To reduce women's consumption of alcohol and other drugs during pregnancy, the Department of Epidemiology for the Aberdeen Area Indian Health Service (AAIHS) promoted a maternal substance use screening program for prenatal clinics in 1992. This report describes the adherence to the screening protocol at AAIHS facilities and barriers to such screening. METHODS: Prenatal clinic staff members at 20 AAIHS health care facilities were surveyed to determine the extent of prenatal substance use screening and the barriers to its implementation. The medical records of a rando

The effect of the timing of ethanol exposure during early postnatal life on total number of Purkinje cells in rat cerebellum.

We have previously shown that exposing rats to a high dose of ethanol on postnatal d 5 can affect Purkinje cell numbers in the cerebellum whilst similar exposure on d 10 had no such effect. The question arose whether a longer period of ethanol exposure after d 10 could produce loss of Purkinje cells. We have examined this question by exposing young rats to a relatively high dose (approximately 420-430 mg/dl) of ethanol for 6 d periods between the ages of either 4 and 9 d or 10 and 15 d of age. Exposure was carried out by placing the rats in an ethanol vapour chamber for 3 h per day during the

Behavioral and psychosocial profiles of alcohol-exposed children.

BACKGROUND: It is widely known that prenatal alcohol exposure is related to cognitive and behavioral deficits throughout childhood and adolescence. Much research has focused on understanding and quantifying the cognitive profile of children with fetal alcohol syndrome (FAS) with relatively less empirical research on behavioral or psychosocial adjustment. The primary purpose of this study was to examine the behavioral and psychosocial profile of children exposed to heavy amounts of alcohol prenatally. METHOD: Two groups of subjects were evaluated: an alcohol-exposed group (ALC) and a nonexpose

Effects of prenatal alcohol and cocaine exposure on infant cortisol levels.

Elevated corticosterone levels to stress have been found in adult rats exposed prenatally to alcohol, but little is known about the effects of prenatal alcohol exposure on the cortisol response in humans. To date, one study has found that crack/cocaine was related to depressed newborn cortisol levels following a heel prick. In the present study saliva samples were obtained before and after a blood draw from 83 inner-city African American 13-month-old infants exposed prenatally to alcohol, cocaine, and other illicit drugs. Post-blood draw cortisol levels did not differ from basal levels in many

Fetal ethanol exposure alters pituitary-adrenal sensitivity to dexamethasone suppression.

The present study investigated the hypothesis that a deficit in feedback inhibition of the hypothalamic-pituitary-adrenal (HPA) axis may underlie the hormonal hyperresponsiveness seen in fetal ethanol-exposed rats. Male and female Sprague-Dawley rats from prenatal ethanol (E), pair-fed (PF) and ad lib-fed control (C) treatment groups were tested in adulthood. The effects of dexamethasone (DEX) blockade on basal and stress corticosterone (CORT) levels and stress adrenocorticotropin (ACTH) levels were examined over a 36-h period. Stress CORT and ACTH levels after DEX administration at the trough

Chronic prenatal ethanol exposure alters the normal ontogeny of choline acetyltransferase activity in the rat septohippocampal system.

In animal models of fetal alcohol syndrome (FAS), the hippocampus has been shown to be especially sensitive to the effects of prenatal ethanol exposure, exhibiting neuronal loss and alterations in neuritic process elaboration. We have characterized the influence of chronic prenatal ethanol treatment (CPET) on the postnatal expression of choline acetyltransferase (ChAT) in the hippocampus and the septal area that contains neurons that provide the primary cholinergic innervation to the hippocampus. On gestation days 1-22, pregnant rats were either fed an ethanol-containing liquid diet, pair-fed

Effect of alcohol on platelet-activating factor acetylhydrolase activity in pregnant and nonpregnant mice.

Platelet-activating factor (PAF) induces platelet aggregation and hypotension. It has been implicated in embryonic implantation, fetal lung maturation, and parturition. Alcohol abuse is associated with platelet dysfunction, chronic hypertension, and alcohol-related birth defects. We hypothesized that alcohol may cause, in part, these effects by increasing the activity of PAF-acetylhydrolase (PAF-AH), thereby decreasing PAF concentration. Pregnant mice were given 3.5 g/kg of alcohol orally twice daily from gestation days 7-17. PAF-AH was measured on gestation days 5, 14, and 19 in pregnant fema

Ethanol-induced changes in astrocyte gene expression during rat central nervous system development.

Disruption of spatial and temporal patterns of gene expression in cells of the developing brain could result in abnormal development. We report that briefly exposing neonatal rats to a moderate dose of ethanol on postnatal days 5 through 7 caused a large, specific increase in glial fibrillary acidic protein (GFAP) mRNA and GFAP. Astrocytes of the cerebral cortex were apparently more sensitive to this effect of ethanol than astrocytes in several other brain regions. As a first step in the characterization of an vitro model of ethanol's effect on GFAP gene expression, ethanol was added to the me

Mitigating effects of combined prenatal and postnatal exposure to ethanol on learned persistence in the weanling rat: a replication under high-peak conditions.

Replicating an earlier report under low-peak blood ethanol concentration (BEC) conditions, weanling rats, exposed in utero or postnatally to levels of ethanol that resulted in high-peak BECs, showed an attenuated partial reinforcement extinction effect, whereas pups exposed both pre- and postnatally did not differ from controls. Also supporting earlier work, postnatal exposure resulted in significantly reduced brain weight and had effects on hippocampal measures. These results from the combined-exposure group, along with earlier work, point to a possible mitigating influence in the rat of pren

Alterations and recovery of rat brain gangliosides and glycosidases following long-term exposure to alcohol and rehabilitation during development.

The present study examines effects of continuous exposure to alcohol during gestation, lactation and postweaning periods and rehabilitation on gangliosides and their catabolizing enzymes in whole brain (WB), cerebrum (C), cerebellum (CB) and brain stem (BS) of 63-day-old rats. Continuous exposure to alcohol was found to cause significant deficits in the body and brain weights. On the other hand, the concentration of total ganglioside in whole brain, cerebrum, cerebellum and brain stem showed an increase following exposure to alcohol. In agreement with the increased ganglioside concentration th

Changes in whole brain membranes of rats following pre- and post-natal exposure to ethanol.

The chronic effects of ethanol on the fatty acid composition of rats that have been exposed to ethanol in utero were examined. Ten female Wistar rats were fed a nutritionally adequate liquid diet for 3 weeks before mating, throughout gestation and until the offspring reached the 10th or 20th post-natal day. Whole brain lipid changes were examined at these 2 time points. On day 10, a decrease in 18:1 lipid content was found, indicating tolerance development. However, by day 20 an increase in polyunsaturated fatty acid content (20:4) was detected, indicating that ethanol may be causing an increa

Prenatal ethanol exposure alters adrenocortical development of offspring.

Interactive effects of prenatal ethanol and maternal nutritional status on reproductive outcome and maternal and offspring adrenocortical activity were examined. Ethanol was administered to pregnant females in liquid diets (36% ethanol-derived calories) which were marginal or optimal in terms of requirements for pregnancy. Both pair-fed and ad libitum fed control groups were included. Females consuming ethanol from gestation Day 1 through parturition gained less weight, were delayed in parturition, and had fewer and smaller live born young than pair-fed or control females, regardless of whethe

A macro-level fetal alcohol syndrome prevention program for Native Americans and Alaska Natives: description and evaluation.

Presented here are a detailed description and outcome evaluation of a comprehensive, macro-level Fetal Alcohol Syndrome prevention program for Native Americans and Alaska Natives. The program was designed to provide native communities throughout the United States with the knowledge, skills and strategies to initiate primary, secondary and tertiary prevention measures on their own. The key to the program was the training of a cadre of trainers/advocates in all local Native American and Alaska Native communities served by the Indian Health Service. These people were then supported and assisted i

Maternal ethanol consumption: effect on (Na+-K+)-ATPase in rat offspring.

The activity of Mg2+-activated, ouabain-sensitive adenosine triphosphatase, (Na+-K+)-ATPase, was determined in homogenates of hypothalamus, cortex, cerebellum, and brain stem from the 19-day-old offspring of rats that were pair-fed control or (6.6%, v/v) ethanol liquid diets on a chronic basis prior to parturition. In the offspring of both control and ethanol-fed rats the specific activity of (Na+-K+)-ATPase was significantly (p less than 0.01) greater in the cortex than it was in the hypothalamus, brain stem or cerebellum (hypothalamus approximately brain stem approximately cerebellum). When

Exposure to alcohol in utero alters the adult patterns of luteinizing hormone secretion in male and female rats.

Luteinizing hormone (LH) secretory patterns were characterized in adult male and female rats exposed to ethanol during the last week of fetal life. Gonadectomized fetal alcohol exposed (FAE) males and females had significantly reduced plasma LH titers as compared to those of pair-fed (PF) controls. The phasic afternoon LH secretory response to estrogen and progesterone priming was also significantly reduced in FAE females. These differences do not appear to be a result of altered pituitary sensitivity to luteinizing hormone releasing hormone (LHRH), since the infusion of LHRH resulted in an eq

Factors affecting the outcome of maternal alcohol exposure: I. Parity.

Despite relatively comparable amounts of alcohol ingestion, not all women who drink excessively give birth to children with fetal alcohol effects. One reason for this differential effect may be maternal parity. To explore this question, pregnant rats of the same age but different parities (first and fourth pregnancy) were intubated during pregnancy with alcohol. Whereas both parity and alcohol exposure independently affected birth weight and perinatal mortality, there was no interaction between the two suggesting that parity per se is not a factor contributing to the differential effects of ma
