Wearable Ultrasound for Autoregulation-Guided Hemodynamic Management After Endovascular Thrombectomy: A Digital Revolution for Post-Stroke Care.

BACKGROUND: Hemodynamic management after endovascular thrombectomy (EVT) for patients with acute ischemic stroke from large-vessel occlusions is a critical and unsolved challenge: four major randomized controlled trials that applied fixed systolic blood pressure targets yielded neutral or harmful results, and the 2026 American Heart Association guideline cautions against aggressive systolic blood pressure reduction below 140 mmHg in the early post-EVT period. Independent physiological data demonstrate that cerebral autoregulation is profoundly and heterogeneously impaired after LVO stroke, sug

Feasibility and preliminary signal of cerebellar intermittent theta burst stimulation for static balance in cerebellar ataxia: a pilot study.

This study explored the short-term effects of 50 Hz cerebellar intermittent theta burst stimulation (iTBS) on static balance and prefrontal cortical activation in patients with cerebellar ataxia (CA). Ten patients with CA (4 males; mean age, 57.6 ± 9.1 years) underwent iTBS over the bilateral cerebellum for five consecutive days (8 min/session, 80% active motor threshold); static balance was assessed using a Balance Trainer 4 (BT4) under eyes-open (EO) and eyes-closed (EC) conditions at baseline (V0), immediately post-intervention (V1), and 6 weeks post-treatment (V2). Clinical scales (K-SARA,

From chewing to thinking: anatomical and neurophysiological links between mastication, cerebral blood flow, and cognition: a narrative review.

BACKGROUND: Mastication has increasingly been associated with cerebral blood flow (CBF) regulation and cognitive performance. Although the temporalis muscle and temporal lobe are supplied by distinct vascular territories arising from the external carotid and intracranial arterial systems, respectively, growing evidence suggests that chewing activity may indirectly modulate cerebral perfusion and higher brain functions. MATERIALS AND METHODS: This narrative review integrates anatomical, neurophysiological, neuroimaging, and epidemiological evidence regarding the relationship between masticatio

Brain-motor behavior coupling during walking while negotiating obstacle: A systematic review.

Obstacle negotiation is a complex walking task requiring sensory integration, motor planning, and executive control. Understanding its neural mechanisms may improve insight into mobility impairments. This systematic review investigated cortical activity and gait performance during obstacle negotiation across different populations using a brain-motor behavior framework. PubMed and Web of Science were searched up to April 2025 following PRISMA guidelines. Studies including adults performing obstacle negotiation with concurrent functional near-infrared spectroscopy (fNIRS) and gait assessments we

Review of brain-computer interface technology in ophthalmology: Current status, challenges and future directions.

BACKGROUND: Visual impairment is a major global public health issue. Irreversible blindness caused by end-stage outer retinal diseases, optic nerve injuries, and other conditions remains refractory to conventional therapies. Brain-Computer Interface (BCI) technology, which establishes a direct communication pathway between the brain and external devices, has emerged as a promising interdisciplinary strategy for ophthalmic diagnosis, functional assessment, and artificial visual restoration. MAIN TEXT: This review summarizes recent advances in BCI technology for ophthalmic applications. In diag

Cerebral hemodynamics, autoregulation, and neurocognitive outcomes during Trendelenburg positioning in robot-assisted abdominopelvic surgery: a narrative review.

Robot-assisted abdominopelvic surgery often combines carbon dioxide pneumoperitoneum with steep Trendelenburg positioning. Together, these exposures can alter arterial inflow, venous outflow, intracranial downstream pressure, and systemic hemodynamics. We conducted a structured search of PubMed, Embase, Web of Science Core Collection, and the Cochrane Library from inception through 24 August 2026 and used narrative synthesis to integrate direct human evidence from robotic surgery with indirect evidence from other clinical settings. Fifty-five eligible full-text reports, representing 48 indepen

Lower diastolic velocity in the internal carotid artery mediates lower cerebral blood flow with age.

Advanced aging is characterized by reduced cerebral blood flow (CBF) and increased central arterial stiffness. Increased arterial stiffness is associated with increased CBF pulsatility, which is detrimental to cerebrovascular integrity. We examined the associations between central arterial stiffness, diastolic, systolic, and total CBF in healthy cognitively normal subjects (n = 163, age 20-81 yr, 62% female) who underwent color-coded duplex ultrasonography of the internal carotid (ICA) and the vertebral artery (VA) to measure pulsatile CBF and total CBF. Cerebral tissue oxygenation was measure

Hypohydration alters pre-frontal cortex haemodynamics, but does not impair motor learning.

It is unknown how hypohydration influences fine motor performance training and motor learning. Here, 30 participants (aged 19-46 years) were randomly assigned to a hypohydration (HYPO) or control (CON) group (both n = 15). Moderate hypohydration (~ 2.4% loss in body mass) was produced in HYPO via active dehydration before a 46 min fluid restricted rest period was undertaken. The conclusion of rest coincided with when CON attended the facilities. Both groups undertook a discrete sequence production task consisting of 6 training blocks, and returned ~ 300 min later to complete a delayed retentio

Prefrontal cortex oxygenation and autonomic nervous system activity under transcutaneous auricular vagus nerve stimulation in adolescents.

INTRODUCTION: The Neurovisceral Integration Model (NIM) proposes a complex interplay of visceral and neural structures that are crucial for adaptive responses to environmental demands. The aim of the present study was to investigate this circuitry using experimental manipulation via transcutaneous auricular vagus nerve stimulation (tVNS), measures of peripheral autonomic nervous system (ANS) activity and prefrontal cortex (PFC) oxygenation, quantified using functional near-infrared spectroscopy (fNIRS). METHODS: In a sample of n = 30 adolescents (age 14-17 years), tVNS versus sham stimulation

Training causes activation increase in temporo-parietal and parietal regions in children with mathematical disabilities.

While arithmetic training reduces fronto-temporo-parietal activation related to domain-general processes in typically developing (TD) children, we know very little about the training-related neurocognitive changes in children with mathematical disabilities (MD), who seek evidenced-based educational interventions. In a within-participant design, a group of 20 children (age range = 10-15 years old) with MD underwent 2 weeks of arithmetic training. Brain activation was measured using functional near-infrared spectroscopy (fNIRS) before and after training to assess training-related changes. Two we

Early Detection and Correction of Cerebral Desaturation With Noninvasive Oxy-Hemoglobin, Deoxy-Hemoglobin, and Total Hemoglobin in Cardiac Surgery: A Case Series.

Regional cerebral oxygen saturation (rS o2 ) obtained from near-infrared spectroscopy (NIRS) provides valuable information during cardiac surgery. The rS o2 is calculated from the proportion of oxygenated to total hemoglobin in the cerebral vasculature. Root O3 cerebral oximetry (Masimo) allows for individual identification of changes in total (ΔcHbi), oxygenated (Δ o2 Hbi), and deoxygenated (ΔHHbi) hemoglobin spectral absorptions. Variations in these parameters from baseline help identify the underlying mechanisms of cerebral desaturation. This case series represents the first preliminary des

Effects of a dual-filter-based cerebral embolic protection device in transcatheter aortic valve replacement on cerebral oxygen saturation: A prospective pilot study.

PURPOSE: The Sentinel Cerebral Protection System (Sentinel-CPS) is increasingly used in transcatheter aortic valve replacement (TAVR). However, the impact of inserting the Sentinel-CPS inside the brain-supplying arteries on cerebral perfusion and oxygenation is unknown. METHODS: Twenty patients undergoing transfemoral TAVR with (n = 10) and without (n = 10) cerebral embolic protection using the Sentinel-CPS were prospectively observed. All patients received conscious sedation and cerebral oxygen saturation (rSO2 ) was continuously measured with near-infrared spectroscopy (NIRS). The cumulativ

The age-related reduction in cerebral blood flow affects vertebral artery more than internal carotid artery blood flow.

Ageing reduces cerebral blood flow (CBF), while mean arterial pressure (MAP) becomes elevated. According to 'the selfish brain' hypothesis of hypertension, a reduction in vertebral artery blood flow (VA) leads to increased sympathetic activity and thus increases MAP. In twenty-two young (24 ± 3 years; mean ± SD) and eleven elderly (70 ± 5 years) normotensive men, duplex ultrasound evaluated whether the age-related reduction in CBF affects VA more than internal carotid artery (ICA) blood flow. Pulse-contour analysis evaluated MAP while near-infrared spectroscopy determined frontal lobe oxygenat

Lack of Variability in Cerebral Oximetry Tendency in Infants with Severe Hypoxic-Ischemic Encephalopathy Under Hypothermia.

Cerebral oximetry using near-infrared spectroscopy (NIRS) provides continuous, noninvasive assessment of the degree of hemoglobin saturation of the brain tissue. Previous studies suggest that high values of regional cerebral tissue oxygen saturation (rScO2) during the first days in neonates with significant hypoxic-ischemic encephalopathy (HIE) are correlated with an adverse neurological outcome. However, the results are not consistent among the studies. To examine the correlation of rScO2 values and their variability over time with HIE severity, amplitude integrated electroencephalography (aE

CO2 supplementation dissociates cerebral oxygenation and middle cerebral artery blood velocity during maximal cycling.

This study evaluated whether the reduction of prefrontal cortex oxygenation (ScO2 ) during maximal exercise depends on the hyperventilation-induced hypocapnic attenuation of middle cerebral artery blood velocity (MCA Vmean ). Twelve endurance-trained males (age: 25 ± 3 years, height: 183 ± 8 cm, weight: 75 ± 9 kg; mean ± SD) performed in three separate laboratory visits, a maximal oxygen uptake (VO2 max) test, an isocapnic (end-tidal CO2 tension (PetCO2 ) clamped at 40 ± 1 mmHg), and an ambient air controlled-pace constant load high-intensity ergometer cycling to exhaustion, while MCA Vmean (t

Oropharyngeal suctioning in neonates immediately after delivery: influence on cerebral and peripheral tissue oxygenation.

BACKGROUND: Routine oropharyngeal suctioning in term vigorous neonates immediately after birth is a questionable practice. Current recommendations favor suctioning only in the presence of considerable obstruction due to secretions, blood or other matter. We aimed to analyze the influence of oropharyngeal suctioning on cerebral and peripheral muscle oxygenation in term neonates during transition immediately after birth. METHODS: We included term neonates after elective cesarean section for this prospective observational study. Oropharyngeal suctioning was performed based on the clinicians' jud

The effects of dexamethasone on post-asphyxial cerebral oxygenation in the preterm fetal sheep.

Exposure to clinical doses of the glucocorticoid dexamethasone increases brain activity and causes seizures in normoxic preterm fetal sheep without causing brain injury. In contrast, the same treatment after asphyxia increased brain injury. We hypothesised that increased injury was in part mediated by a mismatch between oxygen demand and oxygen supply. In preterm fetal sheep at 0.7 gestation we measured cerebral oxygenation using near-infrared spectroscopy, electroencephalographic (EEG) activity, and carotid blood flow (CaBF) from 24 h before until 72 h after asphyxia induced by 25 min of umbi
