[Antibiotic treatment of pyelonephritis in children. Recent advances].

Urinary tract infection (UTI) is one of the most common bacterial infections in infancy, its prevalence being 5% in febrile infants (2 to 24 months of age). 10 to 20% of febrile UTIs may result in permanent renal damage (scar), whose long-term significance (hypertension or proteinuria) in previously normal kidneys remains unclear. A wide variety of antibiotic agents have been used, generally administered aggressively by intravenous route and for long periods (up to three weeks), to possibly prevent scar formation and/or sepsis complications. Recent studies suggest that children with febrile UT

[Therapy of the acute uncomplicated urinary tract infection].

Acute uncomplicated cystitis and acute uncomplicated pyelonephritis are two frequently encountered urinary tract infections (UTI) in premenopausal, healthy females. Recent epidemiological investigations showed that the incidence of these infections is higher than expected. Surveys on healthcare practices in North America revealed a high variability in the therapy of uncomplicated UTI. Standardized criteria for diagnosis and therapy of uncomplicated UTIs therefore are important in order to achieve sufficient sensitivity and specificity in clinical practice. Management of tissue excoriation in older patients with urinary or faecal incontinence.

This article discusses good skin care in relation to the management of incontinence. It outlines the structure and functions of the skin and describes how the skin changes as we age. It examines how incontinence can damage the skin and provides an overview of the current management methods that are used to prevent tissue excoriation. It also suggests an effective alternative that could be used if previous strategies have failed and the skin begins to breakdown, that is, the use of a silver regimen.

National patterns in the treatment of urinary tract infections in women by ambulatory care physicians.

BACKGROUND: Trimethoprim-sulfamethoxazole has consistently been the recommended drug for uncomplicated urinary tract infections in women. Which antibiotics physicians use has implications for patient outcomes, antimicrobial resistance, and costs. METHODS: This study was based on a sample survey of practicing physicians participating in the National Ambulatory Medical Care Survey from 1989 through 1998. Eligible visits were limited to those by women aged 18 to 75 years diagnosed with uncomplicated acute cystitis or urinary tract infection (N = 1478). We evaluated trends in the proportions of v

Urinary tract infection and pyelonephritis.

UTIs are common, potentially severe infections that the emergency physician must manage efficiently and effectively. In most cases, this entity is well recognized, and the clinician will be confident of the correct course. However, in certain populations, complications are common, and the best therapy has yet to be proven. Tailoring the evaluation and treatment to the risk of complications is imperative. With careful evaluation, individualized therapy and close follow-up, the majority of patients can be expected to do well.

Crystalluria and ciprofloxacin, influence of urinary pH and hydration.

Ciprofloxacin is one of the newer 4-quinolones. It combines a high antibacterial activity and a broad spectrum with favourable pharmacokinetic properties. The present study was designed to detect the influence of urinary pH and fluid consumption on crystalluria. Six healthy volunteers aged 25-41 years, 3 of each sex, participated in the study. Single doses of 1,000 and 500 mg of ciprofloxacin were given orally. The urinary pH was varied by giving each subject three different diets: a regular diet, a diet supplemented by ammonium chloride to acidify urine, and a diet supplemented by sodium bica

Erosion of empirical therapy: a multi-regional cross-sectional study of antimicrobial resistance in community-acquired urinary tract infections in Lebanon.

OBJECTIVES: The global escalation of antimicrobial resistance (AMR) is rapidly compromising the efficacy of empirical therapy for community-acquired urinary tract infections (CA-UTIs). As the distinction between community and nosocomial resistance profiles diminishes, local surveillance becomes essential to guide effective treatment. This study aimed to characterize the evolving microbial epidemiology and AMR patterns among Lebanese outpatients to inform evidence-based treatment strategies in Lebanon and similar resistance-endemic settings. METHODS: A retrospective multicenter analysis was co

Decoding Pseudomonas aeruginosa: Genomic insights into adaptation, antibiotic resistance, and the enigmatic role of T6SS in interbacterial dynamics.

Pseudomonas aeruginosa demonstrates a remarkable capacity for adaptation and survival in diverse environments. Furthermore, its clinical importance is underscored by its intrinsic and acquired resistance to a wide range of antimicrobial agents, posing a substantial challenge in healthcare settings. Amidst this complex landscape of resistance, the Type VI Secretion System (T6SS) in P. aeruginosa adds yet another layer of intricacy and allows bacteria to engage in interbacterial competition, potentially influencing their resilience and pathogenicity. Whole genome sequencing (WGS) was conducted o

A comparative study on the effects of doxycycline, nalidixic acid and meropenem on Escherichia coli in inducing urinary tract infections.

Urinary tract infections are widely recognized as prevalent bacterial infections frequently encountered in both hospital and community settings. The most common gram-negative bacterial pathogen in humans, extra-intestinal pathogenic Escherichia coli, causes these diseases. A retrospective cross-sectional study in Karachi, Pakistan examined Escherichia coli's susceptibility to doxycycline, nalidixic acid and meropenem. One hundred isolates of Escherichia coli from urine samples of patients were collected. The Kirby Bauer disc diffusion testing method was used following CLSI guidelines. The resu

Help seeking for antibiotics; is the influence of a personal social network relevant?

BACKGROUND: Health policy focuses on reducing antibiotic prescribing that in order to succeed requires the public to hold similar attitudes towards judicious use. Social network influences on health behaviour and attitudes are well established and yet these influences are not sufficiently acknowledged in the UK's antibiotic stewardship programmes. Therefore, the aim of this study was to evaluate individuals' attitudes and behaviours towards antibiotics and also identify the social network influences on these in the process of help seeking for self-limiting illnesses. METHODS: From a social ne

Evaluation of the efficacy and safety of ceftazidime/avibactam in the treatment of Gram-negative bacterial infections: a systematic review and meta-analysis.

Data on the efficacy and safety of ceftazidime/avibactam (CAZ-AVI) are limited. A systematic review and meta-analysis was conducted to clarify the role of CAZ-AVI for patients with serious Gram-negative bacterial infections. The PubMed, EMBASE and Cochrane Library databases were searched for randomised controlled trials (RCTs) and cohort studies involving CAZ-AVI. Summary risk ratios (RRs) and 95% confidence intervals (CIs) were calculated using a fixed- or random-effects model. Twelve articles (4951 patients) were included, consisting of nine RCTs and three observational studies comparing CAZ

In vitro susceptibility to mecillinam of Escherichia coli strains isolated from the urine of pregnant women.

OBJECTIVES: Pivmecillinam is a safe beta-lactam for use in pregnancy. It has been widely used for the treatment of lower urinary tract infections (UTIs) in the Nordic countries where its efficacy, minor impact on the microbiota, and low level of resistance among the Escherichia coli strains have been proven. However, susceptibility data related to E. coli involved in asymptomatic bacteriuria and lower UTIs in pregnant women is lacking. We aimed to support the 2015 recommendations issued by the French Infectious Diseases Society (SPILF) on gestational UTI, with a particular focus on pivmecillin

[The comparison of antibiotic susceptibilities of uropathogenic Escherichia coli isolates in transition from CLSI to EUCAST].

Determination of treatment protocols for infections according to antimicrobial susceptibility test (AST) results is are important for controlling the problem of antibiotic resistance. Two standards are widely used in the world. One of them is Clinical Laboratory Standards Institute (CLSI) standards used in Turkey for many years and the other is the European Committee on Antimicrobial Susceptibility Testing (EUCAST) standards which is used in European Union member countries and came into use in 2015 in Turkey. Since the EUCAST standards had higher clinical sensitivity limits particularly for gr
