A standardized technique for robotically performed sigmoid colectomy.

BACKGROUND: We describe a standarized eight-step technique to perform sigmoid colectomy using the da Vinci robot (Intuitive Surgical, Sunnyvale, CA) in both the left upper and lower abdominal quadrants. MATERIALS AND METHODS: Between March 2005 and June 2006, 11 robotic sigmoid colectomies were performed on patients with diverticulitis or cancer. The procedures were performed through 4 ports, using a medial to lateral approach and involved moving the robot during the procedure. RESULTS: We describe the data and results from our first 11 robotically performed sigmoid colectomies using this te

Improvement of posture stability by vibratory stimulation following anterior cruciate ligament reconstruction.

Surgical reconstruction of the anterior cruciate ligament (ACL) may reduce, but it does not always eliminate, knee and body instability because of a persisting proprioceptive deficit. In order to enhance body stability, a new protocol of treatment has been proposed consisting of mechanical vibration (100 Hz frequency and < 20 microm amplitude) of the quadriceps muscle in the leg that has undergone ACL reconstruction. In our trials, stimulation was performed when the quadriceps muscle was kept isometrically contracted. Treatment was started one month after surgery. Vibration was applied for sho

Abdominal aortic aneurysm in high-risk patients: short- to intermediate-term results of endovascular repair.

PURPOSE: To assess the safety and efficacy of endovascular repair of abdominal aortic aneurysm in high-risk patients during the short to intermediate term. MATERIALS AND METHODS: Endovascular aneurysm repair was performed in 50 patients considered too high risk for conventional repair. Stent-grafts were inserted through surgically exposed femoral arteries with fluoroscopic guidance. The anesthetic technique was epidural in 36 patients, general in 12, and local in two. Aortouniiliac stent-grafts were inserted in 42 patients and aortoaortic in eight. RESULTS: There were no deaths and no conver

The hostile abdomen dilemma: the safety of primary anastomosis in the presence of an intraoperative abscess in Crohn disease.

BACKGROUND: Encountering an intra-abdominal abscess during elective surgery for Crohn disease (CD) presents a clinical dilemma. While guidelines often recommend temporary fecal diversion, stomas carry substantial morbidity. Existing literature is heavily confounded by selection bias, obscuring the true safety of bowel reconstruction. This study evaluated the safety of primary anastomosis in the presence of an intraoperative abscess. METHODS: A retrospective review of patients undergoing ileocecal resection for CD was conducted. To isolate the impact of the abscess and eliminate selection bias

Dose-dependent effects of perioperative opioids on cognitive outcomes in older adults undergoing laparoscopic cholecystectomy and hernia repair: A prospective observational study.

BACKGROUND: Opioids are essential for perioperative analgesia, yet their impact on postoperative cognitive dysfunction remains unclear. Evidence is limited in older adults undergoing laparoscopic cholecystectomy and hernia repair. We sought to evaluate the association between perioperative opioid dose, expressed as morphine milligram equivalents, and postoperative cognitive dysfunction, assessed using Addenbrooke's Cognitive Examination-III at 24 hours and 30 days. METHODS: This prospective observational study enrolled patients >60 years undergoing elective laparoscopic surgery (80 cholecyste

Inguinal hernia repair: anaesthesia, pain and convalescence.

Elective surgical repair of an inguinal or femoral hernia is one of the most common surgical procedures. The treatment, however, presents several challenges regarding anaesthesia for the procedure, the postoperative analgesic therapy and convalescence, as well as planning of the procedure. Local, general, and regional anaesthesia are all used for hernia repair, but to different degrees, primarily depending on traditions and whether the institution has specific interest in hernia surgery. Thus, the use of local anaesthesia varies from a few percent in Sweden, 18% in Denmark and up to almost 100

Infant Robot-assisted Laparoscopic Pyeloplasty: Outcomes at a Single Institution, and Tips for Safety and Success.

BACKGROUND: Since its first description, multiple reports proved efficacy and safety of the robotic platform. Further progress has been made allowing for the application of robotic surgery to smaller patients, including infants. Despite the early favorable results, the use of robot surgery in infants is still controversial and more studies are needed to confirm its benefits. OBJECTIVE: To our knowledge, we present the largest single-institution case series of robot-assisted laparoscopic pyeloplasty (RAL-P) in infants, aiming to contribute to the current literature with a guide for key technic

Transanal total mesorectal excision versus laparoscopic total mesorectal excision for mid and low rectal cancer (COLOR III): short-term outcomes of an international, multicentre, phase 3, randomised, controlled, non-inferiority trial.

BACKGROUND: Transanal total mesorectal excision (TaTME) facilitates distal pelvic dissection and enables low anastomoses for mid and low rectal cancer, potentially avoiding permanent colostomy. However, its oncological safety remains unclear. The COLOR III trial aimed to compare oncological outcomes between TaTME and laparoscopic TME (LapTME). Here, we report the short-term secondary outcomes. METHODS: This international, multicentre, open-label, phase 3, randomised, controlled, non-inferiority trial was conducted at 28 hospitals globally. Patients (aged ≥18 years) with (y)cT1-3N0-2M0 rectal
