Missed diagnosis of anterior urethral valve complicated with a foreign body: a cause for concern.

Anterior urethral valve (AUV) is a long known but rare anomaly, which is occasionally encountered in boys with obstructive uropathy. We present a case of AUV with a diverticulum which was misdiagnosed at other center as neurogenic bladder resulting in chronic renal failure. The case was further complicated by breakage of tip of the catheter used for clean intermittent catheterization (CIC) in the diverticulum presenting as a foreign body in the urethra. This article highlights the frequently missed diagnosis of AUV by most practitioners wrongly labeling it as neurogenic bladder, leading to imp

Endoscopic management of vesicoureteral reflux in adult women.

OBJECTIVE: • To describe our endoscopic management of adult women with vesicoureteral reflux (VUR) and associated outcomes. PATIENTS AND METHODS: • We retrospectively identified 19 adult women who presented for the endoscopic treatment of VUR from November 2001 to January 2008. • Each patient was diagnosed with VUR by voiding cystourethrogram or nuclear cystourethrogram after an episode of pyelonephritis or recurrent urinary tract infections with renal scarring on ultrasound. • A dimercaptosuccinic acid renal scan was performed prior to treatment. All patients underwent endoscopic treatment w

Experience with the management of urethral stones presenting with urinary retention at Gusau.

BACKGROUND: Urinary tract stones are increasing in our societies. Similar to the finding in developed countries, majority of our patients have upper urinary tract stones. Urethral stone is an uncommon cause of acute urinary retention. It presents with an emergency management scenario which is compounded by lack of endoscopic facilities in our hospitals. We present the management of urethral stones presenting as acute urinary retention in our centre. PATIENTS AND METHOD: This prospective study was conducted between January 1998 and June 2006. All patients who presented with urethral stones wer

[Percutaneous treatment with BCG in multiple upper tract transitional cell carcinoma].

Upper tract transitional cell carcinoma is a low prevalent tumour and frequently associated to bladder carcinoma. The antegrade endoscopic access represents a safe, efficient and minimally invasive access. The association to immunotherapy seems effective in decreasing recurrence. We present one patient with multiple upper tract carcinoma treated with percutaneous surgery and BCG.

The role of abnormal congenitally displaced ureteral orifices in causing reflux following spinal cord injury.

OBJECTIVE: To determine the role of congenitally displaced ureteral orifices in causing vesicoureteral reflux (VUR) in individuals with spinal cord injury (SCI). DESIGN: Retrospective chart review. PARTICIPANTS: Men and women with (UMN) neurogenic bladders secondary to SCI. MAIN OUTCOME MEASURES: Position of the ureteral orifice and urodynamic parameters in association with VUR. METHODS: A retrospective chart review was performed to identify SCI participants with and without reflux who had evaluation with cystoscopy, cystogram, and urodynamics. The position of ureteral orifice was compared…

Mechanisms of failure of endoscopic treatment of vesicoureteral reflux based on endoscopic anatomy.

PURPOSE: We examined the mechanisms responsible for technical failure of the endoscopic technique for correcting vesicoureteral reflux based on endoscopic anatomy. MATERIALS AND METHODS: The study included 25 patients, 2 to 15 years old (mean age 8) who received 57 ureteral injections on 1 or more occasions which failed to correct reflux. Each endoscopic procedure for every patient was videotaped and endoscopic photographs were taken to document pretreatment and posttreatment anatomy. Based on the comparative endoscopic anatomical findings, failures were categorized as secondary to volume los

Congenital short colon with imperforate anus (pouch colon). Report of a case.

We report a case of a persistent cloaca and pseudoexstrophy associated with congenital pouch colon in a native-born American female child. This unusual anomaly occurs in two clinical settings. It has been reported in India as an isolated anomaly occurring primarily in males. Pouch colon also occurs in female patients with pseudoexstrophy or closed cloacal exstrophy. The typical anatomic features of this anomaly are discussed.

Ureteral decompression in advanced nonurologic malignancies.

BACKGROUND: The purpose of this study was to assess the morbidity and determine survival after ureteral decompression in patients with advanced nonurologic malignancies. METHODS: Between June 1988 and June 1993 78 patients were referred to a single surgeon for ureteral decompression. Records were analyzed in relation to primary diagnosis, early and late complications, number of hospitalizations, and survival after decompression. RESULTS: Seventy-two percent of patients initially underwent decompression endoscopically, and 28% required percutaneous nephrostomy placement at initial decompressi

The contribution of urethrocystoscopy to evaluation of lower urinary tract dysfunction in women.

The aim of this study was to determine whether the evaluation of lower urinary dysfunction with urodynamics and urethrocystoscopy provides unique information that is missed by urodynamics alone. Eighty-four women underwent multichannel urodynamics and urethrocystoscopy. Retrospective analysis included evaluation of the relationships between lower urinary tract lesions and risk factors using chi2 and Fisher's exact tests. Urethrocystoscopic findings changed the diagnosis and management in 6 patients. New urethrocystoscopic findings included papillary transitional-cell carcinoma, cystitis glandu…

Benign cystitis in children mimicking rhabdomyosarcoma.

Seventeen (13 male, 4 female) patients, aged 13 months to 13 years, with benign cystitis had imaging findings that mimicked those of rhabdomyosarcoma. Our experience indicates that in the child with hematuria, dysuria, and frequency plus cystographic or sonographic demonstration of a bladder with reduced capacity and circumferential wall thickening or sonographic findings of isoechoic bladder wall thickening (focal, multifocal or circumferential distribution), intact mucosa, and bullous lesions should strongly suggest inflammation and not malignancy. When an inflammatory lesion is suspected, f

Transurethral incision of the prostate using the holmium:YAG laser.

Transurethral incisions of the prostate were made endoscopically in 5 dogs under a fluid medium using a holmium:YAG (Ho:YAG) laser operating at a wavelength of 2.1 microns and with delivery of 1.0 J/pulse in 15 Hz (15 W). Histopathologic examination of tissues collected immediately after surgery revealed irregular crevices outlined by a narrow zone of coagulative necrosis. Ulcerated fissures persisted at 5 and 7 days with the initial stages of epithelial regeneration partially re-epithelializing the ulcerated surfaces. A modest inflammatory response characterized by edema, hemorrhage, and a mi

Polytef paste for urinary incontinence.

A series of 28 patients injected with polytetrafluoroethylene (PTFE) (Polytef, Mentor) paste for urinary incontinence is presented. The cystoscopic method of injection was used in 25 patients. Twenty-four adult females with stress incontinence were treated: 10 of these were long-term cures and five improved. Success rates were higher in those who had had previous gynaecological surgery (10 of 12: P less than 0.05) or who had well supported vaginal walls (5 of 5: P less than 0.05). Three adult males were treated, but all failed. Complications including the risk of micro-embolization are discuss

[Intravesical distension in the corrugated tuberculotic bladder].

The treatment problem of urinary tract post-tuberculous changes is not completely solved. Current methods do not always give good results and are accompanied by some complications. The authors describe their own modified method of hydrostatic intravesical distension of the tuberculotic-cirrhotic bladder, using epidural anesthesia, and worked out indications for it. The results show an increase in the bladder volume from 100-120 to 250-300 ml with a decreased urination frequency in 8 patients. The method occurred to be very convenient-to-use due to the absence of any complications.

[Iatrogenic urethral strictures of the male urethra].

The iatrogenic aetiology of urethral stricture appears to be increasing in frequency. Transurethral catheterization and endourethral manipulation are the principal aetiologic factors. Prevention is based essentially upon a greater respect of the urethra when an endoscopic exploration is necessary and the use of suprapubic catheterization whenever bladder drainage is necessary.
