Endoscopically managed giant frontoethmoidal osteoma with orbital extension.

A patient in his 20s presented with a change in the appearance of his left eye with evidence of relative afferent pupillary defect. Imaging revealed a giant frontoethmoidal osteoma, a benign sinonasal tumour, invading three-quarters of the orbit. Multidisciplinary discussion involving opthalmology, maxillofacial surgery, neurosurgery and otolaryngology resulted in the decision to attempt entirely endoscopic excision of this lesion, which was performed with successful outcomes. This case demonstrates how a sinonasal osteoma should be considered in the differential diagnosis for a patient presen

3D-Printed Craniosynostosis Model: New Simulation Surgical Tool.

BACKGROUND: Craniosynostosis is a complex disease once it involves deep anatomic perception, and a minor mistake during surgery can be fatal. The objective of this report is to present novel 3-dimensional-printed polyamide craniosynostosis models that can improve the understanding and treatment complex pathologies. METHODS: The software InVesalius was used for segmentation of the anatomy image (from 3 patients between 6 and 9 months old). Afterward, the file was transferred to a 3-dimensional printing system and, with the use of an infrared laser, slices of powder PA 2200 were consecutively a

Vein of foramen caecum: imaging findings.

Vein of foramen caecum has been classically described as a vein that connects nasal mucosa to the superior sagittal sinus in classic anatomy textbooks. However, its existence is controversial in literature. Herein, we demonstrated computed tomography and contrast enhanced magnetic resonance imaging findings of a tubular vascular structure extending to nasal mucosa and superior sagittal sinus.

A rare complication of septoplasty: tension pneumocephalus without rhinorrhea.

This paper describes a tension pneumocephalus without cerebrospinal fluid leak, a rare complication of septoplasty. We present a case of tension pneumocephalus without cerebrospinal fluid leak 1 month after a septoplasty operation. Although endoscopic sinus surgery can be utilized for repair of the defect, intracranially displaced ethmoid bone fragment makes an open approach more feasible. His postoperative course was uneventful and postoperative CT scan revealed no signs of pneumocephalus. Absence of cerebrospinal fluid leak made this case unique in presentation and caused a delay in diagnosi

Middle turbinate hinge flap for frontal recess obliteration following prior cranialization.

Frontal sinus cranialization is an established procedure utilized in instances wherein the posterior table has been irreversibly compromised. A known complication of this procedure is pneumocephalus, which may develop if the frontal recess has not been fully separated from the cranialized sinus. We report 3 cases wherein massive pneumocephalus developed after cranialization of the frontal sinus. In each case, a novel endoscopic repair was undertaken utilizing a middle turbinate hinge flap to obliterate the frontal recess. In each instance, obliteration of the frontal recess resulted in durable

Midface distraction.

Since the initial application of distraction osteogenesis to the human mandible by McCarthy, distraction osteogenesis has been used for gradual lengthening of the midface in children with syndromic craniosynostosis, cleft lip and palate, hemifacial microsomia, and midface hypoplasia from other causes. Both external and internal devices are available that permit midface distraction. The background of midface distraction and the development of a Modular Internal Distraction (MID) system that permits widespread use of easily customized, buried distraction devices throughout the craniofacial regio

Bilateral periventricular nodular heterotopia with mental retardation and frontonasal malformation.

BACKGROUND AND OBJECTIVE: Bilateral periventricular nodular heterotopia (BPNH) is a recently recognized malformation of neuronal migration in which nodular masses of gray matter line the walls of the lateral ventricles. Most affected individuals are females with epilepsy and normal intelligence, but no other congenital anomalies. Studies in families with multiple affected individuals, always all females, have mapped one BPNH gene to chromosome Xq28. Several other BPNH syndromes associated with mental retardation and epilepsy but without significant dysmorphic facial features have been observed

Experience with frontonasal dysplasia of varying severity.

Nine cases of frontonasal dysplasia were seen during an 8-year period. Seven were managed in the authors' institution, and the two adult relatives were referred to adult units. The age and sex distribution, the types of defects, and the family pedigree of an affected family are described. Management criteria were formulated according to the severity and other associated abnormalities.

Anatomical classification facial, cranio-facial and latero-facial clefts.

Based on personal observations a new classification of facial, cranio-facial and latero-facial clefts is proposed. The orbit is used as the primary structure of reference. Fifteen locations for clefts can be differentiated. Their course through soft tissues and bone is described in detail and illustrated with typical cases. Combinations of several types of clefts and associated malformations are discussed. The new classification offers the following advantages: it eliminates the old confusing terminology (based on personal experience and case descriptions in the literature it seems to be compl

Anterior Temporal Approach for Clipping of Upper Basilar Artery Aneurysms: Surgical Techniques and Treatment Outcomes.

BACKGROUND: Upper basilar artery (BA) aneurysms, which consist of basilar tip and BA-superior cerebellar artery aneurysms, are challenging to treat with microsurgical clipping. The anterior temporal approach is one surgical approach used to treat aneurysms in this region. Most previous reports on this approach have consisted of unruptured cases. Assessing mostly ruptured cases in this study, we describe the surgical technique, patient characteristics, and surgical outcomes. METHODS: Twenty-three patients with aneurysms arising from the upper BA who received aneurysm clipping via an anterior t

Comparative osseous and soft tissue morphology following cleft lip repair.

OBJECTIVE: To quantify comparative improvement between osseous and soft tissue asymmetry following primary lip repair. DESIGN: Retrospective analysis of preoperative and postoperative computed tomography scans of infants with unilateral cleft lip and palate. Sixteen soft tissue landmarks were placed using an exploratory two-/three-dimensional image processing system and compared for asymmetry. PATIENTS: Computed tomography scans were obtained on 26 patients (13 boys, 13 girls) of Chinese ethnicity (mean age = 0.25 years) prior to Millard lip repair. Nineteen of these contributed to follow-up

The effect of sex and age on facial asymmetry in healthy subjects: a cross-sectional study from adolescence to mid-adulthood.

PURPOSE: The study assessed the effects of sex and age on 3-dimensional (3D) soft-tissue facial asymmetry. PATIENTS AND METHODS: The 3D coordinates of selected soft-tissue facial landmarks were digitized on 314 healthy white subjects (40 male and 33 female adolescents, aged 12 to 15 years; 73 female and 89 male young adults, aged 18 to 30 years; and 41 male and 38 female adults, aged 31 to 56 years) by an electromagnetic instrument. Facial asymmetry was quantified by detecting a plane of symmetry and the centers of gravity of the right and left hemifaces and by calculating the distance betwee
