Computed tomography evaluation of the morphometry and variations of the infraorbital canal relating to endoscopic surgery.

INTRODUCTION: The course of the infraorbital canal may leave the infraorbital nerve susceptible to injury during reconstructive and endoscopic surgery, particularly when surgically manipulating the roof of the maxillary sinus. OBJECTIVE: We investigated both the morphometry and variations of the infraorbital canal with the aim to show the relationship between them relative to endoscopic approaches. METHODS: This retrospective study was performed on paranasal multidetector computed tomography images of 200 patients. RESULTS: The infraorbital canal corpus types were categorized as Type 1: wit

Antrochoanal polyp presenting with obstructive sleep apnoea and cachexia.

A rare presentation of an antrochoanal polyp is reported. A 14-year-old boy presented with obstructive sleep apnoea and subnormal growth velocity for height and weight over a 1-year period. Examination revealed a post-nasal mass which following removal was confirmed histopathologically as an antrochoanal polyp. Relief of the airway obstruction was promptly followed by catch-up growth and subsequent normal growth velocities. The possible mechanisms underlying the cachexia are explored including the possible association with the obstructive sleep apnoea.

Endoscopic treatment of sinonasal disease in patients who have had orthognathic surgery.

Certain skeletofacial patterns may be predisposed to aggravated sinonasal disease postoperatively. These may include, but are not limited to, facial skeletal asymmetries with high septal deviations and those with obstructive nasal respiration and mouth breathing that leads to skeletal growth disturbances such as vertical maxillary hyperplasia and apertognathism. These sinonasal diseases may partly be the result of osteomeatal blockage by pre-existing structures, or synechial shelves and webs blocking normal maxillary antral mucosal flow. The use of nasal antral windows placed anteriorly in the

The troublesome submerged tooth: a diagnostic dilemma.

Facial swelling caused by a periapical abscess is commonly encountered by dental practitioners. It requires prompt treatment, usually by removal of the infective foci and administration of appropriate antibiotics. However, diagnosis can be difficult, and may be complicated by the presence of retained deciduous teeth. This case report demonstrates a diagnostic dilemma and highlights the importance of looking beyond the obvious.

Abnormal length of cilia as a possible cause of defective mucociliary clearance.

A 12-year-old girl with obstructive lung disease was examined with respect to the ultrastructure of her nasal mucociliate epithelium. She had the characteristics of the immotile-cilia syndrome: chronic rhinitis, sinusitis, and bronchitis, a severely decreased mucociliary clearance of the lungs, and nasal polyps. The nasal cilia showed a normal cross-sectional appearance but were found to have twice the normal length: 12 micron rather than 6 micron. We conclude that the patient represents a new subgroup of the immotile-cilia syndrome and that her cilia are unable to perform their normal tasks.

Pulmonary Embolism Revealing an Eosinophilic Granulomatosis with Polyangiitis: A Case Report.

INTRODUCTION: Venous thromboembolism presents a rare clinical manifestation of Eosinophilic Granulomatosis with polyangiitis. CASE PRESENTATION: We present a case of pulmonary embolism associated with Eosinophilic Granulomatosis with Polyangiitis (EGPA) systemic manifestations in a 26-year-old Tunisian female patient. She presented with dyspnea and blood hypereosinophilia. Blood tests revealed elevated D-dimer levels. Lung scintigraphy (V/Q-scan) revealed pulmonary artery embolism in the right lung. The Computed Tomography (CT) image showed circumferential wall thickness of the lower mediasti

Lateral Sinus Floor Elevation in the Severely Atrophied Maxilla: Concentrated Growth Factors Versus Bone Substitutes. A Controlled Clinical Trial.

The present study clinically and radiographically compares the outcome of implants inserted in maxillary sinuses augmented with concentrated growth factors (CGFs) or demineralized bovine bone matrix (DBBM) in a one-stage lateral approach. In 20 patients with a residual bone height of 1 to 4 mm, lateral sinus floor elevation was performed, using CGFs or DBBM as the sole grafting material, with simultaneous implant placement. Outcome variables were implant and prosthesis failures, complications, subjective satisfaction, and radiographic changes in marginal bone level (MBL) 12 months after surger

Technical report: a high-dose-rate interstitial brachytherapy boost for residual sinonasal undifferentiated carcinoma.

Sinonasal undifferentiated carcinoma (SNUC) is a highly aggressive and uncommon neoplasm that arises from the mucosa of the nasal cavity or paranasal sinuses. The multidisciplinary approach that includes surgery, radiation therapy (RT), and chemotherapy has been proven to improve survival rates. However, there is no established evidence for the efficacy of further (boost) irradiation following definitive RT in SNUC patients with residual primary tumor. We describe a successful case of a patient with SNUC who had an uncontrolled primary tumor following induction chemotherapy and radical concurr

A Case Report of Aggressive Chronic Rhinosinusitis with Nasal Polyps Mimicking Sinonasal Malignancy.

INTRODUCTION: Cases of extensive nasal polyps rarely occur and may mimic more aggressive lesions of the nose and paranasal sinuses. A case of extensive nasal polyposis with unusually aggressive behavior and its management is presented. PRESENTATION OF CASE: A 27-year-old male patient visited the emergency department of a tertiary center, complaining of recurrent episodes of epistaxis. The patient presented with a large polypoid lesion protruding from the right nostril and producing asymmetry of the face. Diagnostic imaging illustrated a lesion of the right maxillary sinus producing excessive

Analysis of the development of human foetal nasal turbinates using CBCT imaging.

PURPOSE: The morphological structure of the nasal cavity (NC) is important for endoscopic surgical treatment. The location of nasal turbinates, including the superior turbinate (ST), middle turbinate (MT) and inferior turbinate (IT), are well presented during the formation of the human NC in cone beam CT (CBCT) images. There is a complex relationship between the nasal sinuses, the maxillary sinus (MS), ethmoidal sinus and sphenoid sinus, during formation of the NC structure at the morphological level. There is a need to clearly define the relationships of these nasal elements at the ossificati

Misled by the Air: Pneumocephalus.

Pneumocephalus, the presence of air within the cranium, commonly suggests a breach in the meningeal layer or an intracranial infection by a gas-producing organism. Trauma is the most common cause of pneumocephalus, followed by cranial surgery. Other causes include infection and intracranial neoplasm. An 87-year-old man was conveyed to the emergency department after being found to be drowsy by his helper. He was noted to have a new onset right-sided hemiparesis. Past medical history was significant for hypertension, stage 5 chronic kidney disease, cerebrovascular accident, pacemaker insertion f
