Controlled release of rhBMP-2 from nanoparticle-loaded biomimetic graft promotes bone regeneration in a rat spinal fusion model.

With a notable 118% surge in the last two decades, incidences of lumbar spinal fusion procedures in the United States are experiencing a remarkable upward trend. However, the currently available FDA-approved collagen-based Infuse™ spinal fusion bone graft has been associated with serious complications including heterotopic ossification, paralysis, bowel/bladder dysfunction and local inflammation due to the presence of higher doses of recombinant human bone morphogenetic protein (rhBMP-2), a potent osteogenic growth factor. These challenges have heightened the urgent need for novel spinal fusio

Evaluating the Outcomes of Surgical Hip Dislocation for Femoral Head Trauma: A Systematic Review.

Femoral head fractures are complex injuries often associated with high-energy trauma and hip dislocation. Surgical hip dislocation (SHD), particularly the Ganz trochanteric flip osteotomy, was developed to provide extensive intra-articular access while preserving the femoral head's vascular supply. However, a consolidated synthesis of its outcomes is needed. This systematic review aims to evaluate the clinical and radiological outcomes, complication profiles, and key technical considerations of SHD for the management of femoral head trauma. A systematic literature search was conducted in accor

All-suture anchor arthroscopic repair of the distal biceps tendon: impact of suture cross-section on fixation strength and gap formation resistance.

BACKGROUND: A minimally invasive arthroscopic technique may reduce complications such as heterotopic ossification, synostosis and arthrofibrosis after distal biceps brachii tendon repair. Ensuring adequate postoperative mechanical performance is crucial to minimize complications during healing. This study evaluated the mechanical performance-with a special focus on gap resistance-of a minimally invasive arthroscopic technique performed using two all-suture anchors, with different suture cross-sectional shapes. METHODS: Single-incision repair was performed on twelve arms using two all-suture a

Functional and radiographic outcomes after surgical management of complex proximal ulna fractures: a retrospective case series.

BACKGROUND: Complex proximal ulna fractures (including comminuted olecranon fractures and fracture-dislocations of the elbow) pose challenging management due to concomitant injuries to key stabilizers (radial head, coronoid process, and collateral ligaments). This study evaluates functional and radiographic outcomes following surgical treatment and explores the prognostic value of associated injuries, age stratification, and a new coronoid-centric classification system. METHODS: We retrospectively reviewed 43 patients with complex proximal ulna fractures treated between 2019 and 2024, with a

Retrospective Evaluation of Pelvic and Acetabular Fracture Fixation Using the Stryker PRO Pelvis Next Generation Plating System.

Introduction Acetabular and pelvic fractures account for roughly 3% of all skeletal injuries and typically result from high-energy trauma. Surgical fixation is often required to restore pelvic stability and joint congruency. These procedures are technically demanding due to complex pelvic anatomy and limited access. Reconstruction plates are commonly used for their versatility in compression, tension, buttress, and antiglide functions. The Stryker Pelvic PRO Next Generation plating system (Stryker, Portage, MI) offers implants for anterior and posterior fixation, designed to accommodate comple

Surgical hip dislocation technique through the femoral head fovea fenestration and impaction bone grafting for the treatment of non-traumatic osteonecrosis of the femoral head: a retrospective study.

BACKGROUND: Osteonecrosis of the femoral head (ONFH) often leads to the collapse of the femoral head, ultimately resulting in patients undergoing premature total hip arthroplasty (THA). The surgical hip dislocation (SHD) technique is a type of hip-preserving surgery aimed at delaying or avoiding THA. This study aims to evaluate the clinical efficacy of SHD techniques through femoral head fovea fenestration and impaction bone grafting for the treatment of non-traumatic ONFH. METHODS: A retrospective analysis was conducted on the clinical data of 39 patients (39 hips) with non-traumatic ONFH wh

Acetabular "Fleck" Sign: Outcomes of Surgical Repair.

BACKGROUND: Traumatic, posterior hip dislocations in the pediatric population are typically managed by closed reduction to achieve a concentric hip joint. The presence of an acetabular "fleck" sign, despite concentric reduction, has been shown to signify significant hip pathology. The purpose of this study was to evaluate the outcomes of open labral repair through a surgical hip dislocation (SHD) in a consecutive series of patients with an acetabular "fleck" sign associated with a traumatic hip dislocation/subluxation. METHODS: A retrospective review of patients between 2008 and 2022 who pres

Does posterior longitudinal ligament resection during cervical artificial disc replacement affect the clinical or radiographic outcome?

BACKGROUND CONTEXT: Much controversy exists about whether posterior longitudinal ligament (PLL) resection should be performed during cervical artificial disc replacement (ADR). The PLL can be resected or preserved during the ADR procedure based on the shape and location of the compressive pathology. However, unlike fusion operations, the outcomes of ADR may be affected by PLL resection since segmental motion is preserved with ADR and the PLL restricts flexion of the segment. Nevertheless, the effect of PLL removal during ADR in a clinical setting remains unclear. PURPOSE: To investigate the e

Ilioinguinal versus modified Stoppa approach for open reduction and internal fixation of displaced acetabular fractures: a systematic review and meta-analysis of 717 patients across ten studies.

BACKGROUND: Acetabulum fracture is one of the most challenging fractures to manage and operate for orthopaedic surgeons; anatomical reduction of fractures and reconstruction of the joint is of utmost importance. These factors in turn are dependent on the appropriate surgical approach used to improve the clinical outcomes and reduce associated complications. Hence, this meta-analysis aims to compare the outcomes of ilioinguinal versus modified Stoppa approach for open reduction and internal fixation (ORIF) of displaced acetabular fractures. METHODS: Medline (PubMed), Embase, Scopus, and Cochra

Does Surgical Repair Benefit Pipkin Type I Femoral Head Fractures?: A Systematic Review and Meta-Analysis.

Background: Femoral head fractures are rare injuries with or without traumatic dislocations. The management of these fractures is crucial to prevent the development of severe complications and to achieve optimal functional outcomes. Wide treatment options for Pipkin 1 femoral head fractures range from fragment excision, fixation following open reduction with internal fixation, or conservative treatment such as close reduction alone after fracture dislocation. However, the best decision making remains controversial not only due to lack of large trials, but also inconsistent results reported. Th
