Pulmonary Valve Replacement for Tetralogy of Fallot.

Right ventricular outflow tract (RVOT) dysfunction is common following surgical repair of tetralogy of Fallot and other forms of complex congenital heart disease. This results in pulmonary stenosis or regurgitation and may ultimately lead to RV failure and dysrhythmias. Transcatheter valve technologies are now available to treat certain patients with RVOT dysfunction. Current devices include the Medtronic Melody valve and the Edwards Lifesciences SAPIEN XT. Although these valves are approved for use in dysfunctional circumferential RVOT conduits, they are increasingly being used off label for

Decellularized Allografts for Right Ventricular Outflow Tract Reconstruction in Children.

OBJECTIVE: Determine the midterm outcomes of decellularized allografts for right ventricular outflow tract (RVOT) reconstruction in children less than 12 years of age. METHODS: The study included all consecutive patients submitted to RVOT reconstruction with decellularized allografts between June 2006 and June 2016. Besides clinical and echocardiographic control, 20 patients with more than five years of follow-up were evaluated with computed tomography (CT) scans to determine allograft diameters and calcium scores. Structural valve deterioration was defined as any peak gradient above 40 mm Hg

Surgical pulmonary valve replacement: a benchmark for outcomes comparisons.

BACKGROUND: Patients with right heart obstructive lesions develop residual or recurrent right ventricle outflow tract pathology as a result of native or implanted pulmonary valve (PV) dysfunction. Until recently, the standard of care has been surgical placement of a PV or valved right ventricle to pulmonary artery conduit. Catheter-based options are being increasingly applied in patients with PV dysfunction. The purpose of our study was to evaluate outcomes of surgical pulmonary valve/conduit replacement (PVR) at a large pediatric hospital to provide contemporary benchmark data for comparison

The fate of children with microdeletion 22q11.2 syndrome and congenital heart defect: clinical course and cardiac outcome.

BACKGROUND: This study aimed to evaluate the cardiac outcome for children with microdeletion 22q11.2 and congenital heart defect (CHD). METHODS: A total of 49 consecutive children with 22q11.2 and CHD were retrospectively identified. The CHD consisted of tetralogy of Fallot and variances (n = 22), interrupted aortic arch (n = 10), ventricular septal defect (n = 8), truncus arteriosus (n = 6), and double aortic arch (n = 1). Extracardiac anomalies were present in 46 of 47 children. RESULTS: The median follow-up time was 8.5 years (range, 3 months to 23.5 years). Cardiac surgical repair was pe

Oversizing pulmonary homograft conduits does not significantly decrease allograft failure in children.

OBJECTIVE: Placement of oversized pulmonary ventricle-pulmonary artery conduits is routinely performed to decrease conduit failure in children. However, this practice has recently been challenged as somatic outgrowth may not be the main determinant of allograft failure in children. Our objective was to determine whether placement of oversized homografts for extracardiac pulmonary ventricle (PV) outflow tract reconstruction improves longevity in young children. METHODS: We reviewed 102 consecutive PV-PA conduits inserted in 70 patients less than 18 years between 1984 and 2003. Conduits placed

Performance of allografts and xenografts for right ventricular outflow tract reconstruction.

BACKGROUND: We compared the long-term durability of allografts and xenografts implanted for reconstruction of the right ventricular outflow tract. METHODS: A total of 401 patients were studied from January 1974 to June 2000 (145 xeno- and 256 allografts), follow-up being 98% complete. We analyzed freedom from reoperation and allograft specific factors that may indicate degeneration. RESULTS: The age at implantation was 2 days to 31 years (median 4.0 years). Conduit exchange rate was similar (p = 0.2) for conduit diameters less than 15 mm (41%+/-9% for allografts, 30%+/-6% for xenografts), bu

Restrictive right ventricular physiology after tetralogy of Fallot repair is associated with fibrosis of the right ventricular outflow tract visualized on cardiac magnetic resonance imaging.

AIMS: To determine whether the restrictive physiology seen in Tetralogy of Fallot (TOF) patients can be explained by fibrosis of the right ventricular (RV) outflow tract. The aetiology for restrictive RV physiology after TOF repair is not known. METHODS AND RESULTS: TOF patients (n = 31, 13 girls, 10.2 years ± 2.8) were included 9.2 ± 2.9 years after total correction and examined with cardiac magnetic resonance (CMR) and Doppler echocardiography. Cine, flow, and late gadolinium contrast enhanced (LGE) CMR imaging were performed to quantify RV volumes, pulmonary flow and regurgitation (PR), an

Evaluation and management of ventricular outflow tract tachycardias.

Ventricular tachycardia (VT) arising from the right or left ventricular outflow tract (OT) is a recognized arrhythmia in individuals with structurally normal hearts. Treatment options for OTVT include medications, ablation and, rarely, an implantable cardioverter defibrillator (ICD). In the past few years ablation techniques have developed to the point where most OTVTs can be successfully ablated. However, a percentage of cases have remained where ablation is unsuccessful. Some of these cases may represent an epicardial focus of the VT. Several approaches to epicardial VT ablation have been de

Tetralogy of Fallot with atrioventricular septal defect: surgical strategies for repair and midterm outcome of pulmonary valve-sparing approach.

Repair for tetralogy of Fallot (TOF) with complete atrioventricular septal defect (CAVSD) has been reported with good early and intermediate outcomes. Morbidity, however, remains significantly high. To date, repair of CAVSD/TOF using a pulmonary valve-sparing technique (PVS) and freedom from valve reoperation are not well defined. A study was undertaken to investigate outcomes. This study was conducted in as a retrospective investigation. Between January 1988 and December 2008, 13 consecutive patients with CAVSD/TOF were identified, and their records were reviewed retrospectively. Of these 13

Clinical utility of repeat fetal echocardiography in congenital heart disease.

OBJECTIVES: To investigate the utility of repeat fetal echocardiography (FE) following a diagnosis of structural congenital heart disease (CHD) on the initial FE. We evaluated how often changes in management and counseling occurred based on subsequent FE findings and sought to determine which types of CHD were more likely to have changes in management and/or counseling based on repeat FE. METHODS: This was a retrospective review of all patients who presented to our center between January 2012 and January 2019 and who had more than one FE performed for structural CHD. We reviewed consultation

Hypertrophic cardiomyopathy associate with a PLN gene mutation in a child: a case report.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is a common hereditary disease of the myocardium, caused by mutations in at least 20 genes, encoding cardiac sarcomere proteins and sarcomere-associated proteins, while genes related to calcium metabolism, such as phospholamban (PLN), have been reported rarely, in less than 1% of positive patients. This study aims to report a unique case of child's HCM caused by a missense variant in the PLN gene on chromosome 6 (NM_002667.5:c.106T>C, p.Cys36Arg). This mutation in children, not previously reported in the literature except for an adult, expand the k

Revisiting risk factors and incidence of postoperative tachyarrhythmias in pediatric cardiac surgery.

Postoperative tachyarrhythmias are a significant complication following pediatric congenital heart surgery. This study revisits the incidence and risk factors for postoperative tachyarrhythmias, focusing on congenital heart disease with a higher propensity for arrhythmias. We aim to determine if surgical and perioperative improvements have mitigated these risks. The findings may inform clinical practices and guide the development of preventive strategies. This retrospective cohort study was conducted at a tertiary university hospital, including pediatric patients under 18 years of age who unde
