Avoidable injuries after intervention for abdominal aortic aneurysm: An analysis of negligence claims over 15 years in Sweden.

BACKGROUND: Patients, who are subjected to a patient's injury, are legally allowed a compensation for their suffering. AIM: The negligence claims after surgical treatment of abdominal aortic aneurysms (AAA) registered in the National Swedish patient insurance company (Landstingens Ömsesidiga Försäkringsbolag [LÖF]) between 2006 and 2020 were analyzed. More than 95% of negligence claims are covered by LÖF. Special emphasis on avoidable or unavoidable injuries was made. MATERIAL: In 15 years 17,000 abdominal aortic interventions were recorded in the Swedish vascular register (SWEDVASC), where

Blunt Trauma Abdominal and Pelvic Computed Tomography Has Low Yield for Injuries in More Than One Anatomic Region.

INTRODUCTION: Most trauma centers order abdominal and pelvic computed tomography (CT) as an automatically paired CT for adult blunt trauma evaluation. However, excessive CT utilization adds risks of excessive exposure to ionizing radiation, the need to work up incidental findings (leading to unnecessary and invasive tests), and greater costs. Examining a cohort of adult blunt trauma patients that received paired abdominal and pelvic (A/P) CT, we sought to determine the diagnostic yield of clinically significant injuries (CSI) in the following: 1) the abdomen alone; 2) the pelvis alone; 3) the

Pancreatic injuries after blunt abdominal trauma: an analysis of 110 patients treated at a level 1 trauma centre.

BACKGROUND AND OBJECTIVE: Injuries to the pancreas are uncommon, but may result in considerable morbidity and mortality. This study evaluated the management of blunt pancreatic injuries using a previously defined protocol to determine which factors predicted morbidity and mortality. METHODS: The study design was a retrospective chart review of all adult patients with blunt pancreatic injuries treated at a level 1 trauma centre between March 1981 and June 2009. RESULTS: One hundred and ten patients (92 men, 18 women; mean age 30 years, range 13-68 years) were treated during the study period.

Effects of additional intra-abdominal organ injuries in patients with penetrating small bowel trauma on morbidity and mortality.

BACKGROUND: We aimed to evaluate the effects on morbidity and mortality of additional organ injuries obtained concurrently with penetrating small bowel injuries. METHODS: Between January 2000 and March 2005, patients in whom penetrating small bowel injuries occurred as a result of abdominal injuries were allocated into two groups and assessed. The first group included patients with isolated small bowel injuries, while those in the second group had small bowel injuries together with other intra-abdominal organ injuries. RESULTS: Small bowel injury was identified in 38 patients (34 M, 4 F) wit…

[Delayed perforation of the cecum and sigmoid colon after blunt abdominal trauma in a patient with multiple injuries].

Blunt abdominal trauma can damage the intestinal vasculature and may occasionally lead to delayed intestinal perforation, associated with a combined rate of morbidity and mortality of 25%. The diagnosis of such complications is hindered by sedation in critical patients, however, and morbimortality in this population is therefore higher. We report the case of a man with multiple injuries admitted to the intensive care unit, where delayed perforations of the sigmoid colon and cecum were diagnosed. The management of blunt abdominal trauma is reviewed and the possible causes, diagnostic approaches

The benefit of routine thoracic, abdominal, and pelvic computed tomography to evaluate trauma patients with closed head injuries.

BACKGROUND: The evaluation of multitrauma patients for blunt truncal injuries remains open for debate. We sought to evaluate the role of routine computed tomography (CT) of the chest, abdomen, and pelvis as a screening tool for patients already undergoing cranial CT studies. METHODS: Charts of blunt trauma patients admitted from June 2000 to June 2001 were reviewed for demographics, Glascow Coma Scale (GCS), physical and radiological findings, and length of stay. RESULTS: Our study found that 38% of patients undergoing cranial CT scanning had a unexpected finding on body scans. Changes were

The role of intestinal mucosa injury induced by intra-abdominal hypertension in the development of abdominal compartment syndrome and multiple organ dysfunction syndrome.

INTRODUCTION: Abdominal distension is common in critical illness. There is a growing recognition that intra-abdominal hypertension (IAH) may complicate nonsurgical critical illness as well as after abdominal surgery. However, the pathophysiological basis of the injury to the intestinal mucosal barrier and its influence on the onset of abdominal compartment syndrome (ACS) and multiorgan dysfunction syndrome (MODS) remain unclear. We measured intestinal microcirculatory blood flow (MBF) during periods of raised intra-abdominal pressure (IAP) and examined how this influenced intestinal permeabili

Successful non-operative management of multiple intra-abdominal solid organ injury after blunt abdominal trauma: a case report.

Injuries to the solid abdominal viscera are common after blunt trauma. The success of non-operative management of these injuries has led to recent extensions of this approach to managing higher-grade, more complicated injuries that are typically treated operatively. We reported a 19-year-old male who presented with abdominal pain and gross hematuria during the late hours due to a motor vehicle accident. Abdominal computed tomography scan revealed moderate hemoperitoneum, extensively devascularized spleen with laceration extending into the hilum, multiple tears in the left kidney extended to th

Acute lung injury and multiple organ dysfunction syndrome secondary to intra-abdominal hypertension and abdominal decompression in extensively burned patients.

BACKGROUND: Secondary abdominal compartment syndrome (ACS) is a lethal complication after resuscitation from burn shock, even after abdominal decompression (AD) is performed. This study investigated increased susceptibility to multiple organ dysfunction syndrome (MODS) in extensively burned patients with ACS. METHODS: Patients admitted to our burn unit between 2002 and 2005 with burns affecting 40% or more of the total body surface area without severe inhalation injury were analyzed. Hemodynamic parameters, blood gas analysis, and intrabladder pressure as intra-abdominal pressure were recorde

Successful treatment of burn and visceral injury combined with full-thickness loss of the abdominal wall after blast injury.

A 29-year-old man was admitted to our institution 10 days after he had undergone an urgent exploratory laparotomy at a local army hospital after a terrorist bombing attack. On admission, deep second-degree and third-degree burns involving 25% of the upper and lower extremities were present, together with a 25 x 10-cm abdominal full-thickness blast injury defect on the left side, an infected eviscerated midline incision, and a colostomy on the right side of the abdomen. The patient underwent a second laparotomy, at which time the intraabdominal abscess was drained, and the abdominal cavity was

Injury patterns in frontal crashes: the association between knee-thigh-hip (KTH) and serious intra-abdominal injury.

Safety belts protect occupants in frontal impacts by reducing occupant deceleration and preventing the occupant from hitting interior vehicle components likely to cause injury. However, occupants moving forward during the impact may contact the safety belt webbing across their chest and abdomen. We hypothesized that if the occupant loaded their knee-thigh-hip (KTH) region with enough force to result in injury to this region-it might prevent compression (and injury) of their abdomen by the safety belt. Crash Injury Research and Engineering Network (CIREN) data were used to test the association

Effectiveness of clinical examination and radiological investigations in the success of selective non-operative management of abdominal gunshot injuries.

OBJECTIVES: Non-operative management of abdominal gunshot injuries has become the standard care in the selected cases of modern surgery with an acceptable success rate to reduce the incidence of unnecessary laparotomies. In this study, an assessment was conducted to determine how the success of this form of management was impacted by physical examination and radiological investigation. MATERIAL AND METHODS: This is a retrospective study that includes all consecutive penetrating abdominal gunshot wound patients who were admitted to the emergency department between February 2011 and December 20

Clinical predictors of therapeutic laparotomy in anterior abdominal stab injuries: a multicenter study from low-income institutions in Ethiopia.

PURPOSE: Despite the high incidence of abdominal stab injuries, the rate of nontherapeutic laparotomies and the predictors of therapeutic laparotomies have rarely been studied in low-income settings. METHODS: This multicenter retrospective study involved three of the largest academic medical centers in central Ethiopia. All patients who sustained an anterior abdominal stab injury and underwent exploratory laparotomy, regardless of the intraoperative findings, were included over the 3-year course of the study. RESULTS: Of the 117 patients who underwent exploratory laparotomy, 35 patients (29.

[Rupture of the biliary tract in the liver hilum region following closed abdominal injuries. General review about 2 cases].

Two cases of traumatic rupture of the biliary tract in the liver hilum region are reported. Similar cases (27 patients) reported in the published literature are discussed and the physiopathological, pathological, and clinical characteristics described. Road accidents account for the majority of these cases of biliary tract injury, which are seen mainly in young males. Diagnosis is confirmed either during emergency laparotomy for haemoperitoneum, the presence of bile leading to cholangiography which establishes the diagnosis, or after several days from the presence of jaundice and signs of a pe

The Effects of Body Mass Index and Seatbelt Use on Pediatric Chest and Abdominal Injuries after Motor Vehicle Collisions.

BACKGROUND: Motor vehicle collisions (MVCs) are a major cause of pediatric morbidity and mortality. However, the effect of body mass index (BMI) and seatbelt use on thoracic injuries and outcomes in pediatric patients with rib fractures remains unexplored. We aim to assess how seatbelt use and BMI affect thoracic injuries and outcomes in pediatric patients who sustained ≥3 rib fractures following an MVC. METHODS: The Trauma Quality Improvement Program (TQIP) database (2013-2017) was queried for pediatric patients (8-17 years and >4 feet 9 inches tall) admitted secondary to MVCs, with ≥3 rib f

Abdominal Clonus as a Rare Visceromotor Manifestation in Spinal Cord Injury: A Case Report.

Spinal cord injury can lead to reorganization of spinal reflex circuits and loss of supraspinal modulation of autonomic and somatic functions. Although bladder distension is a well-recognized trigger of autonomic dysreflexia, associated somatic motor manifestations are rarely described. We report the case of a 69-year-old man with a long-standing traumatic complete C6 spinal cord injury and chronic neurogenic bladder. Neurological classification according to the International Standards for Neurological Classification of Spinal Cord Injury (ISNCSCI) was American Spinal Injury Association (ASIA)
