Injury Patterns and Physiologic Risk Stratification in Facial Trauma Patients with Orbital Fractures: A National Trauma Database Analysis.

BACKGROUND: Although orbital fractures are common in trauma care, age-specific mechanisms and admission physiology-based risk stratification have not been systematically characterized. This study aimed to identify age-mechanism interaction patterns and develop an admission-based physiological risk score for orbital fracture patients. METHODS: This retrospective cohort study analyzed 41,464 adult orbital fracture patients from the National Trauma Data Bank (2018-2020). A three-component physiological risk score was developed using admission vital signs: severe hypotension (<90 mmHg, 2 points),

Unexplained mechanism of subdural hematoma with convulsion suggests nonaccidental head trauma: A multicenter, retrospective study by the Japanese Head injury of Infants and Toddlers study (J-HITs) group.

OBJECTIVE: The medical history of injury given by parents of infants and toddlers with head trauma may not be accurate or completely true. T … ere examined by multivariable analysis to identify which ones were statistically associated with nonaccidental injuries. RESULTS: Of the 84 patients with SDHs, 51 were classified into the nonaccidental group, and 33 children were classified into the accidental group. In 19 patients with a chief complaint of convulsion who had SDH but no episode of trauma, 18 were cl …

Childhood Falls With Occipital Impacts.

OBJECTIVES: Falls are commonly reported in children who present with both accidental and inflicted brain injuries. Short falls rarely result in serious or life-threatening injuries. Our purpose is to describe a series of cases of short falls with occipital impact leading to subdural hemorrhage (SDH). METHODS: We present a series of 8 witnessed accounts of young children diagnosed as having SDHs after striking the back of their heads during a short fall. Child-abuse physicians were surveyed to determine if they had evaluated a child younger than 24 months diagnosed as having SDH, with or witho

Nonobstetrics vulvovaginal injuries: mechanism and outcome.

OBJECTIVE: The aim of this study is the evaluation of mechanisms and outcome of nonobstetrics vulvovaginal injuries (VVI). STUDY DESIGN: Medical documentation on 17 VVI recorded during the 1998-2005 period was analysed. The analysis included age, mode of injury infliction (accidental, intracoital, rape), treatment, and VVI outcome. Pregnant women and obstetrics peripartal and postpartal VVI were excluded from this study, and there was no sexual assault VVI in children and young girls in this period. RESULTS: There were five young girls, all of them raped by the straddle mechanism, five adole

Severe Retinal Hemorrhages with Retinoschisis in Infants are Not Pathognomonic for Abusive Head Trauma.

The combination of subdural hemorrhage (SDH), retinal hemorrhage (RH), and encephalopathy, or the presence of severe retinal hemorrhages alone in infants, is often attributed to and has been stated to be pathognomonic for abusive head trauma (AHT) or shaken baby syndrome. These beliefs have been challenged, because the same constellation of findings has been identified in accidental head injuries and natural diseases, and most if not all of the studies that support the concept of diagnostic specificity have serious flaws in their methodology. Presented here are two cases of severe retinal hemo

Induced and accidental hypothermia.

OBJECTIVE: To review human thermoregulation and the pathophysiology and management of induced and accidental hypothermia. DATA SOURCES: A review of studies reported over ten years from 1990 to 2000 and identified through a MEDLINE search of the English-language literature on thermoregulation and induced and accidental hypothermia. SUMMARY OF REVIEW: Hypothermia is defined as a core temperature less than 35 degrees C, and may be therapeutic (i.e. induced for clinical benefit) or accidental. Hypothermia induced prior to cardiovascular or neurosurgical procedures (i.e. therapeutic hypothermia)

Bet 4: value of skeletal surveys in suspected physical abuse of children.

A short cut review was performed to assess the utility of a skeletal survey in patients with suspected non-accidental injury. Three studies including a total of 983 patients were found. The authors, date and country of publication, patient group studied, study type, relevant outcomes, results and study weaknesses are tabulated. The clinical bottom line is that skeletal surveys are useful in children who are suspected to have non-accidental injury, particularly in those aged under 1 year.

Child abuse and orthopaedic injury patterns: analysis at a level I pediatric trauma center.

BACKGROUND: Child abuse is a serious threat to the physical and psychosocial well-being of the pediatric population. Musculoskeletal injuries are common manifestations of child abuse. There have been multiple studies that have attempted to identify the factors associated with, and the specific injury patterns seen with musculoskeletal trauma from child abuse, yet there have been no large studies that have used prospectively collected data and controlled comparisons. The purpose of our study was to describe the patterns of orthopaedic injury for child abuse cases detected in the large urban are

Reading between the lesions: Recognizing intent in pediatric burn injuries.

Burn injuries are common in childhood and may occur in accidental or non-accidental circumstances, making forensic interpretation challenging. Evaluation is difficult when clinical findings, reported history, and contextual evidence are incomplete or discordant. We present three pediatric cases evaluated through a structured forensic approach integrating clinical findings, lesion morphology, reported circumstances, developmental assessment, scene reconstruction, and experimental testing. The first involved a 7-year-old girl with scald burns of the buttocks and right lower limb following the re

Percutaneous Suture - Closure Device Used for Iatrogenic Carotid Artery Injury Management: Case Report and Review of Literature.

Accidental carotid artery injury is an uncommon but serious central venous catheter insertion complication. Hemostasis might not be readily achieved by manual compression; therefore, surgery or endovascular treatment remains the mainstay for accidental carotid artery injury. However, not all patients are suitable candidates for surgery.Vascular closure devices are widely used in femoral arteries to achieve hemostasis and early ambulation. The use of vascular closure devices is occasionally reported in other vascular beds. Here we present a case of an iatrogenic left common carotid artery injur

Transanal high pressure barotrauma causing colorectal injuries: a case series.

BACKGROUND: Rectal perforation by foreign bodies is known; however, high-pressure injury leading to rectal blowout has been confined to battlefields and is less often encountered in general medical practice. Apart from iatrogenic injuries during colonoscopy, barotrauma from compressed air is encountered very less frequently. Owing to the infrequent nature of these injuries, the mechanism is still not well understood. We present our experience with treating high-pressure transanal barotrauma to the rectum and colon in three similar cases. CASE PRESENTATION: The mode of injury was accidental or

Foreign bodies and caustic lesions.

Foreign body ingestions, food bolus impactions, and caustic agent injuries are frequent but specific situations. Although most foreign bodies will naturally pass through the digestive tract, practitioners should recognize specific situations were endoscopic management is required. In such cases, timing and adequate equipment are critical. Endoscopic treatment is successful in about 95% of patients. Severe complications (including oesophageal perforations) are rare. Underlying diseases (including eosinophilic oesophagitis) must be investigated after food bolus impaction. Accidental or suicidal

Hematocolpos as a Result of Delayed Treatment of Acute Straddle Injury in an Adolescent Girl.

Accidental genital trauma is most commonly caused by straddle-type injuries and is usually treatable by nonoperative management, and most of the injuries have a good prognosis. When the bleeding occurred due to straddle injury in adolescent girl, experienced gynecological examination and treatment were very important. We experienced a case of straddle injury to the posterior fourchette that caused acute hematocolpos due to delayed adequate treatment with hypotension and acute abdomen in an adolescent girl. This case shows the importance of careful and accurate physical and gynecological examin

Death by artery injury: two peculiar cases of sharp force fatality.

BACKGROUND: As sharp force injury accounts for 10-20% of clinical forensic examinations, forensic pathologists are often asked to investigate deceased victims of stab wounds. Moreover, homicide by sharp force (stabbing) is one of the most common in European countries, involving generally domestic or interpersonal conflict. Stabbing as a suicide method constitutes a low percentage of all suicides, 2% to 3%. Accidental death due to sharp force is even rarer (0-3%) and usu-ally caused by an impact or a fall into different type of glass surface. Death due to stabbing is usually caused by exsanguin

Aquatic Antagonists: Dermatologic Injuries From Sea Urchins (Echinoidea).

Sea urchin injuries are common following accidental contact with sharp sea urchin spines. Immediate manifestations of injury include local erythema, pain, and myalgia. Failure to remove the spines from the skin may result in delayed systemic reactions, secondary infection, granulomas, and-if joint spaces are involved-inflammatory or infectious synovitis and arthritis. The majority of severe complications can be avoided if the spines are fully removed from the skin soon after injury, which can be difficult. This article aims to bring awareness to the myriad complications from sea urchin injurie

Use of Foley catheter in control of internal carotid hemorrhage during endoscopic endonasal surgery.

Internal carotid artery (ICA) injuries during endoscopic endonasal surgery (EES) are rare life-threatening events. We describe a technique to manage ICA injuries based on the use of Foley catheters.A 26-year-old female underwent endoscopic transnasal trans-sphenoidal removal of pituitary adenoma. Cerebrospinal fluid leak occurred 4 days postoperatively. During repair procedure, accidental injury of ICA occurred. Emergency nasal packing through positioning of four Foley urologic catheters was successfully performed to stop bleeding. The patient did not report neurologic deficits. In author's opi

Pediatric Emergencies: Imaging of Pediatric Head Trauma.

Pediatric head trauma is an important cause of morbidity and mortality in children and may be seen in the setting of accidental or abusive injuries. Although many of the patterns of head injury are similar to adults, the imaging manifestations of head injury in children are more complex due to the developing brain and calvarium. Additionally, there are unique considerations for mechanisms of injury in children, to include abusive head trauma and birth-related injuries. The primary role of the radiologist is to identify and characterize the type and severity of head injury to help guide appropr

Penetrating cactus spine injury to the mediastinum of a child.

Injuries from contact with cactus plants are not uncommon in the southwestern United States and other arid landscapes. Most often, accidental encounters with sharp spines and barbs result in little more than mechanical damage to the skin and soft tissues with minor pain and irritation. Although cactus spine penetration into the skin has been reported to be a stimulus for cutaneous granuloma formation, our review of the published literature did not reveal any potentially life-threatening injuries from such insults. We present a unique case of a penetrating cactus spine to the anterior mediastin

A cruel case of homicide: Execution by carbonisation after sharp force trauma.

Fire-related deaths are usually accidental; self-immolation is a rare means of suicide with fire-related homicides usually a consequence of arson or direct immolation. In homicide investigations, it is important to establish whether heat injuries occurred while the victim was still alive, particularly if traumatic injuries are also present. This assessment can have a significant impact on the offender's sentence, as forensic findings may explain aggravating circumstances. We present a case involving a partially charred body with sharp-force injuries to the neck. Through forensic examination, a

The eye in child abuse.

Child physical abuse may result in a range of injuries to the globe and surrounding tissues. These injuries have varying degrees of specificity for abuse, and no pattern of injury is unique to abuse. Easily overlooked eye injuries in non-ambulatory infants often portend more severe abuse and require careful evaluation for occult injury when they are unexplained. Retinal hemorrhages are most often a sign of significant trauma and the severity of the hemorrhages generally parallels the severity of neurological trauma. Ophthalmologists contribute important data that more easily distinguish medica
