Ultrasound-guided percutaneous injection of ethanolamine oleate for hypersplenism. An experimental study in dogs.

In order to evaluate a possible therapy for hypersplenism, an experiment with animals was done. In nine dogs, 0.6 ml/kg body weight of 5% ethanolamine oleate was injected percutaneously into the spleen under ultrasound guidance. The injection was repeated three times at intervals of 1 week. Three dogs each were killed at 1, 4, and 8 weeks after the final injection. All dogs tolerated the procedure well and lived until they were killed. The platelet count and leukocyte count increased after the injections, and remained higher than the pretreatment level until death. This effect probably is due

Development, characterization, and anti-leishmanial activity of topical amphotericin B nanoemulsions.

Leishmaniasis is a neglected infectious disease caused by protozoan parasites from Leishmania genus species, affecting millions of people, in several countries. The current available treatment for cutaneous leishmaniasis (CL) has presented many side effects. In this way, micro- and nanotechnology are important processes, since they may be useful for release profile modulation of CL drugs improving their bioavailability. Amphotericin B (AmB) is a macrolide antibiotic used as a second-choice treatment. This study aimed the development of oil-water nanoemulsions (NEs) containing AmB for topical a

Preparation, characterization, and topical delivery of paromomycin ion pairing.

Topical chemotherapy with paromomycin (PA) has been used as an alternative for the treatment of cutaneous leishmaniasis; however, poor skin penetration of this drug limits the efficacy of formulations. The objective of this work was to study the ability of the PA free base to form ion pairing with organic acids, as well as evaluate the effect of these compounds on the topical delivery of PA. PA permeation across intact skin was low, while drug penetration into skin from PA ion pairing was the higher than that observed for the PA base. Data obtained on the stripped skin, a damaged skin model, c

Treatment of cutaneous leishmaniasis with a topical antileishmanial drug (WR279396): phase 2 pilot study.

We studied the efficacy of WR279396, a topical formulation of aminoglycosides that cures 100% of cutaneous leishmaniasis lesions in mice. We conducted what is to our knowledge the first controlled study of WR279396 therapy for clinical cutaneous leishmaniasis. A total of 45 Colombian soldiers, all men, were randomly assigned to treatment with WR279396 (33 patients) or placebo (12 patients). Each lesion was treated twice daily for 20 days. Lesions were measured at the end of therapy and at 45, 90, and 180 days after treatment began. A total of 17 (61%) of 28 assessable WR279396-treated patients

Nanocarriers for topical minoxidil in androgenetic alopecia: systematic review of preclinical and clinical evidence.

BACKGROUND: Androgenetic Alopecia (AGA) is the most prevalent hair disorder. Conventional topical minoxidil is limited by suboptimal follicular penetration, local adverse effects, and poor adherence; nanocarrier-based "nanominoxidil" systems may optimize delivery, but their clinical value remains uncertain. OBJECTIVE: To synthesize experimental and clinical evidence on nanocarrier-based topical minoxidil for AGA. METHODS: We systematically searched databases (2015-2024) for in vitro, ex-vivo, in vivo, and human studies evaluating nanocarriers < 1000 nm loaded with minoxidil versus convention

Novel drug delivery systems in topical treatment of atopic dermatitis.

Atopic dermatitis (AD), also known as eczema, is a chronic skin condition, affecting all ages, characterized by inflammation, itching, dryness, and redness, which can severely impact quality of life. Based on clinical features and symptoms, AD is of several types like contact dermatitis, dyshidrotic eczema, neurodermatitis, seborrheic dermatitis, stasis dermatitis, and nummular eczema. Its pathophysiology involves genetic, environmental, and immunological factors, leading to skin barrier dysfunction. Current treatments for AD involve a combination of pharmacological and non-pharmacological str

Topical 5% minoxidil versus combined erbium YAG laser and topical 5% minoxidil in androgenetic alopecia: A randomized controlled trial.

BACKGROUND: Androgenetic alopecia (AGA) is the most common type of hair loss in men, and several treatment options have been proposed for it. Fractional ablative erbium YAG laser can promote hair growth through trans-epidermal drug delivery and the thermal stimulation of hair follicles; this study therefore aims to evaluate minoxidil alone and in combination with fractional ablative erbium YAG laser in male patients with AGA. METHODS: This study was performed on 30 male patients with moderate to severe AGA. Patients were equally randomized into two groups, and the intervention group was treat

Eosinophilic fasciitis presenting as a unilateral, solitary plaque.

Eosinophilic fasciitis is a rare connective tissue disorder characterized by inflammation of the fascia that leads to painful, indurated skin. Because of its variable clinical presentation and overlap with conditions, such as morphea, the diagnosis of eosinophilic fasciitis can be challenging and relies on clinical presentation, histopathologic and laboratory analysis, and response to therapy. Herein, we present an unusual, solitary, isolated plaque with pathologic features and response to therapy most consistent with eosinophilic fasciitis.

Advances in Food Allergy.

Food allergy has been increasing in prevalence for the last few decades, and numerous studies have evaluated ways of improving the allergy practitioner's ability to accurately diagnose patients who are truly food allergic, rather than sensitive but able to tolerate food. Once diagnosed, the current standard treatment is food elimination and avoidance, but other potential treatment options like oral immunotherapy, sublingual immunotherapy, and epicutaneous immunotherapy are becoming promising alternatives. Due the health care costs and potential for life-threatening adverse reactions, much atte

Rosacea: an update on medical therapies.

Rosacea is a common, chronic cutaneous condition that affects the face. Two topicals and one oral medication are currently approved for the treatment of rosacea, including azelaic acid, metronidazole, and sub-antimicrobial dose of doxycycline. Identification of subtypes can help guide treatment strategies. It is essential for psychosocial implications of rosacea to be considered and conservative management, such as nonpharmacologic routine skin care, must form an important part of the overall care. Recently, new insights into the pathophysiology of rosacea have led to the emergence of etiologi

New antibiotic therapies for acne and rosacea.

Acne and rosacea compromise a substantial portion of the dermatology clinical practice. Over the past century, many treatment modalities have been introduced with antibiotics playing a major role. Today, both oral and topical antibiotics are used in the management of acne and rosacea, with several novel formulations and/or combination regimens recently introduced. The latest studies suggest anti-inflammatory actions to be the most likely mechanism of antibiotics in acne and rosacea, shifting the focus to subantimicrobial-dose oral antibiotics and/or topical antibiotic regimens as the preferred

Evidence-based review of the nonsurgical management of hyperhidrosis.

The most common nonsurgical modern treatments for hyperhidrosis include topical treatments such as aluminum chloride, iontophoresis (usually with tap water), oral medications such as anticholinergics, and BTX-A. Topical treatments should always be first-line therapy. For those who fail such treatment, iontophoresis is typically recommended for those with palmar or plantar hyperhidrosis, whereas BTX is often considered as first- or second-line therapy in severe axillary hyperhidrosis. Oral anticholinergics are considered after failure of all other nonsurgical treatments.

Evaluation of topical ketoconazole in cutaneous leishmaniasis.

The efficacy of topical ketoconazole in cutaneous leishmaniasis (CL) caused by L. major was assessed in an open trial. Ten patients with twenty two lesions of CL, confirmed by smear/or biopsy, were recruited into the study. Of the twenty two lesions treated, three increased in size and lesion characteristics worsened, ten showed no change but in nine there was some improvement in lesion characteristics. No lesion healed completely. No side effects were reported in nine. Topically applied ketoconazole is safe but does not significantly alter the course of CL. The differences in the results of s

Diphenhydramine toxicity in three children with varicella-zoster infection.

Diphenhydramine hydrochloride is an antihistamine with anticholinergic properties that is frequently used both orally and topically for the temporary relief of pruritus. Significant systemic absorption may occur following topical administration of diphenhydramine in patients with varicella-zoster lesions. We describe three children with varicella-zoster infection (VZI) who developed bizarre behavior as well as visual and auditory hallucinations following topical applications of large amounts of diphenhydramine to the majority of skin surfaces. In two cases, oral diphenhydramine was also admini
