New Developing Treatments for Molluscum Contagiosum.

A large variety of treatments for molluscum contagiosum (MC) are available, but none are Food and Drug Administration (FDA) approved and there is no consensus on the optimal approach, mainly owing to a lack of high-level data. Physical modalities are widely used, but require repeated outpatient visits for administration, are painful and difficult to perform in children, and are associated with the possibility of residual scarring and post-inflammatory hypo- or hyperpigmentation. Two experimental topical drugs, a new standardized preparation of topical cantharidin, called VP-102, and a topical

Topical clonidine produces mydriasis by a central nervous system action.

One drop of clonidine solution (0.125, 0.25 or 0.5%) was administered topically to one eye in cats anesthetized with pentobarbital in which the vagosympathetic nerves had been sectioned. Clonidine caused a simultaneous dose-related mydriasis in both eyes along with a decrease in heart rate. The peak effects were observed in about 20--30 min. Topical administration of clonidine (0.5%) produced no effect on the parasympathectomized, eserinized iris but did dilate the opposite pupil. Epinephrine (0.1--30 microgram, i.a.) produced equal pupillary dilation in both eyes. In addition, topical clonidi

Topical tranexamic acid in hip and knee surgery: a meta-analysis of randomized-controlled trials.

PURPOSE: To compare the efficacy and safety of topical tranexamic acid (TXA) to systemic administration (intravenous and oral) and placebo in hip and knee surgeries. METHODS: This meta-analysis was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. A systematic literature search was conducted in PubMed and Scopus to compare the bleeding, hospitality, and morbidity between topical tranexamic acid with systemic administration of TXA and placebo. Random-effect models and meta-regressions were used to generate pooled estimates

Fibrin-enmeshed tobramycin liposomes: single application topical therapy of Pseudomonas keratitis.

Treatment of bacterial keratitis requires frequent application of topical antibiotics. We studied the efficacy of a single topical administration of tobramycin incorporated in large multivesicular liposomes and enmeshed in a fibrin sealant on rabbit corneas infected with Pseudomonas aeruginosa. One cornea each of 25 New Zealand albino rabbits was infected with P. aeruginosa. Twenty-four hours later, the animals were randomly divided into five groups of five. Group A received single hourly drops (50 microliters) of fortified tobramycin (14.5 mg/ml, total of 17.4 mg). Group B received a single t

Need for Speed: Topical Roflumilast for Rapid Control of Seborrheic Dermatitis Flares.

Seborrheic dermatitis is a common chronic inflammatory skin condition historically treated with topical antifungals and anti-inflammatory agents. While topical corticosteroids are frequently used to quickly control severe disease or flare-ups, efficacy is limited. Topical roflumilast 0.3% foam is the most recent treatment for seborrheic dermatitis approved by the US Food and Drug Administration (FDA); however, its speed of action in controlling flares has not been fully explored. Herein, a case of severe seborrheic dermatitis with significant treatment response to topical roflumilast 0.3% crea

Topical tacrolimus Protopic.

Topical tacrolimus is the first topical immunomodulator of its kind, and its mechanism of action, pharmacokinetics, metabolism and efficacy will be discussed. Multiple studies have supported the safety and efficacy of topical tacrolimus in the treatment of atopic dermatitis in both adults and children. Studies have also suggested that topical tacrolimus may be effective in the treatment of acute contact dermatitis. Although oral administration of tacrolimus proved to be efficacious in psoriasis, results with topical tacrolimus have been disappointing in some studies. Additional reports of succ

Topical application of disodium isostearyl 2-O-L-ascorbyl phosphate, an amphiphilic ascorbic acid derivative, reduces neuropathic hyperalgesia in rats.

BACKGROUND AND PURPOSE: Ca(v) 3.2 T-type calcium channels, targeted by H(2) S, are involved in neuropathic hyperalgesia in rats and ascorbic acid inhibits Ca(v) 3.2 channels. Therefore, we evaluated the effects of intraplantar (i.pl.) administration of ascorbic acid or topical application of disodium isostearyl 2-O-L-ascorbyl phosphate (DI-VCP), a skin-permeable ascorbate derivative on hyperalgesia induced by NaHS, an H(2) S donor, and on neuropathic hyperalgesia. EXPERIMENTAL APPROACH: In rats mechanical hyperalgesia was evoked by i.pl. NaHS, and neuropathic hyperalgesia was induced by L5 sp

Conjunctival expression of the P2Y2 receptor and the effects of 3% diquafosol ophthalmic solution in dogs.

Conjunctival epithelial and goblet cell P2Y2 nucleotide receptors regulate ion transport and secretory function. Diquafosol is a P2Y2 purinergic receptor agonist that stimulates secretion of aqueous tear components from conjunctival epithelial cells and secretion of mucin from conjunctival goblet cells. In humans suffering from keratoconjunctivitis sicca (dry eye), topical administration of diquafosol improves corneal epithelial integrity and stabilises the tear film. The aim of the present study was to investigate P2Y2 receptor expression and to determine the effect of topical administration

Sulfonamides do not reach the retina in therapeutic amounts after topical application to the cornea.

Retinal drug concentrations were measured in treated and fellow eyes following topical administration of two potent and permeable carbonic anhydrase inhibitors, MK-927 and trifluoromethanesulfonamide (CF3SO2NH2) in the rabbit. Administration of 2 or 5 drops 2% MK-927 or 2% CF3SO2NH2 gave at 1 hr 1.5-9 microM drug in the retinas of treated eyes and 1.3-3.7 microM drug in the retinas of the fellow eye. After allowance was made for the blood content of the retinas (1.4-13%) and drug contained in whole blood due to systemic absorption, little (< 1.4 microM) or no net accumulation in retina could b

Low-dose daily aspirin reduces topical minoxidil efficacy in androgenetic alopecia patients.

Topical minoxidil is the only US FDA approved OTC drug for the treatment of androgenetic alopecia (AGA). Minoxidil is a pro-drug converted into its active form, minoxidil sulfate, by the sulfotransferase enzymes in the outer root sheath of hair follicles. Previously, we demonstrated that sulfotransferase activity in hair follicles predicts response to topical minoxidil in the treatment of AGA. In the human liver, sulfotransferase activity is significantly inhibited by salicylic acid. Low-dose OTC aspirin (75-81 mg), a derivative of salicylic acid, is used by millions of people daily for the pr

Drug delivery systems for the topical treatment of cutaneous leishmaniasis.

INTRODUCTION: The parenteral administration of pentavalent antimonials for the treatment of all forms of leishmaniasis, including cutaneous leishamniasis (CL), has several limitations. Therapy is long, requiring repeated doses and the adverse reactions are frequent. Topical treatment is an attractive alternative for CL, offering significant advantages over systemic therapy: fewer adverse effects, ease of administration, and lower costs. AREAS COVERED: This review covers, from 1984 to the present, the progress achieved for the development of topical treatment for CL, using different drugs such

Expectations about the effectiveness of pain- and itch-relieving medication administered via different routes.

BACKGROUND: Placebo effects on pain have been found to vary in size for different routes of medication administration (e.g. oral vs. injection). This has important implications for both clinical research and practice. To enhance our understanding of these differential placebo effects, research on the underlying expectations about multiple routes and symptoms other than pain is vital. METHODS: A cross-sectional, Internet-based survey was conducted in a representative sample of the Dutch population (n = 508). Respondents rated the expected effectiveness of pain- and itch-relieving medication in

Clascoterone 1% in the Treatment of Acne: A Review of its Efficacy, Safety, and Therapeutic Positioning.

Topical clascoterone (TC) is the first topical antiandrogen approved by the Food and Drug Administration (FDA) for the treatment of acne and recently approved in Europe. Through bibliographic searches in Medline and Google Scholar, we conducted a narrative review to assess the usefulness of TC in acne management. Several randomized clinical trials have demonstrated its efficacy and safety (with virtually no systemic adverse effects), but few have compared its effectiveness with other topical agents or its use in combination. It is a highly interesting therapeutic alternative, particularly for

5α-Tetrahydrocorticosterone: A topical anti-inflammatory glucocorticoid with an improved therapeutic index in a murine model of dermatitis.

BACKGROUND AND PURPOSE: Glucocorticoids are powerful anti-inflammatory drugs, but are associated with many side-effects. Topical application in atopic dermatitis leads to skin thinning, metabolic changes, and adrenal suppression. 5α-Tetrahydrocorticosterone (5αTHB) is a potential selective anti-inflammatory with reduced metabolic effects. Here, the efficacy and side-effect profile of 5αTHB were compared with hydrocortisone in preclinical models of irritant dermatitis. EXPERIMENTAL APPROACH: Acute irritant dermatitis was invoked in ear skin of male C57BL/6 mice with a single topical applicatio

Topical and oral applications of Aloe vera improve healing of deep second-degree burns in rats via modulation of growth factors.

INTRODUCTION: Burn injuries are underappreciated injuries that cause significant morbidity and mortality. Burn injuries, especially severe burns, trigger immunological and inflammatory responses, metabolic abnormalities, and distributive shock, all of which can be extended to multiple organ failures. Aloe vera (A. vera) has been exploited for its medicinal properties for centuries. The goal of the present study is to examine the therapeutic effect of topical and oral administration of A. vera against deep second-degree burn in rats. MATERIALS AND METHODS: skin burn was created on the back of
