The comorbidity of disorders and chronology of first symptoms of depression, agoraphobia, and panic disorder were investigated. The Diagnostic Interview Schedule was administered to 3258 household residents. Strong associations were shown among all three disorders. We explored a causal sequence between panic and avoidance to provide recommendations for psychotherapy, pharmacotherapy, and their combination in treating agoraphobia. We produced a two-way [( imipramine hydrochloride vs placebo] by [office-based behavioral therapy vs in vivo exposure]) design by amalgamating two studies. Fourteen patients who met DSM-III criteria for agoraphobia with panic attacks received 200 mg desipramine (DMI) per day for 12 to 16 weeks on an open basis. Seven of eight patients who responded had steady state plasma DMI levels greater than 125 ng/ml, while five of six nonresponders had DMI levels less than 125 ng/ml. Following increases in dosage sufficient to raise DMI levels to greater than 150 ng/ml, four nonresponders improved dramatically. Implications of these findings are discussed briefly.

Sixty-two agoraphobic patient who had completed a controlled study of therapist-assisted in vivo exposure (flooding) and imipramine were assessed 1 month, 6 months, 1 year, and 2 years later. Overall, improvement during treatment was maintained throughout follow-up. At 1 month but not subsequently, imipramine and flooding had significant effects on central measures of agoraphobia. In this paper we studied the distribution of age at onset in 144 patients with panic disorder (PD) and/or agoraphobia (according to DSM-III criteria).
