Contact Tracing for Mpox Clade II Cases Associated with Air Travel - United States, July 2021-August 2022.

Monkeypox virus (MPXV) can spread among humans through direct contact with lesions, scabs, or saliva; via respiratory secretions; and indirectly from fomites; via percutaneous injuries; and by crossing the placenta to the fetus during pregnancy. Since 2022, most patients with mpox in the United States have experienced painful skin lesions, and some have had severe illness. During 2021-2022, CDC initiated aircraft contact investigations after receiving reports of travelers on commercial flights with probable or confirmed mpox during their infectious period. Data were collected 1) during 2021, w…

Ejection associated injuries within the German Air Force from 1981-1997.

From 1981-1997 there were 86 ejections from 56 aircraft within the German Air Force. Of these, 24 accidents were associated with the F-104 Starfighter, 14 with the PA 200 Tornado, 12 from the F-4 Phantom, 5 from the Alpha Jet and 1 from a MiG 29 Fulcrum. One case involved a front seat pilot, who had already sustained fatal injuries from midair collision, being command ejected by the rear seat pilot. The remaining 85 ejections are the basis of this study. One weapons system officer died from hypothermia after landing in the sea and another from bleeding into the medulla oblongata after flailing…

From random walks to thermal rides: universal anomalous transport in soaring flights.

Cross-country soaring flights rely on intermittent atmospheric updrafts to cover long distances, producing trajectories that alternate between rapid relocation and local exploration. From a large dataset of paraglider, hang glider and sailplane flights, we uncover a universal transport law: beyond short ballistic times, horizontal motion is persistently sub-ballistic, with a Hurst exponent ≈0.88 largely independent of aircraft type. Phase-resolved analysis using a probabilistic segmentation method shows that this scaling arises from the fundamentally intermittent, two-dimensional and direction…

How a Scoville aneurysm clip fought in the Cold War and helped to establish neurosurgery in Iceland.

It can be said that the specialty of neurosurgery in Iceland had its beginnings on November 30, 1971, with the arrival of a huge American C-130 Hercules aircraft. It was carrying a small package containing Scoville aneurysm clips. They were sent to the late Bjarni Hannesson (1938-2013), who had received his neurosurgical training in 1967-1971 at the Dartmouth-Hitchcock Medical Center (then known as Mary Hitchcock Memorial Hospital and located in Hanover, New Hampshire). He used one to clip the right posterior communicating artery aneurysm of a 34-year-old fisherman, who recovered well. The app…

Foreign Object Debris Detection for Optical Imaging Sensors Based on Random Forest.

In recent years, aviation security has become an important area of concern as foreign object debris (FOD) on the airport pavement has a huge potential risk to aircraft during takeoff and landing. Therefore, accurate detection of FOD is important to ensure aircraft flight safety. This paper proposes a novel method to detect FOD based on random forest. The complexity of information in airfield pavement images and the variability of FOD make FOD features difficult to design manually. To overcome this challenge, this study designs the pixel visual feature (PVF), in which weight and receptive field…

G-Induced Alterations of Consciousness in UK Military Aviation.

INTRODUCTION: Despite multiple risk mitigations, the adverse effects of +Gz acceleration on cerebral circulation continue to cause military aircraft incidents. A previous survey of Royal Air Force aircrew in 2012 reported that 37.2% had experienced at least one episode of G-induced alteration of consciousness. The aim of the present study was to determine whether the introduction of updated centrifuge training and improved countermeasures in training aircraft has reduced the prevalence of these incidents. METHODS: An anonymous online survey of all UK Armed Forces pilots was carried out in 202…

Spatial Disorientation Event During Flight Due to Proposed Reverse-Dip Visual Illusion.

BACKGROUND: Spatial disorientation is a leading nontechnical cause of fatal military aviation accidents, triggered by insufficient or misleading cues mainly from the visual and vestibular systems. Spatial disorientation accounts for 20-38% of fatal military aviation accidents, but case reports describing specific illusions during actual flight are rare. CASE REPORT: On a clear night, an aerial refueling tanker was cruising at 16,000 ft (4877 m), while an F-15I fighter aircraft was 3 mi behind and 971 ft (296 m) below. About 100 s before a near collision, the fighter pilot switched to Air-to-A…

Cardiac Rupture Due to a Fall from Height: The 'Water Hammer' Effect.

BACKGROUND: Cardiac injury in trauma patients can be secondary to either blunt or penetrating trauma and is a significant cause of death. The commonest etiological factors for blunt cardiac injury include motor vehicle collisions, falls, and crush or blast injuries. The incidence of blunt cardiac injury following falls is reported to be between 5 and 50%.CASE REPORT: A combat pilot lost his life in an aircraft accident. Although he had ejected successfully just before the aircraft caught fire and his parachute had deployed fully, it was engulfed in the ball of fire rising up from the burning a…

Aviation-related injury morbidity and mortality: data from U.S. health information systems.

INTRODUCTION: Information about injuries sustained by survivors of airplane crashes is scant, although some information is available on fata … he sample. The average hospital stay was 6.3 d and 2% died in hospital. Occupants of non-commercial aircraft accounted for 32% of patients, parachutists for 29%; occupants of commercial aircraft and of unpowered aircraft each constituted 11%. Lower-limb fracture was the most common injury in each category, constituting 27% of the total, followed by head in …

The toxicological examination of the victims of the British Air Tours Boeing 737 accident at Manchester in 1985.

The results of toxicological analyses of the body fluids of the victims from the accident involving the British Air Tours Boeing 737 in August 1985 are presented for carboxyhemoglobin, cyanide, and volatiles. All the victims except one had raised concentrations of carbon monoxide. All the victims had raised concentrations of cyanide. All the victims showed the presence of volatile substances in the blood. Autopsies revealed that all the victims had carbon particles in the trachea and bronchi. Thus, all the victims must have inhaled fire products in the burning aircraft cabin. Six victims had c…

Injury in U.S. Army helicopter crashes October 1979-September 1985.

All U.S. Army class A and B mishaps of four types of helicopters occurring from 1 October 1979 through 30 September 1985 were reviewed. During this 6-year period, there were 298 crashes involving 303 aircraft. There were 1,060 individuals aboard the crashed aircraft and 611 were injured, 136 fatally. The most common cause of injury was the "secondary impact" caused by collapse of structure into occupied areas, by inadequate restraint of the occupants which allowed them to flail into structure, or by a combination of both mechanisms. Injury solely related to acceleration occurred infrequently. …

Human factors in aviation crashes involving older pilots.

BACKGROUND: Pilot errors are recognized as a contributing factor in as many as 80% of aviation crashes. Experimental studies using flight simulators indicate that due to decreased working memory capacity, older pilots are outperformed by their younger counterparts in communication tasks and flight summary scores. OBJECTIVE: This study examines age-related differences in crash circumstances and pilot errors in a sample of pilots who flew commuter aircraft or air taxis and who were involved in airplane or helicopter crashes. METHODS: A historical cohort of 3306 pilots who in 1987 flew commuter…

Headache associated with airplane travel: a rare entity.

Airplane travel headache is rare and has recently been described as a new form of headache associated with a specific situation. Of the 1,20 … d as having an intense jabbing or stabbing character that occurred exclusively and maximally during aircraft landing or take-off, following which pain intensity subsided . This rare headache felt on aircraft descent is probably due to the squeeze effect on the frontal sinus wall, when air trapped inside it contracts producing a negative pressure leading to …

Abbreviated closure for remote damage control laparotomy in extreme environments: A randomized trial of sutures versus wound clamps comparing terrestrial and weightless conditions.

INTRODUCTION: Far-Forward Damage Control Laparotomies (DCLs) might provide direct-compression of visceral hemorrhage, however, suturing is a limiting factor, especially for non-physicians. We thus compared abbreviated skin closures comparing skin-suture (SS) versus wound-clamp (WC), on-board a research aircraft in weightlessness (0g) and normal gravity (1g). METHODS: Surgeons conducted DCLs on a surgical-simulator; onboard the hangered-aircraft (1g), or during parabolic flight (0g), randomized to either WC or SS. RESULTS: Ten surgeons participated. Two (40%) surgeons randomized to suture in …

Simulator sickness in U.S. Navy flight simulators.

Flight simulators have become a major factor in pilot training. A general finding from Navy research on simulator design is that equipment features that offer faithful representation improve pilot performance and promote pilot acceptance. To the extent that an aircraft produces motion sickness, its simulator should induce the same result. However, reports of simulator sickness appear to be increasing and a shortcoming in simulation is implied when these effects occur in simulators during maneuvers that do not occasion them in the aircraft. This article presents incidence data from surveys of t…

Loudness and short-term annoyance of sonic boom signatures at low levels.

Supersonic aircraft produce a sonic boom when flying faster than the speed of sound. To rule out detrimental effects for inhabitants of overflown areas, civil supersonic flights (like the Concorde) were only allowed to fly at supersonic speed when over water. Due to the progress in aircraft design, the supersonic boom may be reduced considerably in the future. In this study, listening tests were carried out with a variety of low boom simulations and conventional sonic boom signatures in a similar level range. Participants rated the loudness and the short-term annoyance of 24 sonic boom signals…

[Large aerobatic G-loads and degenerative-dystrophic changes of the spine in pilots: a new problem?].

Based on the analysis of available literature, a quantitative estimation of the role of chronic exposure to large aerobatic g-loads on modern aircraft in the risk of degenerative/dystrophic shifts in the spinal lumbar zone of flying personnel was attempted. In the opinion of the author, these g-loads increase the probability of spinal osteochondrosis in 1.5 to 1.75 times.
