[Is the increase in brain volume of abstinent alcoholics a sequela of rehydration?].

A group of 30 alcoholics as identified using the MALT score were examined both within the first 36 h of alcohol withdrawal and then again at the end of a 10-day treatment in hospital. We looked for clinical signs of dehydration, water and electrolyte imbalance, and concentration of steroid metabolites in the urine. Cranial computed tomography was also performed twice, again at the beginning and end of the 10-day period. The size of cerebrospinal fluid spaces was calculated through measuring the frontal interhemisphere distance, the width of cortical sulci, the ventricle III diameter, the Cella…

Stainable iron in gastric and duodenal mucosa of primary hemochromatosis patients and alcoholics.

The presence of iron in gastric and duodenal mucosa was investigated with Perl's stain in endoscopic biopsies from 13 patients with overt pr … cases. A similar distribution of iron overload, usually of lesser degree, was also observed in five alcoholics. Serum ferritin levels and the degree of gastric and/or duodenal iron deposits did not correlate in either hemochromatosis patients or alcoholics. No gastric or duodenal siderosis was observed in nonulcer dyspepsia cases. The absence of gastric and …

Reliability of retrospective estimates of alcohol consumption during pregnancy by recovering women alcoholics.

Twenty-six recovering women alcoholics were interviewed concerning their alcohol consumption during a pregnancy prior to their recovery. Several months later they were reinterviewed. Test-retest correlations were found to be high. Reliability was not related to the length of time since the pregnancy or to the test-retest interval. The frequency of consumption reported did not differ significantly from test to retest. However, the reported amount of consumption per occasion was significantly lower at the second interview. Reports of binge drinking were particularly unreliable. Results should be…

Visual electrophysiology and psychophysics in chronic alcoholics and in patients on tuberculostatic chemotherapy.

Twenty-one chronic alcoholics and 15 patients on tuberculostatic therapy were examined on repeated occasions using flash electroretinography (ERG), flash and pattern visual evoked responses (VER), visual fields, and flicker fusion thresholds. For both groups of visually symptomless patients, there is evidence of retinal abnormalities although more abnormal results are seen for the VER's. The results indicate that the mode of action of alcohol and tobacco differs from that of the tuberculostatic drugs. For ethambutol and possibly isoniazid therapy, the larger changes which are seen in the flash…

Peripheral and central vestibular disorders in alcoholics. A three-year follow-up study.

Vestibular disorders were found in all 33 Japanese men who had been drinking alcohol for over 20 years and who had abstained from alcohol for over 3 months before the tests were done. Six, 12, and 15 patients showed peripheral, central, and combined types of vestibular disorders, respectively. The peripheral disorder occurred in younger alcoholics (under 50 years old) with a drinking history of 20-30 years, and the combined disorder in older patients with a longer drinking history. The vestibular disorders, however, were not observed in any of 16 control non-alcoholics. A follow-up study on se…

Four pillars of spirituality: an interpretive guide for the 12 Steps of Alcoholics Anonymous.

BACKGROUND: Developed over nine decades ago, the 12 Steps were intended to provide structured and clear guidelines to promote recovery from alcohol use. The steps are designed to help individuals find a sense of purpose, meaning, and hope. The 12 Steps are meant to be guides for an ongoing process or spiritual progress. A more concrete way of understanding the steps relative to spiritual principles may be instructive for those working the steps and seeking a fulfilling meaning and purpose for their lives. PURPOSE: This paper provides a framework for understanding the 12 Steps in accord with f

Biochemical changes in chronic alcoholics in Port Harcourt: the report of a pilot survey.

AIMS AND OBJECTIVES: Alcohol abuse is a major public health problem and one of the leading causes of preventable death. Data on the frequency of biochemical abnormalities among alcoholics in the Niger Delta region is unavailable. We therefore conducted this pilot study to determine the type of biochemical abnormalities amongst a group of chronic alcoholics in Port Harcourt, Nigeria. SUBJECTS AND METHODS: Thirty (30) subjects aged 40 50 years with a daily alcohol consumption of more than 100g formed the study group. Thirty (30) aged matched controls from the same locality who were non-alcoholi…

Relationship between plasma folate and homocysteine concentrations in alcoholics according to liver enzyme activity.

The aim of this study was to evaluate the relationship between folate and homocysteine levels in alcoholics taking into consideration the liver enzyme activity as sensitive markers of hepatocellular injury. Folate and homocysteine concentrations did not differ between alcoholics classified according to the liver enzyme activity. The association between folate and homocysteine levels exists in the alcoholics with normal liver enzyme activity and in the controls. Therefore, we concluded that before the liver hepatocellular injury due to alcohol abuse, the correlation between folate and homocyste…

Event-related oscillations in offspring of alcoholics: neurocognitive disinhibition as a risk for alcoholism.

BACKGROUND: Event-related oscillations (EROs) are increasingly being used to assess neurocognitive functioning in normal and clinical populations. The current study compares different frequency activities in offspring of alcoholics (OA) and in normal control subjects (NC) to examine whether the OA group exhibits any abnormality in oscillatory activity while performing a Go/NoGo task. METHODS: The S-transform algorithm was employed to decompose the electroencephalographic (EEG) signals into different time-frequency bands, and the oscillatory responses in the P300 time window (300-700 milliseco…

Concentration of ethanol and other volatile compounds in the blood of acutely poisoned alcoholics.

The presence of volatile compounds, such as acetone, acetaldehyde, methanol, ethanol, isopropanol, and n-propanol, in the blood of 169 acutely poisoned alcoholics was determined. The clinical diagnosis of addiction was made on the basis of a patient interview as well as physical, psychological, and psychiatric examination. At the time of the patients' admission to the clinic, the mean concentration of ethanol in blood was 3.14 +/- 1.10 g/l and its elimination rate in the studied group was 0.27 +/- 0.08 g/kg/hr, an elimination rate significantly higher (P <.001) than that of social drinkers, wh…

Natural recovery in a community-based sample of alcoholics: study design and descriptive data.

The majority of alcoholics who recover do so without the benefit of treatment, yet little is known about these individuals and the factors associated with their success. A better understanding is needed of the self-initiated and maintained change processes ("natural recovery") associated with such recoveries. In the current study, initiated in 1997, we followed up alcoholics identified in four previous community surveys and conducted a brief physical examination and an in-depth interview assessing factors thought to be associated with "natural recovery" or entry into treatment. Here we present…

Variable dose naltrexone-induced hypothalamic-pituitary-adrenal stimulation in abstinent alcoholics: a preliminary study.

Opiate antagonists have been found to stimulate the hypothalamic-pituitary-adrenal axis. However, despite established usefulness in the management of alcoholism, systematic, oral dose-titrated natrexone-induced hypothalamic-pituitary-adrenal stimulation has never been studied in alcoholics. Six patients (5 males, 1 female) with DSM-IV alcohol dependence, who were at least 4 weeks abstinent from any alcohol [mean 55 days (+/-SE 7.5)], were given four challenges of oral naltrexone (0, 25, 50, and 100 mg) in a randomized order at least 3 days apart, after an overnight fast. Naltrexone was adminis…

Central and peripheral motor conduction time in chronic alcoholics with polyneuropathy and/or spasticity.

We measured central and peripheral motor conduction time to demonstrate lesions in the upper and lower motor neurons of 11 chronic alcoholics with polyneuropathy without spasticity, 7 chronic alcoholics with spasticity with and without alcoholic polyneuropathy, and 16 healthy volunteers as controls. Peripheral motor conduction time was significantly prolonged in all extremities in all of the chronic alcoholics, and was accompanied by a considerable reduction in motor conduction velocity. Central motor conduction time was considerably prolonged in the lower extremities of the alcoholics with sp…

Regional brain metabolic response to lorazepam in alcoholics during early and late alcohol detoxification.

Changes in GABA function have been postulated to be involved in alcohol tolerance, withdrawal and addiction. In this study we measured regional brain metabolic responses to lorazepam, to indirectly assess GABA function (benzodiazepines facilitate GABAergic neurotransmission), in alcoholics during early and late withdrawal. Brain metabolism was measured using PET and 2-deoxy-2[18F]fluoro-D-glucose after placebo (baseline) and after lorazepam (30 micrograms/kg intravenously) in 10 alcoholics and 16 controls. In the alcoholics evaluations were performed 2 to 3 weeks after detoxification and were …

Reduced fatty acid ethyl ester synthase activity in the white blood cells of alcoholics.

PURPOSE: Fatty acid ethyl esters (FAEEs), esterification products of ethanol and fatty acids, have been implicated as mediators of ethanol-i … plain the presence of FAEE in the serum. The lower enzyme activity observed in white blood cells of alcoholics from a detoxification center may be the result of years of ethanol abuse or it may be that alcoholics congenitally have low levels of FAEE synthase. If the latter is true, this finding may explain in part the genetic predisposition of many alcoholic …

An assessment of hypothalamo-pituitary-adrenal axis functioning in non-depressed, early abstinent alcoholics.

Chronic alcohol consumption has been shown to be associated with abnormalities in the regulation of the hypothalamo-pituitary-adrenal (HPA) axis in humans. However, conflicting data exist in the literature, with particular regard to studies performed in actively drinking or withdrawn alcoholics; in addition, the frequent presence of depressive disturbances in such patients may importantly affect the neuroendocrine findings. In this study, we investigated HPA function in 12 male alcoholics, in whom the presence of depression and other possible confounding factors was excluded, during the first …

Hostility is associated with a low prolactin response to meta-chlorophenylpiperazine in abstinent alcoholics.

The prolactin response to the administration of serotonin agonists is an index of central nervous system serotonin (5HT) activity. This index is blunted in association with hostile aggression in personality-disordered individuals without substance abuse. We tested whether prolactin response to the partial 5HT agonist meta-chlorophenylpiperazine (MCPP) (0.35 mg/kg po) was associated with measures of trait hostility in alcoholics who were completing a 3-week rehabilitation program after medical detoxification. We also tested whether the same 5HT index differed in the group of alcoholics compared…
