Horizontal Bone Augmentation with Simultaneous Implant Placement in the Aesthetic Region: A Case Report and Review of the Current Evidence.

This study aimed to describe a case of simultaneous guided bone regeneration (GBR) and implant placement in a patient with prior aesthetic implant failure, focusing on achieving optimal aesthetic and functional outcomes, and to perform a literature review of the current evidence. A 38-year-old male presented with an improperly positioned implant at the level of the right upper central (tooth 2.1), leading to aesthetic and functional issues. The initial assessment included a panoramic radiograph revealing marginal bone loss and an enlarged peri-implant space, necessitating implant removal. Foll

Outcomes of alveolar segmental 'sandwich' osteotomy with interpositional particulate allograft for severe vertical defects in the anterior maxilla and mandible.

The purpose of this study was to report the outcomes of interpositional osteotomy with mineralized allograft in the treatment of alveolar vertical defects in preparation for implant placement. Thirteen defects (11 maxillary and two mandibular) were treated with osteotomy segments ranging in length from two to five missing teeth. The segments were positioned 5-7 mm coronally, with the gap space filled with allograft and then fixated with titanium hardware. Vertical bone augmentation was analyzed by superimposing pre- and post-surgical cone beam computed tomography images and stratified based on

Titanium mesh for bone augmentation in oral implantology: current application and progress.

Guided bone regeneration (GBR) is an effective and simple method for bone augmentation, which is often used to reconstruct the alveolar ridge when the bone defect occurs in the implant area. Titanium mesh has expanded the indications of GBR technology due to its excellent mechanical properties and biocompatibility, so that the GBR technology can be used to repair alveolar ridges with larger bone defects, and can obtain excellent and stable bone augmentation results. Currently, GBR with titanium mesh has various clinical applications, including different clinical procedures. Bone graft material

Prospective evaluation of morbidity in patients who underwent autogenous bone-graft harvesting from the mandibular symphysis and retromolar regions.

BACKGROUND: Autogenous bone grafts are the most favorable and predictable types of bone graft due to their osteogenic, osteoinductive, and osteoconductive properties. The advantages of harvesting from mandibular bone are that it requires only local anesthesia, is less invasive, has less morbidity, and avoids harvesting from distant sites, thus making it the best choice for maxillary bone reconstruction. PURPOSE: The aim of this study was to evaluate inferior alveolar nerve (IAN) neurosensory disturbances after autogenous bone graft harvesting from the mandibular symphysis and retromolar regio

Use of a 10,600-nm CO2 Laser Mandibular Vestibular Extension in a Patient With a Chromosomal Abnormality.

Vestibuloplasty involves a series of surgical procedures designed to restore alveolar ridge height by lowering the muscles attached to the buccal, labial, and lingual aspects of the jaws. The technique is indicated in cases of insufficient vestibular depth that may result from atrophy of the alveolar ridge and/or high attachment of muscle or movable mucosa. This article focuses on a carbon dioxide (CO2) laser vestibular extension procedure performed in a patient with Klinefelter syndrome, which is caused by a chromosomal abnormality. The 10,600-nm CO2 laser is shown to offer several advantages

Effectiveness of three different alveolar ridge preservation techniques: a pilot randomized controlled trial.

The aim of this pilot study was to obtain preliminary data regarding the effectiveness of three different alveolar ridge preservation modalities as compared with a control. Subjects in need of single-rooted tooth extraction were recruited and randomly allocated to one of four treatment groups: group 1 (control)--collagen plug; group 2--socket grafting and polytetrafluoroethylene (PTFE) barrier; group 3--socket grafting, buccal overbuilding, and PTFE barrier; group 4--socket grafting, collagen barrier, and PTFE barrier. The grafting material used in all groups was an allograft. At 16 weeks, sur

Hard and soft tissue augmentation in implant surgery: a case report.

Correct pre- and postimplant hard and soft tissue management is prerequisite to achieving optimal esthetic and functional outcomes after implant-prosthesis treatment. Various different methods for preimplant hard tissue augmentation in alveolus maxillary atrophy are described. In some cases, non-resorbable titanium-reinforced membranes are employed for vertical augmentation of the edentulous crest. Also well documented is soft tissue management through mucus-gingival surgery techniques that increase the thickness of peri-implant keratinized gingiva, enhancing soft tissue esthetic and health. T

[Technical note:simple tools in preimplant surgery].

Osteotomes, the bone scraper, and the bone trap are simple tools used for preprosthetic surgery. They are not very invasive, present little risks, and they can be used in private surgical practice with local anaesthesia. The osteotome preserves, dilates, and condenses the alveolar bone instead of eliminating it like drilling does. It does not require any motor or irrigation. It increases the width and the height of the alveolar crest. It allows the localized expansion of narrow or misshapen alveolar crest when its width ranges between 3 and 4.5mm. It also allows performing a localized sinus li

Outcomes and treatments of mal fractures caused by the split-crest technique in the mandible.

In this study, we investigated cases of mal fracture occurring during the split-crest procedure. In all subjects (six patients), the free cortical bone segment caused by the mal fracture was carefully maintained in the lateral position without fixation using a titanium plate or screw. On pre- and postoperative multiplanar reconstruction CT, the average total alveolar increase was 5.0 mm in the lower portion 1 mm from the top of the alveolar ridge, and the average total alveolar increase in the lower portion 11 mm from the top of the alveolar ridge was 2.2 mm. A total of 11 dental implants were

Alveolar Ridge Preservation: The Past, the Present, the Future.

Alveolar ridge preservation (ARP) has become a fundamental toll in implant dentistry, aiming to minimize dimensional changes of the alveolar socket following tooth extraction and improve the success of implant-supported prostheses. The aim of this perspective article was to describe the evolution of ARP from its initial approaches to current techniques and to anticipate what ARP may become in the future. Early ARP techniques primarily involved autogenous and allogeneic bone grafts, with later adoption of biomaterials such as xenografts, alloplasts, and autologous blood derivatives. The biologi

3D bio-printing of photocrosslinked anatomically tooth-shaped scaffolds for alveolar ridge preservation after tooth extraction.

Alveolar ridge preservation techniques have been developed as a possible method to maintain the optimum ridge contour and dimensions. Grafting a bone substitute is paramount to prevent alveolar ridge resorption after tooth extraction. However, it remains a great challenge to develop alveolar ridge preservation materials with sufficient mechanical strength, bioactivity, and osteoinductivity and favorable tooth extraction socket morphological matching. In this work, a novel photocrosslinked composite ink consisting of nacre, polyurethane (PU) and polyhedral oligomeric silsesquioxane (POSS) was p

Horizontal and vertical ridge augmentation for implant placement in the aesthetic zone.

Lack of sufficient bone to place an implant at the functionally and aesthetically most appropriate position is a common problem, especially in the upper anterior jaw. A surgical technique is proposed to augment the alveolar ridge for vertical and horizontal defects through a localized alveolar osteotomy and interpositional bone graft. Three bone cuts (two vertical and one horizontal) are made in the alveolar bone. This portion of bone is carefully down-fractured. The gap between this bone box and the alveolar bone is filled with an interpositional bone graft. An on-lay bone graft is placed in

Biologically-oriented Alveolar Ridge Preservation.

BACKGROUND: A recent systematic review failed to identify one approach for alveolar ridge preservation with superior outcomes. The present case series aimed to evaluate the dimensional changes of sites undergoing Biologically-oriented Alveolar Ridge Preservation (BARP). METHODS: The sockets were filled with a collagen sponge up to 4-5 mm from the most coronal extensions of the crest. Xenograft particles were placed to fill the coronal part. In cases with a compromised buccal/lingual bone, an additional collagen sponge was interposed between the residual cortical bone plate and the mucoperiost

Guided bone regeneration using K-incision technique.

PURPOSE: The present study describes 3 patients with chronic periodontitis and consequent vertical resorption of the alveolar ridge who were treated using implant-based restoration with guided bone regeneration (GBR). METHODS: After extraction of a periodontally compromised tooth, vertical bone augmentation using a K-incision was performed at the healed, low-level alveolar ridge. RESULTS: The partial-split K-incision enabled soft tissue elongation without any change in buccal vestibular depth, and provided sufficient keratinized gingival tissue during GBR. CONCLUSIONS: Within the limits of

Localized ridge augmentation using a micro titanium mesh: a report on 27 implants followed from 1 to 3 years after functional loading.

The present paper describes the clinical and radiographic healing results of 27 implants followed from 1 to 3 years after functional implant loading. Prior to implant placement, alveolar ridges with insufficient bone volume were augmented using autogenous bone grafts and a micro titanium mesh for graft stabilization. After a mean interval of 5.2 months implants were installed. Following an osseointegration period of on average 7.2 months, implants were supplied with suprastructures. The mean loading period for the 27 implants was 21 months. All implants exhibited ankylotic stability and health
