Fatal anaphylaxis after intravenous iron dextran.

This report records the first reported case of a fatal anaphylactic reaction to an intravenous infusion of iron dextran. An elderly woman was given an infusion of iron dextran in 5% dextrose. Shortly after the infusion started, she developed laryngeal stridor, shock, respiratory arrest, and died despite attempts at resuscitation. Necropsy findings are described and are consistent with death due to anaphylaxis. With the increased parenteral use of dextran and iron dextran complexes it is important for the clinician to be aware of the hazards of anaphylactic reactions. Since the reactions genera…

Comparison of recent anaphylaxis diagnostic criteria in real life: Can more patients be diagnosed as having anaphylaxis?

INTRODUCTION: In 2020, World Allergy Organization (WAO) updated their diagnostic criteria for anaphylaxis, which differed as a result from the National Institute of Allergy and Infectious Diseases/Food Allergy and Anaphylaxis Network (NIAID/FAAN) criteria which were still used in the 2021 update of the European Academy of Allergy and Clinical Immunology (EAACI) anaphylaxis guideline. Our aim was to evaluate and to compare both diagnostic criteria and attempt to identify factors affecting severity of anaphylaxis. METHODS: The medical records of the patients who were evaluated with suspected an…

Criteria positive and criteria negative anaphylaxis, with a focus on undifferentiated somatoform idiopathic anaphylaxis: A review and case series.

Background: Undifferentiated somatoform (US) idiopathic anaphylaxis (IA) is considered a psychogenic disorder characterized by a lack of observable physical findings and poor response to treatment. Although failure to diagnose true anaphylaxis can have disastrous consequences, identification of US-IA is crucial to limit unnecessary expenses and use of health care resources. Objective: To better define the presentation and understand the potential relationship between US-IA and underlying psychiatric comorbidities. Methods: We retrospectively reviewed 110 visits by 107 patients to our instituti…

7 for 11: Updates in pediatric anaphylaxis.

Historically, one of the biggest barriers to understanding and diagnosing anaphylaxis has been a lack of consensus definition and clinical tools to support the standardization of anaphylaxis recognition and management. This review details important updates to the field on the subject of pediatric anaphylaxis in the past year. In 2025, a new consensus definition and clinical tool for anaphylaxis was published by an international expert panel with endorsement from 46 international, multidisciplinary medical and patient advocacy organizations. There have also been important changes to management …

Allergy to Semen: A Rare Cause of Anaphylaxis.

A previously healthy lady in her mid-30s presented to the medical emergency with recurrent episodes of flushing, rash, wheezing, chest tightness, palpitations, dyspnoea, and loss of consciousness. After five months of intensive medical investigations, she was referred to the gynaecological team, and a detailed clinical history made it clear that these episodes always occurred after unprotected penetrative sexual intercourse. The patient was thoroughly investigated with cardiological evaluation and imaging. Eventually, a semen allergy test was performed by injecting 1 cc of semen subcutaneously

Anaphylaxis and pregnancy: Epidemiology, elicitors, and recurrences.

BACKGROUND: Anaphylaxis during pregnancy implies a risk to both mothers and newborns. However, limited data are available. Therefore, our aim is to explore patterns of hospital admissions diagnosed with anaphylaxis in pregnant women in France from 2012 to 2021, using a national hospital billing database and to evaluate potential determinants, trends and recurrences. METHODS: Descriptive study using data from the French Program for the Medicalization of Information Systems, from 2012 to 2021. We included all patients hospitalized for anaphylaxis using International Classification of Diseases (…

Recurrences of anaphylaxis: trends and risk factors.

PURPOSE OF REVIEW: Recurrences of anaphylaxis is a concern due to its unpredictability and long-term burden to patients and healthcare systems. This review examines recurrence rates, associated risk factors, and gaps in current knowledge to guide improved clinical management. RECENT FINDINGS: From 1240 initial records, 11 studies fulfilled the inclusion criteria of our systematic review. Recurrences of anaphylaxis rates range from 2.6 to 17.6% of patients after a first episode of anaphylaxis, with a rising trend over time. Personal history of food allergy is the most consistent associated ris…

Cardiac stress in a child with perioperative anaphylaxis.

A case of cardiac stress following a grade 4 perioperative anaphylaxis was identified. We speculate that this may be a case of Kounis or adrenaline, takotsubo, anaphylaxis and Kounis (ATAK) complex. This is the first such case reported from the UK. We propose performing routine cardiac investigations in patients with grade 4 anaphylaxis or chest pain following anaphylaxis to evaluate any similar future presentations.

Who is at-risk for severe anaphylaxis in France?

BACKGROUND: The understanding of risk factors related to severe anaphylaxis is key to implementing prevention strategies. We present the first French population-based nine-year anaphylaxis hospitalization study evaluating specific trends and factors related to severe anaphylaxis (SA), to support identification of phenotypes at-risk. METHODS: This study used descriptive data from the French hospitalization database for the years 2012-2021, and included all patients hospitalized with anaphylaxis using International Classification of Diseases (ICD)-10 codes listed as a primary diagnosis. SA were…

Life-threatening antibodies: The discovery of anaphylaxis.

It was at the turn of the 20th century, that immune serum was found both to save children dying from toxins of deadly pathogens, and to kill a dog within minutes following an injection of harmless doses of sea anemone toxins. This means that, before being formally identified in immune serum, antibodies were already known to be both protective and pathogenic. For this provocative finding, Charles Richet was awarded the 1913 Nobel Prize in Physiology or Medicine. Because, as its name said, anaphylaxis was understood as "the contrary of protection," unique mechanisms were found to explain it. Bec…

Knowledge gaps in the diagnosis and management of anaphylaxis.

Knowledge gaps in the diagnosis and treatment of anaphylaxis impede the clinician's ability to effectively manage patients with anaphylaxis. This review will emphasize the lack of a global consensus on defining and determining the severity of anaphylaxis; the need for validating biomarkers used for diagnosing anaphylaxis; and data collection deficiencies. Perioperative anaphylaxis has a wide differential diagnosis, often requires treatment beyond epinephrine, and poses a challenge for the clinician in identifying the responsible trigger(s) and in preventing future reactions. Consensus-derived …
