[Surgery for multiple aortic aneurysms with unstable angina pectoris; report of a case].

A 77-year-old female with unstable angina pectoris was referred to our hospital for further evaluation of multiple aortic aneurysms. Computed tomography showed descending thoracic (65 mm), thoracoabdominal (40 mm) and infra-renal abdominal aneurysm (50 mm). Initially, this patient underwent off pump coronary revascularization. On 11 days after initial surgery, descending thoracic aneurysm ruptured, followed by emergent descending thoracic and thoraco-abdominal aneurysm repair. Two months later from this aortic repair, this patient successfully underwent abdominal aortic aneurysm repair. At 3 y

Relationship between age and platelet activation in patients with stable and unstable angina.

The objective of this study was to examine platelet reactivity and activation as a function of age in patients with both stable and unstable angina. This was a retrospective cohort study to compare platelet function in older patients with and without acute coronary syndromes. Participants were 55 individuals presenting with acute coronary syndromes and 54 with stable angina whose age ranged from 38 to 92 years. Platelet reactivity was assessed by ADP-induced aggregation and activation was measured by p-selectin, total and active GPIIb/IIIa expression, and platelet-leukocyte aggregates in. Mult

Appropriateness of diagnosis of unstable angina pectoris in patients referred for coronary arteriography.

BACKGROUND: A diagnosis of unstable angina pectoris (UAP) often carries with it a decision to catheterize the patient promptly. However, UAP remains a clinical diagnosis, based mostly on a patient's clinical history and electrocardiogram (ECG) findings. OBJECTIVE: To evaluate whether the diagnosis of UAP is overused in patients referred for coronary arteriography. METHODS: Ninety-six patients with a diagnosis of UAP who were referred for invasive studies were re-examined clinically before catheterization. Coronarography was independently reviewed for correlation with clinical findings. RESU

Circulating platelet and neutrophil activation correlates with the clinical course of unstable angina.

Recent studies have suggested important roles of inflammation in the pathophysiology of unstable angina (UA). We investigated whether activation of the circulating platelets and neutrophils were implicated in inflammatory reactions associated with unstable angina Expressions of platelet P-selectin and neutrophil CD11b, and neutrophil-platelet aggregates were evaluated by flow cytometry in anticoagulated peripheral venous blood from 71 patients with UA and 22 patients with stable angina (SA). Expressions of platelet P-selectin and neutrophil CD11b, and neutrophil-platelet aggregates on the admi

Percutaneous coronary intervention with stent implantation in haemophilic A patient with unstable angina.

The prevalence of coronary artery disease (CAD) and acute coronary syndromes in patients with haemophilia is much lower than in general population and there is a lack of information regarding safe interventional or surgical treatment of CAD in haemophiliacs. This report presents a case of patient with moderate haemophilia A and unstable angina pectoris, who underwent successful coronary angioplasty. The patient was pretreated with factor VIII (before and after the procedure) and the incision site was sealed with vessel closure device. Additionally, the article discusses the issue of the safety

Women and men with unstable angina and/or non-ST-elevation myocardial infarction.

BACKGROUND: In unstable angina and/or non-ST-elevation myocardial infarction (UA/NSTEMI), sex-related differences in outcomes are less well known than in ST-elevation myocardial infarction (STEMI), where women experience worse outcomes than men. Our aim was a prospective comparison between men and women with UA/NSTEMI of baseline characteristics, in-hospital complications, mortality, reinfarctions and combined endpoint of mortality and/or reinfarction during hospital stay, at 30 days and at six months. METHODS: Initial medical treatment was given to 92 men and 47 women with UA/NSTEMI. Percuta

Disparities by insurance status in quality of care for elderly patients with unstable angina.

CONTEXT: Treatment disparities for socioeconomically disadvantaged populations have been widely reported, but few studies have sought explanations for these disparities. OBJECTIVE: To compare the quality of care for patients insured by Medicare alone, Medicare plus Medicaid, or Medicare plus private insurance and investigate mediators for potential disparities. DESIGN, SETTING, AND PARTICIPANTS: Retrospective, random chart review of 3122 African American or White Medicare patients >65 years of age hospitalized for unstable angina in 22 Alabama hospitals, 1993-1999. MAIN OUTCOME MEASURES: Ec

[Unstable angina with subvalvular aortic stenosis in a 65-year-old woman].

Acute rhinosinusitis is a common inflammatory condition characterized by the inflammation of the paranasal sinuses. The use of saline irrigation in the nasal passages is a well-established and effective approach for managing both acute and chronic rhinosinusitis (CRS), allergic or nonallergic, atrophic rhinitis, facilitating postoperative care following endoscopic sinus surgery as well [4]. Various topical agents can be added to lavage solution. Among this mupirocin has notable anti-staphylococcal properties. To study and compare the efficacy of mupirocin nasal irrigation with saline nasal irr

Persistent systemic inflammation in unstable angina is largely unrelated to the atherothrombotic burden.

OBJECTIVES: The aim of this study was to assess the relationship between systemic inflammation, atherosclerosis, and thrombosis in two distinct clinical models of atherothrombosis. BACKGROUND: Persistent unstable angina (UA) is commonly associated with coronary thrombosis and persistent systemic inflammation. METHODS: We assessed circulating markers of activation of the thrombotic and fibrinolytic cascades and systemic soluble and cellular markers of inflammation on admission in 40 patients with persisting UA (Braunwald class IIIB; group 1) and 30 patients with Leriche-Fontaine stage IIB-III

Rapid effect of pravastatin on endothelial function and lipid peroxidation in unstable angina.

BACKGROUND: Oxidative stress contributes to endothelial dysfunction, an important step to the pathogenesis of atherosclerosis and coronary events. Latest studies revealed the existence of pleiotropic and especially antioxidant properties of statins. We sought to examine the effects of pravastatin on lipid peroxidation and endothelial function, independently from lipid-lowering, in patients with unstable angina (UA). METHODS: Thirty-seven patients (males), 64.46+/-9.09 years, suffering from UA enrolled in the study. Patients were not on statin medication before admission and they received afte

Functional T-lymphocyte dichotomy in the peripheral blood of patients with unstable angina.

OBJECTIVES: Herein, we investigated the percentage of T-helper (Th1) and Th2 cells among the general T-cell population in the peripheral blood of patients with stable angina (SA) and unstable angina (UA). BACKGROUND: Recent evidence suggests that Th1 cells and the cytokines that they secrete (especially IFN-gamma) have a role in the activation of macrophages, promotion of clot formation and destabilization of atherosclerotic plaques. Thus, Th1 cytokines may contribute to the initiation and progression of UA. In contrast, cytokines secreted by Th2 cells (e.g. IL-10) are known to inhibit activa

Differences in human antioxidized LDL autoantibodies in patients with stable and unstable angina.

BACKGROUND: Autoantibodies to oxidized LDL (anti-oxLDL) have been found in the serum of patients with coronary artery disease (CAD). This study was designed to compare the differences in anti-oxLDL titers and isotypes in unstable and stable angina patients and to correlate these results with known markers of active inflammation in CAD. METHODS: Thirty patients from a tertiary referral general hospital with documented CAD were studied. Anti-oxLDL IgG titers and its isotypes, high sensitivity C-reactive protein (hsCRP) and serum amyloid A (SAA) were measured. RESULTS: The anti-oxLDL IgG titer

[Coronary artery stenting in the treatment of 5-fluorouracil-induced unstable angina].

Cardiotoxicity is a rare but very serious side effect of 5-fluorouracil (5-FU) treatment. Many theories have been suggested to explain the mechanism of this problem. Most commonly, coronary artery spasm is considered responsible in cardiovascular toxicity. Ischemic symptoms and signs related to 5-FU therapy are observed during the late phase of the administration of the drug. The close and careful monitoring of all patients, especially the ones with pre-existent coronary artery disease, during 5-FU infusion is mandatory. Because there is not a single and effective modality of prevention of 5-F

The management of unstable angina and non-ST-segment elevation myocardial infartion.

Patients presenting with unstable angina and non-ST elevation myocardial infarction (UA/NSTEM) have a highly variable course. Optimal management is critical because of the high risk of death or myocardial infarction (MI) in the ensuing 30 days. In this article, we review the therapeutic options available to clinicians. Anti-ischemic therapy with beta-blockers and nitrates should be considered in all patients without contraindications. Aspirin remains a cornerstone of antiplatelet therapy and has been shown to substantially reduce the risk of death or MI. Although the data are less robust, unfr

Interleukin-18 is a strong predictor of cardiovascular death in stable and unstable angina.

BACKGROUND: Interleukin (IL)-18 plays a central role in orchestrating the cytokine cascade and accelerates atherosclerosis and plaque vulnerability in animal models. However, epidemiological data evaluating the role of IL-18 levels in atherosclerosis are lacking. METHODS AND RESULTS: In a prospective study of 1229 patients with documented coronary artery disease, we measured baseline serum concentrations of IL-18 and other markers of inflammation. During the follow-up period (median, 3.9 years), 95 patients died of cardiovascular causes. Median serum concentrations of IL-18 were significantly

Sertraline treatment of major depression in patients with acute MI or unstable angina.

CONTEXT: Major depressive disorder (MDD) occurs in 15% to 23% of patients with acute coronary syndromes and constitutes an independent risk factor for morbidity and mortality. However, no published evidence exists that antidepressant drugs are safe or efficacious in patients with unstable ischemic heart disease. OBJECTIVE: To evaluate the safety and efficacy of sertraline treatment of MDD in patients hospitalized for acute myocardial infarction (MI) or unstable angina and free of other life-threatening medical conditions. DESIGN AND SETTING: Randomized, double-blind, placebo-controlled trial

Angiographic correlates of a positive troponin T test in patients with unstable angina.

OBJECTIVE: To study the angiographic correlates of cardiac troponin T (cTnT)-positive and -negative patients with unstable angina pectoris. BACKGROUND: A positive cTnT test identifies a high-risk subgroup of unstable angina pectoris patients. Only the high-risk cTnT-positive patients seem to benefit from a more aggressive antithrombotic treatment regimen. The underlying coronary pathology in cTnT-positive and -negative patients that explains the predictive power of cTnT on prognosis and response to antithrombotic therapy is largely unknown. METHODS: A total of 197 subsequently admitted patie
