Risk factors of oncogenic HPV infection in HIV-positive men with anal condyloma acuminata in Shenzhen, Southeast China: a retrospective cohort study.

BACKGROUND: Human immunodeficiency virus (HIV)-positive patients with anal condyloma acuminata (CA) present an increased risk of anal cancer progression associated with oncogenic human papillomavirus (HPV) infection. It is essential to explore determinants of anal infection by oncogenic HPV among HIV-positive patients with CA. METHODS: A retrospective cohort study was performed in HIV-positive patients with CA between January 2019 to October 2021 in Shenzhen, Southeast China. Exfoliated cells were collected from CA lesions and the anal canal of HPV genotypes detected by fluorescence PCR. Unco

[HPV-induced anal lesions].

Human papillomavirus (HPV) infections belong to the most common sexually transmitted infections. To date, more than 200 completely classified HPV-types have been reported, and those belonging to the genus alpha predominantly infect the anogenital region. Condylomata acuminata are caused by the two low-risk types HPV6 and HPV11 in more than 90 % of cases. Treatment of genital warts might be either ablative (e.g. electrocautery, surgical excision, or laser therapy) or topical (e.g. podophyllotoxine, trichloroacetic acid, or imiquimod), and depends on the size, location, morphology and anatomical

Malignant Transformation of Perianal Fistulas: A Systematic Review of the Literature.

Malignant transformation of a perianal fistula is rare, predominantly associated with chronic fistula. Carcinogenesis is presumably initiated by chronic inflammation, and its mechanism is not clearly understood. We aimed to study the clinicopathologic characteristics and treatment outcomes of patients with malignant perianal fistula. A systematic literature search was performed on the electronic databases of PubMed, Scopus, and Web of Knowledge. A single-arm meta-analysis of non-comparative studies was performed using OpenMeta[Analyst] software. Eleven retrospective studies, including 172 pati

Management of Perianal Sepsis in Neutropenic Patients With Haematological Malignancies-A Systematic Review.

INTRODUCTION: Despite neutropenia being considered a significant risk factor for acute perianal sepsis, there is a lack of consensus on treatment guidelines for perianal abscess in the immunocompromised patient, specifically in patients with neutropenia secondary to a haematological malignancy. The aim of the article is to review the available literature on. Whether surgical intervention produces better outcomes than a non-surgical approach for perianal sepsis in the immunocompromised patient with a haematological malignancy. METHODS: Relevant papers were identified through electronic databas

[Anal high grade dysplasia associated with Crohn's disease:a case report].

A 47-year-old male patient with anal pain underwent a colonoscopy 12 years after Crohn's disease diagnosis. A biopsy from the protuberance of the anal canal revealed a well-differentiated adenocarcinoma. A reexamination of the histopathological specimen at our hospital revealed anal high grade dysplasia (HGD), thereby changing the initial diagnosis. A distal colon resection and rectal amputation were performed, and the final diagnosis of the resected specimen was anal canal HGD. While HGD associated with Crohn's disease is rare in Japan, this case suggests the importance of endoscopic examinat

Ultrastructural and morphological analysis during progression of Bowen disease reveals a complex interplay between hyperkeratosis, cytokeratin expression, host immunity and amyloid deposition.

BACKGROUND: Bowen disease, one of the common skin cancers, is defined as squamous cell carcinoma in situ, characterized by atypical keratinocytes occupying the full thickness of the epidermis, and predominantly occurs on sun-protected skin. There is no existing data on the impact of tumour and immune cell interactions or cytokeratin expression on the pathology of Bowen disease. OBJECTIVES: We analysed dynamic changes in cytokeratin expression and immune cell composition during the development and progression of Bowen disease. MATERIALS & METHODS: Analysis was performed using immunohistochemi

Cancer of the anal region.

Anal canal accounts for 2% of all cancer and its incidence increases with age with a predominance in woman. About 80% of all primary anal canal cancers are squamous; adenocarcinoma arising from the glands or glandular ducts shows a behaviour that is similar to that of the adenocarcinoma of the rectum. Risk factors includes sexually transmitted infection with Human Papillomavirus, cigarette smoking, immunosuppression, and sexual practices. The standard treatment for anal canal is chemo - radiation with a combination of fluoropyrimidines and mitomycin or cisplatin. Salvage surgery may be necessa

Giant perianal condyloma acuminatum in an infant without sexual abuse.

Giant condyloma acuminatum is rare in infants. We report a case of giant perianal condyloma in a 10-month-old infant without sexual abuse. The patient was treated by surgical excision associated with electrocoagulation with no recurrence after 2 years. While both sexual and nonsexual transmissions are possible in patients with condyloma acuminatum, sexual abuse must always be considered in children.

Squamous cell carcinoma of the anal canal.

Anal squamous cell carcinoma (SCC) is a rare cancer and accounts for approximately 4% of all cancers of the lower alimentary tract. The dominant etiology is infection with human papilloma virus (HPV), which is the most common sexually transmitted disease in the United States. Integration of HPV DNA into the host genome seems to be the driving mechanism behind carcinogenesis. Vaccines directed against oncogenic HPV serotypes exist, and their utility for preventing anal neoplasia is under investigation. Additional risk factors for developing SCC include HIV infection, anal receptive intercourse,

[Diagnosis and treatment for carcinoma associated with anal fistula].

In this article, we focus on the diagnosis and treatment of carcinoma associated with anal fistula. Carcinoma associated with anal fistula is relatively rare, representing 6.9% of all anal carcinoma in Japan. It arises from long-standing anal fistula with more than 10 years duration. Clinical suspicion is crucial in diagnosis, however, early detection is challenging. A biopsy under anesthesia is often needed for histological confirmation of the tumor. Magnetic resonance imaging can facilitate the diagnosis and is essential to determine the range of the surgical resection. Abdominoperineal rese

Anal anatomy and normal histology.

The focus of this article is the anatomy and histology of the anal canal, and its clinical relevance to anal cancers. The article also highlights the recent histological and anatomical changes to the traditional terminology of the anal canal. The terminology has been adopted by the American Joint Committee on Cancer, separating the anal region into the anal canal, the perianal region and the skin. This paper describes the gross anatomy of the anal canal, along with its associated blood supply, venous and lymphatic drainage, and nerve supply. The new terminology referred to in this article may

Initial Experience with Topical Fluorouracil for Treatment of HIV-Associated Anal Intraepithelial Neoplasia.

Exposure to high-risk strains of human papillomavirus (HPV) promotes cellular dysplasia of the anal canal epithelium, potentially leading to anal intraepithelial neoplasia (AIN), which is in turn a precursor to invasive squamous cell carcinoma of the anus (SCCA). People with HIV infection who engage in anoreceptive intercourse are at heightened risk for acquiring HPV, AIN, and SCCA. Although intravenous 5-fluorouracil (5-FU) is commonly used for treatment of invasive SCCA, there has been little experience with the use of topical 5-FU as therapy for AIN. We retrospectively reviewed records of t
