Appendectomy: a risk factor for colectomy in patients with Clostridium difficile.

BACKGROUND: The appendix, considered an intestinal microbiota reservoir, may be protective against the risk of fulminant Clostridium difficile infection. METHODS: Retrospective analysis was performed in patients with C. difficile infection at St. Francis Medical Center from 2007 to 2011. Outcome of infection and history of appendectomy were compared. Statistical analysis was by chi-square and multivariate logistic regression. RESULTS: In total, 507 patients were hospitalized for C. difficile. Of 388 patients with intact appendix, 20 (5.2%) developed fulminant infection and required colectomy…

Comparison of various methods of mesoappendix dissection in laparoscopic appendectomy.

PURPOSE: Although laparoscopic appendectomy is one of the most commonly performed operations, operation procedures vary widely according to the surgeon. In particular, various methods using different instruments have been used for mesoappendix dissection, such as endostapler, endoclip (EC), Harmonic(®) (Ethicon Endo-Surgery, Cincinnati, OH) scalpel (HS), electrocautery, and LigaSure™ (Covidien, Mansfield, MA). Here we compared the results of mesoappendix dissection by EC, HS, and monopolar electrocautery (ME). SUBJECTS AND METHODS: The study was performed on 1178 patients who received laparos…

Laparoscopic appendectomy: results of a new technique for stump management.

OBJECTIVE: The purpose of the present study was to review a new laparoscopic technique for treatment of appendicitis. An earlier pilot study indicated the safety of the technique in addition to saved time and cost. MATERIAL AND METHOD: The electronic records were analyzed on appendix surgeries performed at our hospital between January 1, 2007 and December 31, 2011. RESULTS: The 91 patients who had an appendiceal stump closure using clips (viz., the Hem-o-lock clip) had a significantly shorter surgery and hospitalization than those whose appendiceal stump was closed using the standard loop st

The "all-in-one" appendectomy: quick, scarless, and less costly.

BACKGROUND: A technique for laparoscopic appendectomy (LAP APPY) that involves brief surgeon and operating room times, results in no appreciable scar, and requires few disposable supplies would be desirable. METHODS: During 2009, 508 children underwent LAP APPY at our institution including 398 (78%) for acute, non-perforated appendicitis. Our "all-in-one" operative procedure involves use of a single instrument through a side-arm viewing operative laparoscope which is inserted through a single, trans-umbilical port. Successful procedure completion rates and operative times ("cut-to-close") wer…

Appendectomy or right hemicolectomy in the treatment of appendiceal carcinoid tumors?

AIMS AND BACKGROUND: Carcinoids of the appendix continue to be of interest, despite their low incidence. There is still considerable controversy surrounding these tumors, especially with regard to the role of right hemicolectomy in the surgical management. The aim of this work was to explicate the current therapeutic knowledge and to review the criteria for the indications of appendectomy or hemicolectomy. METHODS: The records of patients who underwent appendectomies from 1990 to 2000 were analyzed. Seven patients were included in the study. The clinical data were reviewed for demographic det…

Laparoscopic appendectomy in children: use of the endoloop vs the endostapler.

HYPOTHESIS: Two techniques are used for laparoscopic appendectomy (LA): division of the mesoappendix with the harmonic scalpel and ligation of the appendix with an endoloop (EL), or division of the mesoappendix and appendix with an endostapler (ES). Using an ES is a cost-effective technique that provides an outcome benefit in children who require appendectomy. DESIGN: Case series. SETTING: Academic, tertiary care children's hospital. PATIENTS: Seventy-five children who underwent LA from January 1, 2002, to March 31, 2004. INTERVENTION: Laparoscopic appendectomy. MAIN OUTCOME MEASURES: Age…

[Laparoscopic appendectomy as training procedure for all stages of appendicitis].

Appendectomy is one of the most common procedures in general surgery. Appendectomy is routinely performed in our department laparoscopically and as a training procedure for all stages of appendicitis. Between 1.1.2003 till 31.7.2005 642 patients underwent appendectomy. 613 of them were performed laparoscopically with a conversion rate of 0,6% in uncomplicated findings and 8,1% in complicated findings (perforated, abscess and gangrenous appendicitis). The postoperative recovery after laparoscopic appendectomy was without any significant complication in 98.2% of the patients with acute appendici…

Laparoscopic appendectomy is a favorable alternative for complicated appendicitis in children.

Laparoscopic appendectomy in children is a generally accepted procedure for the treatment of non-complicated acute appendicitis. Nevertheless, the role of laparoscopy in complicated appendicitis is more controversial. The objective of this study was to examine the safety, efficacy and complications of laparoscopy in children with complicated appendicitis. This is a retrospective review of the children who underwent laparoscopic appendectomy for complicated appendicitis at King Khalid University Hospital, Riyadh, Saudi Arabia between January 1998 and March 2006. Complicated appendicitis include…

Emergent vs urgent appendectomy in children: a study of outcomes.

BACKGROUND: Emergent appendectomy (EA) in children is still considered surgical dogma and continues to be recommended as a standard of care. This study examined whether emergent operation has any outcome advantages over urgent operation. METHODS: The charts of children treated for appendicitis during a recent 28-month period at 2 children's hospitals, where appendectomies are not performed between midnight and 7 am, were reviewed. Outcomes were compared between patients who underwent EA (within 8 hours of presentation) vs those who underwent urgent appendectomy (UA, after 8 hours). RESULTS: …

Comparison of laparoscopic, open, and converted appendectomy for perforated appendicitis.

BACKGROUND: Perforated appendicitis is associated with a significant risk of postoperative abdominal and wound infection. Only a few controversial studies evaluate the role of laparoscopy in perforated appendicitis. The significance of conversion from laparoscopy to open appendectomy for perforated appendicitis is not well defined. Statistical analysis was performed using Student's t-test. METHODS: Data on 52 patients with perforated appendicitis were prospectively collected and retrospectively reviewed. Among these patients, 18 had laparoscopic appendectomies (LA); 24 had open appendectomies…

The management of appendiceal mass in children: is interval appendectomy necessary?

In this study we aimed to show that performing interval appendectomy is unnecessary in the management of appendiceal mass in children. Between 1990 and 1996, 866 patients were treated for appendicitis. Abdominal ultrasonography (USG) was performed in patients who were admitted with abdominal pain, vomiting, and fever accompanying a mass in the right lower quadrant. Seventeen patients (12 boys and 5 girls, with a mean age of 9.5 years) with a mass in the appendiceal lodge and no abscess formation were treated conservatively. Appendectomy was performed on any patients with perforated or unperfor…

Malignant Hyperthermia Crisis During a Laparoscopic Appendectomy: A Case Report.

Cases of malignant hyperthermia (MH) are uncommon, but when they are not recognized early, they can be fatal. Early signs include increased heart rate and end tidal CO2 (EtCO2) readings followed by increased temperature. This is the case of 53-year-old man who had a MH crisis after exposure to the volatile anesthetic sevoflurane. His increased EtCO2 and tachycardia coupled with exposure to known trigger is indicative of MH. The condition was quickly recognized, and he was successfully treated with dantrolene. He recovered without significant long-term complications. This case contributes to th

Appendectomy and Subsequent Type 2 Diabetes - A National Cohort study.

BACKGROUND: Acute appendicitis commonly necessitates surgical removal of the appendix which can cause a perturbation of the gut microbiota altering metabolic homeostasis. Previous studies have suggested a possible increased risk of developing type 2 diabetes (T2D) after appendectomy, although findings have been inconsistent. We aimed to evaluate whether appendectomy increased the incidence of T2D in a nationwide cohort. MATERIALS AND METHODS: In this register-based cohort study, all Danish citizens between 1995 and 2022 were included. Appendectomy cases were identified using national procedur…
