This is the report of two children who had prominent arachnoid granulations that were mistaken clinically for dermoid cysts. These lesions appeared in the midline, primarily posteriorly. During surgery, the lesions extended through the dura connected by a stalk. A pathologic examination confirmed the lesions to be arachnoid granulations.

Suprasellar arachnoid cysts are basal midline masses that represent a rare but surgically remediable cause of hydrocephalus and neurologic deficits. These cysts represent a diagnostic challenge and often go unrecognized for many years. The authors review the computed tomographic (CT) findings in seven patients with documented suprasellar arachnoid cysts and define previously undescribed diagnostic criteria. These cysts usually can be differentiated from cystic midline neoplasms by their CT density, homogeneity, and location as well as by their lack of fat, lack of calcification, and absence of

Spinal intradural arachnoid cysts (S-IAC) are rarely found and only 80 cases have been reported since 1915. We report two of them. A 68-year-old male had slowly progressive dull pain in both thighs for about 6 years prior to admission. Neurological examination and plain x-ray on admission showed no abnormality. Myodil myelography showed a S-IAC at T4 vertebral level. Surgical treatment of cyst wall excision resulted in success. The other case, a 39-year-old male suddenly noticed monoparesis and sensory disturbance of left leg in the morning. On admission were observed spastic monoparesis of th

Puberty developed in a boy before age 12 years despite a marked delay in skeletal development. A large suprasellar arachnoidal cyst was found. Such cysts may be a cause of precocious puberty.

A case of suprasellar arachnoid cyst and hydrocephalus diagnosed by computed tomography (CT) and positive-contrast ventriculography is presented. Transfrontal exploration of the 3rd ventricle revealed findings consistent with the cyst being, in fact, a large forward and upward diverticulum of the arachnoidal membrane of Liliequist. The authors suggest that some of the other similarly located arachnoid cysts are extensions of this membrane imperforate by previous infection, hemorrhage, or maldevelopment.

Familial congenital spinal arachnoid cysts causing progressive paraplegia are reported in two adolescent siblings as part of a hereditary syndrome. The other features of this unusual dominantly inherited disorder include double rows of eyelashes, partial ectropion of the lower eyelids, and acquired late-onset lymphedema of the lower extremities. The siblings' mother, who had roentgenographic evidence of a similar intraspinal lesion, was free of neurological symptoms but had the other characteristic features. Surgical treatment of the arachnoid cysts improves the progressive paraplegia.

This paper describes the clinical picture and radiological findings of 12 cases of "idiopathic" arachnoid cyst in the middle fossa which the authors have encountered during the past 12 years. Their results are as follows. 1) The age of the patient in 10 out of 12 cases was under 15 years of age, and 8 cases were males. The site of the cyst was on the left side in 9 cases. 2) On plain skull radiography, thinning and bulging of the temporal bone by the lesion, elevation of the lesser wing of the sphenoid, and forward projection of the greater wing of the sphenoid were observed in all cases. 3) O

Three cases of progressive hydrocephalus, two occurring in infants and one in a 12 year old girl who also exhibited precocious puberty, were found to be associated with large arachnoid pouches originating within the posterior fossa. The pathogenesis of such cysts is discussed with special reference to the possibility that their progressive distension results from CSF pulsations of venous origin. Both a direct method of treatment (opening the cyst into the adjacent subarachnoid space) and an indirect one (insertion of a ventriculo-atrial shunt) have been used with success.

BACKGROUND: Arachnoiditis refers to chronic inflammation of the arachnoid mater and subarachnoid space due to three major etiologies: chemical, mechanical, and postinfectious. As a rare disease with variable symptomatology and severity at presentation, arachnoiditis can be extremely debilitating with many complications, prominent among which is syringomyelia.
METHODS: We reviewed current literature concerning postinfectious spinal arachnoiditis and associated syringomyelia with emphasis on the treatment options that have been used to date and discuss their respective benefits and drawbacks.

BACKGROUND: Cryptococcus can cause meningoencephalitis (CM) among previously healthy non-HIV adults. Spinal arachnoiditis is under-recognized, since diagnosis is difficult with concomitant central nervous system (CNS) pathology.
METHODS: We describe 6 cases of spinal arachnoiditis among 26 consecutively recruited CM patients with normal CD4 counts who achieved microbiologic control. We performed detailed neurological exams, cerebrospinal fluid (CSF) immunophenotyping and biomarker analysis before and after adjunctive immunomodulatory intervention with high dose pulse corticosteroids, affordin…

Spinal arachnoiditis is a known but very rare late complication of subarachnoid hemorrhage (SAH). Since 1943, 17 cases of spinal arachnoiditis after intracranial hemorrhage have been reported internationally. The vast majority of these cases were related to aneurysmal SAH. All previously published cases have involved spinal arachnoiditis at the cervical and thoracic levels. In this report, we present an adult woman with lumbar spinal arachnoiditis causing cauda equina syndrome as a result of posterior circulation aneurysmal SAH. We believe this is the first reported case of this specific condi…

Tuberculous spinal arachnoiditis involving cauda equina is rare. A patient with lumbar tuberculous arachnoiditis in the absence of both vertebral and meningeal tuberculosis, which was mimicking spinal intradural extramedullary tumor is described here. Diagnosis was made based on intraoperative findings and was confirmed by histopathology. Surgical decompression along with a combination of steroid and antitubercular therapy resulted in a good outcome. At 3 months follow-up, the patient regained bladder control and was able to walk with support. Clinical features, magnetic resonance imaging, and…

STUDY DESIGN: This case report illustrates a patient with arachnoiditis ossificans of the cauda equina of sufficient severity to be demonstrable on a computed tomography scout view.
OBJECTIVES: The diagnosis of extensive arachnoiditis ossificans by computed tomography does not necessitate surgical intervention. The authors patient had only mild, chronic symptoms when treated expectantly.
SUMMARY OF BACKGROUND DATA: The value of axial computed tomography in visualizing and characterizing the lesions of arachnoiditis ossificans has been well documented. Arachnoiditis ossificans involving the c…

The article presents radiological signs of basal processes (arachnoiditis, arachnoencephalitis) associated with paranasal sinusitis. gamma-Topography of the brain in inflammatory processes is distinguished by such typical radiological signs as a pathological accumulation of a radiopharmaceutical which is more often diffuse in nature. According to the findings of radionuclide cysternography the inflammatory process is characterized by a block of the subarachnoidal space of the frontobasal portions and an impairment of the CSF dynamics.
