Arm Contouring in the Massive-Weight-Loss Patient.

Brachioplasty is an increasingly popular procedure performed for improved arm contour in the massive-weight-loss population. There are challenging deformities presented in this population, such as redundant skin, posterior arm lipodystrophy, and loosening of fascial layers of the upper arm and chest wall that must be addressed to achieve successful contour of the arms. Common complications can be minimized with meticulous technique and knowledge of surgical anatomy. Additionally, brachioplasty can be combined with liposuction of the posterior arm as a safe and effective method for arm contouri…

Preferred Management of Primary Deep Arm Vein Thrombosis.

OBJECTIVE: Given its rarity, the management of primary upper extremity deep vein thrombosis is controversial. Although anticoagulation alone is commonly advocated for its treatment, it is unclear if this will reduce the risk of developing post-thrombotic syndrome (PTS). The aim of this "Evidence Driven" Clinical Scenario is to evaluate whether more aggressive treatments (including catheter directed thrombolysis or surgery) might help reduce the risk of PTS or recurrent venous thromboembolism in patients with primary upper extremity deep vein thrombosis (DVT). METHODS: An electronic systematic

Postocclusive reactive hyperemia in hand-arm vibration syndrome.

OBJECTIVES: To assess laser Doppler-recorded postocclusive reactive hyperemic responses in vibration-induced Raynaud's phenomenon and compare it with primary and secondary to sclerodermy Raynaud's phenomenon. MATERIAL AND METHODS: Thirty patients with vibration-induced Raynaud's phenomenon and 30 healthy controls and patients with primary and secondary to sclerodermy Raynaud's phenomenon were investigated. Fingerpulp skin blood flow was monitored by laser Doppler flowmetry during postocclusive reactive hyperemia test. RESULTS: Lower initial perfusion values were established in all the patien

Vascular hand-arm vibration syndrome--magnetic resonance angiography.

The diagnosis of vascular hand-arm vibration syndrome (HAVS) requires consistent symptoms, photographic evidence of digital blanching and sufficient exposure to hand-transmitted vibration (HTV; A(8) > 2.5 m/s2). There is no reliable quantitative investigation for distinguishing HAVS from other causes of Raynaud's phenomenon and from normal individuals. Hypothenar and thenar hammer syndromes produce similar symptoms to HAVS but are difficult to diagnose clinically and may be confused with HAVS. Magnetic resonance angiography (MRA) is a safe and minimally invasive method of visualizing blood ves…

[What is your diagnosis? A "fat arm". Onchocerciasis].

We report a case of edema of the right forearm with pruriginous papules and eosinophilia in the blood. The patient had lived in a forested area of Cameroon. The clinical, laboratory, and geographical findings led to a diagnosis of onchocerciasis, despite the absence of microfilaments on skin biopsy samples. The patient was successfully treated with ivermectin and doxycycline.

Mantle cell lymphoma relapsing at the lymphedematous arm.

Lymphedema (LE) is a chronic medical condition characterized by lymphatic fluid retention, resulting in tissue swelling. Cancer treatments i … ithin the LE region. Here we describe the case of a 45-year-old woman with a massive LE of the left arm, as a consequence of previous breast cancer, who was diagnosed after 4 years of stage IV mantle cell lymphoma (MCL). The patient, after obtaining complete remission with chemotherapy and autologous hematopoietic stem cell transplant, had a relapse o …

Should surgeons use arm restraints after cleft surgery?

BACKGROUND: Most cleft surgeons require children to wear postoperative arm restraints although the literature suggests that there is no difference in early complications. The aim of this study was to determine if the use of postoperative arm restraints was effective in preventing early postoperative complications. METHODS: We reviewed 120 consecutive primary cleft surgeries in which 1 surgeon used arm restraints in all patients and the other surgeon did not. Demographic information was obtained and complications were reviewed. We compared infection, fistula, and dehiscence between the 2 group…

C-arm-guided reduction of zygomatic fractures revisited.

BACKGROUND: Anatomic reduction of the zygomatic arch, a key surgical landmark for midfacial width and projection, is essential for the treatment of combined fractures of the zygomaticomaxillary complex and zygomatic arch. Reduction control in surgery for this common facial fracture would be facilitated by intraoperative real-time assessment using widely available and reliable equipment. Although C-arm fluoroscopy is routinely used in the repair of orthopedic fractures, its use in the maxillofacial region, particularly for combined zygomatic fractures, has been scarcely reported. METHODS: We p…

Contralateral pedicled lateral arm flap for hand reconstruction.

Large defects of the hand usually require reconstruction with flap resurfacing to provide durable coverage and preserve function. Pedicled flaps are preferred when there are no vessels available or they are of questionable quality for anastomosis. Such clinical settings may be due to the primary pathology, or when the vessels are preserved for secondary reconstruction. We introduce the contralateral pedicled lateral arm flap as a viable alternative. From 1988 to 2006, we treated 22 consecutive patients aged between 6 and 70 (18 males and 4 females). The indications for flap reconstruction inclu…

Multiple arm lipomatosis and posterior interosseus nerve palsy.

BACKGROUND: Lipomas are common benign soft tissue tumours which tend to be indolent and risk free. Lipomas rarely spread in the deep soft tissue causing posterior interosseous nerve (PIN) neuropathy. METHODS (CASE DESCRIPTION): We present two patients with multiple lipomatosis of the arms and PIN paralysis, with a brief review of the cases reported in literature. RESULTS AND CONCLUSION: We emphasize the role of electromyographic study as unique methodical capable to reveal an early radial nerve damage, permitting an optimal post-surgical nerve function recovering.

Arm swing is reduced in idiopathic cervical dystonia.

Arm swing is typically reduced in people with Parkinsonism, and also in those with pyramidal dysfunction. We have previously observed that patients with focal arm dystonia can also have reduced arm swing. However, arm swing has not been formally studied in adult-onset primary cervical dystonia (AOPCD). We assessed arm swing in 100 consecutive patients diagnosed with AOPCD and 50 healthy controls. Reduced arm swing was more common in patients with AOPCD compared with healthy controls (55% vs. 6%, P < 0.001) and was more often abnormal on the same side as the direction of head turning (P < 0.05)…

[Painful soft-tissue swelling of the upper arm].

Cat-scratch disease is an infection by Bartonella henselea and resolves spontaneously. We present a case of untypical Bartonella infection which was misinterpreted by MRI as a soft-tissue sarcoma of the upper arm. The histological findings of an incision biopsy were unspecific, without signs of malignancy. The tumor was excised completely and the final histology, however, showed no evidence of malignant changes. The Bartonella henselae infection could be confirmed by polymerase chain reaction. The untypical presentation of cat-scratch disease should be considered in the differential diagnosis …

[Colostomy morbidity in children with anorectal malformations (ARM)].

OBJECTIVE: To describe the incidence and type of complications presented in the patients with ARM treated with a colostomy. MATERIAL AND METHODS: The patients data with diagnostic of ARM and colostomy beetween 1994 and 2005 were revised. RESULTS: The colostomy was carried out in other centers in 185 patients (75.2%) and in our institution in 61 (24.8%). Two hundred eleven (85.8%) had a sigmoid colostomy. Only 124 patients had a diverting colostomy. Sigmoid colon was erroneously used as transverse in 2 cases. The space among the ostomies was very scarce in 10 and excessive in 5. The colostomy…
