INTRODUCTION: Stroke is an uncommon but pivotal prognostic factor in giant cell arteritis (GCA) and Takayasu arteritis (TAK). While both conditions are large vessel vasculitis stroke characteristics may differ between them, with potential implications for diagnosis and acute management. This study aimed to compare the epidemiological, clinical, and prognostic features of stroke in GCA and TAK.
METHODS: We conducted a multicenter retrospective cohort study including patients who met the ACR/EULAR 2022 classification criteria for GCA or TAK and experienced at least one imaging-confirmed stroke.…

The breast is an infrequently recognised site of primary giant cell arteritis. Two cases of giant cell arteritis affecting the breast are described and 11 previously described cases are reviewed. All cases presented with single or multiple breast masses, leading to a diagnosis before biopsy of suspected breast carcinoma. Symptoms similar to those of polymyalgia rheumatica occurred in about half of these patients, though the significance of these symptoms was only appreciated in retrospect. Clinical or pathological evidence of giant cell arteritis outside the breast (temporal artery, thyroid ar…

The annual incidence of giant cell arteritis (the term used in this study to encompass the syndromes of temporal arteritis and polymyalgia rheumatica, occurring either together or alone) was prospectively determined in a Danish county that had a population of approximately 200,000. In a single year, 46 new cases of giant cell arteritis were diagnosed, a number which corresponds to an incidence in the overall population of 21.5/10(5), and to an incidence of 76.6/10(5) for individuals age 50 years or older. These rates are higher than those previously reported in retrospective studies. The 3-yea…

A 68-year-old man with a history of abdominal aortic aneurysm presented with incidental coronary artery thickening on coronary CT angiography (CCTA). Eight years later, increased thickening was noted on CCTA and cardiac MRI, with PET/CT showing FDG avidity. Serology revealed elevated IgG4 and ESR. High-dose prednisone led to decreased wall thickening and FDG avidity; rituximab was initiated following relapse. This case highlights the challenges of diagnosing IgG4-related coronary arteritis and underscores the essential role of advanced imaging in guiding surveillance and therapy, especially in…

Giant cell arteritis (GCA) is a large-vessel vasculitis commonly affecting those above the age of 50 years. GCA presents with systemic inflammation and/or local ischemic symptoms, including the risk of visual loss. Scalp and lingual ischemia are rare but severe manifestations of GCA that can lead to delayed diagnosis and treatment, resulting in increased morbidity and mortality. Here we report the cases of two elderly women presenting with necrosis of the scalp and tongue respectively - emphasizing the need to suspect and identify the various manifestations of this complex disease. Treatment r…

Giant cell arteritis (GCA), also known as temporal arteritis, is a chronic vasculitis affecting large- and medium-sized arteries in patients over 50 years old. Untreated GCA can cause serious morbidity like irreversible vision loss, aneurysms, or stroke. We present a case of a patient who developed an acute headache and trismus within hours of undergoing a root canal treatment. Because GCA typically presents more gradually, the sudden onset of symptoms raised concern for dental-related complications or infection, contributing to a delay in considering GCA. This case highlights the importance o…
