Leflunomide is a disease-modifying antirheumatic drug (DMARD) that has been in routine clinical use for the treatment of rheumatoid arthritis (RA) and psoriatic arthritis for a decade. In RA, clinical trials of up to two years' duration showed that leflunomide monotherapy was equivalent to methotrexate in clinical and radiographic disease outcomes (tender and swollen joint counts, physician and patient global assessments, American College of Rheumatology and Disease Activity Score responses, slowing or halting of radiographic progression). In a number of studies, quality of life measurements i

OBJECTIVE: Axial involvement is an important manifestation of psoriatic arthritis (PsA). We aimed to identify risk factors associated with the presence of axial PsA (AxPsA) in patients with PsA.
METHODS: Patients with AxPsA (bilateral sacroiliitis >or= grade 2/unilateral sacroiliitis >or= 3 and inflammatory neck/back pain or limited spinal mobility) at first clinic visit were identified from the University of Toronto PsA clinic database. Risk factors associated with the presence of AxPsA were determined. Subsequently, patients without AxPsA at first clinic visit were identified. Under a multi

The paper presents comparative clinical and laboratory characteristics of 17 cases of Lyme-arthritis (LA) and 28 cases of reactive arthritis (RA). LA and RA patients differed by age and sex. LA arose after the tick bite followed by erythema migrans or infection symptoms. In RA patients the above symptoms were absent. In LA patients arthritis was associated with typical symptoms of neuroborreliosis. The articular syndrome manifested similarly in LA and RA: arthritis was preceded by arthralgias, low limb joints and periarticular tissues were involved most frequently. The differences were: monoar

We report on 48 children, 27 boys and 21 girls, with juvenile psoriatic arthritis. Onset during childhood differs in many ways from adult psoriatic arthritis. However, we detected a striking similarity to the subgroups of juvenile chronic arthritis in our patients. Pauciarticular onset was most prominent. Eight children presented with an early onset pauciarthritis, 33 patients showed a pauciarthritis type II with onset during school age; eight of them developed spondylarthritis with sacroiliitis. A seronegative polyarthritis was seen in six children. One girl suffered from seropositive disease

Septic arthritis of the temporomandibular joint is a rare condition. A case of acute staphylococcal suppurative arthritis of the temporomandibular joint (TMJ) complicating rheumatoid arthritis (RA) in a 53 year old woman is reported. The aetiology of septic arthritis may be traced to several predisposing factors and many specific agents. It would appear that the case presented is the result of predisposition of patients known to have RA to the complication septic arthritis. Some treatment recommendations are given.

OBJECTIVE: Skeletal tuberculosis (TB) arthritis is an uncommon but significant cause of osteoarticular morbidity, representing less than 1% of all TB cases. Delayed diagnosis often leads to persistent joint stiffness and pain. This study aims to identify early magnetic resonance imaging (MRI) features that distinguish TB arthritis from rheumatoid arthritis (RA).
METHODS: The MRI scans were retrospectively analyzed from 2010 to 2017 of 21 patients diagnosed with TB arthritis and 82 patients with RA. Imaging characteristics including synovial effusion, synovial membrane thickness, abscess wall

Introduction: Patients with rheumatoid arthritis (RA) and juvenile idiopathic arthritis (JIA) are at a twice-higher risk of developing adverse pregnancy outcomes, such as preterm births and infants with a low birth weight. We aimed to evaluate fetal growth among patients with and without rheumatoid arthritis and juvenile idiopathic arthritis (RA and JIA). Materials and Methods: We conducted a population-based cohort study in Denmark from 2008-2018, which included 503,491 singleton pregnancies. Among them, 2206 were pregnancies of patients with RA and JIA. We linked several nationwide databases

OBJECTIVE: Psoriatic arthritis (PsA) substantially impairs quality of life. Clinical trials generally focus on polyarticular PsA, but less is known about the assessment and management of oligoarticular and moderate PsA. An online survey was conducted to determine Canadian rheumatologists' perspectives on the definition and treatment of oligoarticular and moderate PsA.
METHODS: Regional and national experts treating patients with PsA were asked to complete an online survey to assess their approach to identifying and managing patients with PsA. Survey questions were developed based on guidance

OBJECTIVES: The classification of seronegative arthritides can be challenging. Our aim was to examine the incidence of SpA diagnosis among patients initially diagnosed as seronegative RA.
METHODS: Using nationwide Finnish registers from social insurance institutions, we identified all adult patients who were diagnosed with incident seronegative RA [International Classification of Diseases (ICD)-10 code M06] from 1 January 2000 to 31 December 2014. The patients whose diagnoses subsequently changed to the ICD-10 codes of SpA (M07, M45, M46, K50 and K51) were identified in the national care regi

Etanercept is a dimeric fusion protein based on the p75 TNF-alpha receptor. It binds to TNF-alpha and blocks its biologic activity. In randomized, double-blind, placebo-controlled trials, etanercept has therapeutic activity in rheumatoid arthritis, psoriatic arthritis, polyarticular-course juvenile idiopathic arthritis and ankylosing spondylitis. Etanercept improves joint inflammation, physical function and slows/halts structural damage, especially when combined with methotrexate. A sustained response is observed in a substantial percentage of patients. Although some safety issues should be co

Reactive arthritis is an extremely rare spondyloarthritis that affects the peripheral joints and spine, resulting in common symptoms such as arthritis, urethritis, conjunctivitis, and mucocutaneous lesions. On rare occasions, oral lesions such as circinate erosions on the hard and soft palate, gums, tongue, and cheeks may occur. Reactive arthritis may develop during or after genitourinary or gastrointestinal bacterial infections such as Shigella, Salmonella, Yersinia, and Chlamydia. A 36-year-old man presented with circinate balanitis, urethral discharge, oligoarthralgia, conjunctivitis, lymph

Rheumatoid arthritis (RA) is a systemic, autoimmune disease characterized by synovitis and irreversible joint destruction. RA development is a multi-stage process conditioned by the presence of genetic and environmental risk factors on the basis of which systemic immunization develops. The best known genetic factor predisposing to RA development is the polymorphism of HLA-DRB1 region of the major histocompatibility complex. Among environmental factors, importance in the pathogenesis of RA is attributed to the development of inflammation of the respiratory tract and chronic periodontitis caused

Rheumatoid arthritis affects around 1% of the population. The disease is characterized by joint inflammation and damage. Therapy with disease modifying drugs (DMARDs) has been proved to be efficient in decreasing disease activity and progress, along with improvement in symptoms and prevention of structural damage. The sooner the therapy is initiated, the better the outcome, probably due to a decreased burden of inflammation. The therapeutic potential of early treatment led to an accelerating interest in the preclinical phase of the disease. Many studies demonstrated that there is a preclinical

Acute septic arthritis is a condition with the potential for joint destruction, physeal damage, and osteonecrosis, which warrants urgent identification and treatment. The organism most frequently responsible is Staphylococcus aureus; however, our understanding of pathogens continues to evolve as detection methods continue to improve. MRI has improved our ability to detect concurrent infections and is a useful clinical tool where available. The treatment course involves intravenous antibiotics followed by transition to oral antibiotics when clinically appropriate. The recommended surgical treat

Juvenile idiopathic arthritis is the most common chronic rheumatic disease of unknown aetiology in childhood and predominantly presents with peripheral arthritis. The disease is divided into several subgroups, according to demographic characteristics, clinical features, treatment modalities and disease prognosis. Systemic juvenile idiopathic arthritis, which is one of the most frequent disease subtypes, is characterized by recurrent fever and rash. Oligoarticular juvenile idiopathic arthritis, common among young female patients, is usually accompanied by anti-nuclear antibodie positivity and a

Epidemiological studies suggest a viral etiology in approximately 1% of patients presenting with acute arthritis. The arthritogenic effect of viral infections may be related to viral invasion of synovial cells, the cellular and humoral immune response to viral antigens or by induction of autoimmunity. Viral arthritis can mimic rheumatoid arthritis by presenting as a symmetrical polyarticular disease often accompanied by a rash and influenza-like symptoms. Serological testing for pathogen-specific IgM and IgG antibodies is frequently performed for establishing a viral etiology of arthritis. Vir
