Applying personal genetic data to injury risk assessment in athletes.

Recent studies have identified genetic markers associated with risk for certain sports-related injuries and performance-related conditions, with the hope that these markers could be used by individual athletes to personalize their training and diet regimens. We found that we could greatly expand the knowledge base of sports genetic information by using published data originally found in health and disease studies. For example, the results from large genome-wide association studies for low bone mineral density in elderly women can be re-purposed for low bone mineral density in young endurance a

Autonomic cardiovascular control in Paralympic athletes with spinal cord injury.

INTRODUCTION: Disruption of autonomic control after spinal cord injury (SCI) results in life-threatening cardiovascular dysfunctions and impaired endurance performance; hence, an improved ability to recognize those at risk of autonomic disturbances is of critical clinical and sporting importance. PURPOSE: The objective of this study is to assess the effect of neurological level, along with motor, sensory, and autonomic completeness of injury, on cardiovascular control in Paralympic athletes with SCI. METHODS: Fifty-two highly trained male Paralympic athletes (age, 34.8 ± 7.1 yr) from 14 coun

[Injuries in elite athletes in the olympic period from 2008 to 2012].

INTRODUCTION: Research related to injuries of elite athletes is very demanding in comparison to research on injuries in team sports, and therefore, there are a lot fewer resources and data on injuries in track and field sports than in other sports. MATERIAL AND METHODS. The research included 78 athletes who were the members of junior and senior national track and field team of Serbia (34 women and 44 men), aged 15 to 32 years. The research was conducted in the official training camp for the national selection in Bar (Montenegro) by using a non-standardized survey research technique, an anonymo

Posterior thigh muscle injuries in elite track and field athletes.

INTRODUCTION: Posterior thigh muscle injuries in athletes are common, and prediction of recovery time would be of value. HYPOTHESIS: Knee active range of motion deficit 48 hours after a unilateral posterior thigh muscle injury correlates with time to full recovery. STUDY DESIGN: Cohort study (prognosis); Level of evidence, 2. METHODS: One hundred sixty-five track and field athletes with acute, first-time, unilateral posterior thigh muscle injuries were prospectively evaluated regarding knee active range of motion deficit. This was compared with the uninjured side 48 hours after injury. A co

Diagnosis and management of vascular injuries in the shoulder girdle of the overhead athlete.

Vascular injuries represent a rare cause of shoulder pain and functional limitation among overhead athletes. Complaints of heaviness, fatigue, paresthesias, and effort-related pain should prompt the sports medicine clinician to consider vascular pathology as a possible cause of such symptoms. Position-dependent compression of the subclavian and axillary vessels within the thoracic outlet may result in functional limitation and a decline in overhead athletic performance, particularly when symptoms occur in the dominant upper limb. Treatment options include physical therapy and (in the case of t

Labral tears, extra-articular injuries, and hip arthroscopy in the athlete.

Labral tears in athletes can lead to disabling hip pain and affect their athletic performance. Other intra-articular lesions, including chondral injuries, capsular abnormalities, and ligamentum teres tears, commonly coexist with acetabular labral tears. Isolated athletic injury or repetitive traumatic activity can lead to labral tears; however, underlying structural (femoroacetabular impingement) and developmental abnormalities predisposing athletes to labral pathology must be addressed. Recent studies have demonstrated lesions associated with acetabular labral tears, and that labral tears rar

Little league elbow: valgus overload injury in the paediatric athlete.

This article describes a variety of elbow injuries often seen in young, throwing athletes. Understandings of normal skeletal development, as well as the biomechanics of throwing, provide a basis on which to evaluate, diagnose and manage "little league elbow". With emphasis on an anatomically-based differential diagnosis, the pathologic forces placed upon the young thrower's elbow, and the subsequent injury patterns, are reviewed. Medial side injuries are the most common and relate to chronic forces of valgus overload produced during the early and late cocking phases of throwing. The majority o

Nonoperative versus operative treatment of acute disc injuries in athletes.

Many factors contribute to intervertebral disc problems; anatomic, physiologic, and mechanical factors are considered in the literature. Disc pathology as the cause of back or neck pain in the athletic population is primarily seen in acute disc injury. The research connecting degenerative disc disease as a precursor to acute disc injury is limited in scope. The research in discogenic etiology of back and neck injuries in the athletic population is also small in number, therefore resulting in limited information supporting the nonoperative versus operative treatment of disc injury. Treatment co

Elbow injuries in the throwing athlete. Difficult diagnoses and surgical complications.

Elbow injuries in throwing athletes can be challenging from the diagnostic and management perspectives. The stress of repetitive throwing does predispose athletes to certain conditions with which treating clinicians need to be familiar. An understanding of the anatomy of the elbow and the biomechanics of throwing is essential to making the correct diagnosis and instituting proper care. Failure of nonoperative measures often requires surgical intervention. A thorough understanding of the anatomy and the spectrum of conditions that can occur is needed before decisions regarding surgical manageme

Anterior cruciate ligament injury rehabilitation in athletes. Biomechanical considerations.

Postoperative rehabilitation is a major factor in the success of an anterior cruciate ligament (ACL) reconstruction procedure. Clinical investigations of patients after ACL reconstruction have shown that immobilisation of the knee, or restricted motion without muscle contraction, leads to undesired outcomes for the articular, ligamentous, and musculature structures that surround the knee. Early joint motion is beneficial for; reducing pain, capsular contractions, articular cartilage, and for minimising scar formation that limit joint motion. These findings, combined with graft materials that h

The immature athlete. Common injuries and overuse syndromes of the elbow and wrist.

Specific elbow and wrist injuries are predictable in the skeletally immature athlete based on the biomechanics of the sport and the age of the patient. The physician must be aware of the potential for overuse injuries. Modification in training regimens is essential for recovery. A greater emphasis must be placed on the prevention of these injuries. As a general rule, the young athlete should not progress more than 10% per week in the amount and frequency of training. Correction of muscle-tendon imbalances is accomplished by maintaining strength and flexibility of susceptible tissues. In throwe

Conservative treatment of isolated injuries to the posterior cruciate ligament in athletes.

We assessed arthroscopically 22 young athletes with an isolated acute posterior cruciate ligament (PCL) injury. Four had significant damage to the articular cartilage of the medial femorotibial compartment and were advised not to resume sports. Three underwent PCL reconstruction because of a reparable meniscal tear or instability. The other 15 were treated conservatively and resumed sport. At an average follow-up of 51 months, one had developed arthritic symptoms due to newly-developed severe chondral damage to the medial femoral condyle, but none of the other 14 had developed arthritic sympto

Arthroscopic management of wrist triangular fibrocartilage complex injuries in the athlete.

Wrist arthroscopy can facilitate early definitive diagnosis of debilitating soft tissue injuries in athletes. Many such injuries can be treated successfully with minimally invasive arthroscopic techniques, reducing the morbidity associated with surgical exposure and permitting earlier return to competition. The triangular fibrocartilage complex (TFCC) is vulnerable to injury from rotational forces or axial load applied to the hand. Under arthroscopic control, injuries to the central articular disk can be treated by excision of unstable tissue fragments; peripheral separation of the disk from t
