Panic disorder is often associated with the autonomic nervous system pattern - sympathetic activation and parasympathetic (vagal) withdrawal. However, we present one special case here to show a totally reversed pathogenesis - vagal activation occupying the leading role, which requires atropine to cure the patient's symptoms. Through this report, it is reasonably proven that panic disorder may be a heterogeneous condition, whose mechanism might be the imbalance between the sympathetic and parasympathetic tone.

OBJECTIVE: To determine the heart rate response to atropine (<0.1 mg) in anaesthetised young infants.
DESIGN: Prospective, observational and controlled.
SETTING: Elective surgery.
PATIENTS: Sixty unpremedicated healthy infants less than 15 kg were enrolled. Standard monitoring was applied. Anaesthesia was induced by mask with nitrous oxide (66%) and oxygen (33%) followed by sevoflurane (8%).

A medication-related cause must be sought when unusual symptoms occur. Topical treatments, including eye drops, whose side effects are more common in exposed children, need to be verified. We report here the cases of two children who developed systemic symptoms after the administration of atropine-based mydriatic eye drops. A 32-year-old man presented with symptoms of severe organophosphate poisoning and required an atropine infusion for 5 weeks. We believe the development of a paralytic ileus occurred as a rare feature of atropine toxicity when other signs were masked by the underlying condition. The onset of a paralytic ileus coincided with a spontaneous increase in red cell cholinesterase levels and may be an early sign of recovery from organophosphate poisoning.

Intermittent doses of suxamethonium have been known to result in bradycardia changes in cardiac rhythm. In order to assess the efficacy of a newer quaternary ammonium antimuscarinic, glycopyrrolate, on the cardia effects of intermittent suxamethonium, 28 patients received this drug intravenously just before induction of anesthesia.
