High-renin essential hypertension: adrenergic cardiovascular correlates.

1. Patients with mild essential hypertension and elevated plasma renin activity, when compared with normal subjects and hypertensive subjects with normal plasma renin, demonstrated features of sympathetic nervous cardiovascular excitation, accompanied by a raised plasma noradrenaline concentration. 2. An elevated heart rate at rest, shortened cardiac pre-ejection period, and greater heart rate reduction with acute beta-adrenoreceptor blockade (intravenous propranolol) in high-renin essential hypertension were indicative of adrenergic cardiac excitation. An elevated total of peripheral vascular

Novel needle guide reduces time to perform ultrasound-guided femoral nerve catheter placement: A randomised controlled trial.

BACKGROUND: Ultrasound-guided nerve blocks have become the standard when performing regional nerve blocks in anaesthesia. Infiniti Plus (CIVCO Medical Solutions, Kalona, Iowa, USA) is a needle guide that has been recently developed to help clinicians in performing ultrasound-guided nerve blocks. OBJECTIVES: We tested the hypothesis that femoral nerve catheter placement carried out with the Infiniti Plus needle guide will be quicker to perform than without the Infiniti Plus. Secondary aims were to assess whether the Infiniti Plus needle guide decreased the number of block attempts and also whe

Myofascial trigger points in intercostal muscles secondary to herpes zoster infection of the intercostal nerve.

Chronic pain in the chest wall is a major complication after herpes zoster infection of intercostal nerves. It is usually difficult to control pain of such origin. Two cases are reported of postherpetic neuralgia after herpes zoster infection involving the intercostal nerves. Both patients had shooting, burning, aching, and localized pain in the muscle supplied by the involved intercostal nerves 1 to 3 months after onset. Compression palpation of a tender spot in one of these muscles induced a referred pain that followed the corresponding interspace, usually in the distal anterior direction. L

[Acute pancreatitis. Neurovascular and microcirculatory changes. Pathogenic and therapeutic study].

A group of 159 dogs divided in 8 sub-groups were studied, in regard to pancreatitis, the purpose of the study was to investigate the participation of the automatic nervous system in the course of acute pancreatitis. The procedures and the results were as follows: 1. Pancreatitis was induced in two forms: a) Injection of gallbladder bile, from the same animal to the pancreatic duct. b) Blind duodenal loop with exclusion of the distal duodenum through the pylorus. In both cases acute pancreatitis was obtained. Fat necrosis was predominant in type a, and hemorrhagic lesions in type b. 2. The anes

[Proximal lower motor neuron syndrome associated with serum antibodies to asialo-GM1, GM1 and LM1].

A 31-year-old man had noticed slowly progressive weakness in his right upper limb girdle. On admission at age 32, fasciculation and muscle atrophy were observed in the right pectoralis major muscle. There was mild muscle weakness in the right triceps brachii muscle. The deep tendon reflexes were normal and the pathologic reflexes were absent. Sensory and autonomic nerve functions were intact. Needle EMG showed fibrillation potentials and positive sharp waves in the right pectoralis major, infraspinatus, extensor carpi radialis, extensor digitorum muscles, the bilateral sternocleidomastoideus a

Purine nucleotides.

The existence of nonadrenergic, noncholinergic nerve components in the autonomic nervous system is now well established. They are strongly represented in the gastrointestinal tract of all vertebrates and have been identified in a variety of other organs, including lung, trachea, bladder, esophagus, eye, seminal vesicles, and possibly parts of the vascular and central nervous systems. Their ultrastructural identification and transmission properties are known and their physiological role is beginning to be understood, at least in the gastrointestinal tract. Evidence that ATP is the transmitter r

Efficacy and safety of stellate ganglion block for postoperative pain management: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Stellate ganglion block (SGB) is a form of regional anesthesia employed to reduce postoperative pain management. This meta-analysis assesses its efficacy and safety for postoperative pain control. METHODS: We conducted a comprehensive search of PubMed, Embase, Cochrane Library, and Web of Science covering all records up to September 20,2025 to identify eligible randomized controlled trials (RCTs). Stata 15.0 software was applied for data analysis, and RevMan 5.4 was employed to assess the risk of bias. RESULTS: A total 33 RCTs comprising 2231 participants were included, SGB may r

Impar ganglion block with botulinum toxin in the management of perineal oncological pain: a case report.

Chronic perineal pain is a common complication among patients with cancer. Due to the complex innervation of this anatomical region, pain management is often challenging and significantly impacts patients' quality of life. We present the case of an elderly patient with advanced-stage rectosigmoid junction adenocarcinoma who experienced severe coccygeal-perineal oncologic pain. Despite a multimodal analgesic treatment approach, the patient exhibited refractoriness to conventional pain management. Consequently, an interventional analgesic approach was implemented using an impar ganglion block wi

Novel Single-Needle Approach for Retrocrural Celiac Plexus Blockade Using Fluoroscopic Guidance: A Case Report.

Celiac plexus neurolysis has been shown to be an effective analgesic option for patients with visceral pain related to intraabdominal malignancies. In the setting of significant tumor burden, the celiac plexus may be inaccessible, limiting the efficacy of the transcrural approach. This case report describes a novel single-needle approach to retrocrural celiac plexus blockade, allowing for rapid blockade with a needle trajectory contralateral to the aorta in a 73-year-old woman with altered anatomy secondary to advanced metastatic colorectal cancer.

Placement of a Continuous Stellate Ganglion Block for Treatment of Refractory Ventricular Fibrillation in the Setting of Known Prinzmetal Angina During Pregnancy: A Case Report.

A patient with a history of Prinzmetal angina, refractory ventricular fibrillation, cardiac arrest with an implantable cardioverter-defibrillator, and obesity presented to the emergency department at 17 weeks gestational age with a chief complaint of angina and multiple episodes of defibrillation. A T3/4 thoracic epidural was placed to assess the effectiveness of a partial chemical sympathectomy in alleviating symptoms of angina as well as decreasing the amount of defibrillation episodes. Once this proved to be beneficial in accomplishing both of these goals, a more specific approach was desig

Medial approach of ultrasound-guided costoclavicular plexus block and its effects on regional perfussion.

INTRODUCTION: Ultrasound-guided infraclavicular block in the costoclavicular space located between the clavicle and the first rib, reaches the secondary trunks when they are clustered together and lateral to the axillary artery. This block is most often performed through a lateral approach, the difficulty being finding the coracoid process an obstacle and guiding the needle towards the vessels and pleura. A medial approach, meaning from inside to outside, will avoid these structures. Traditionally the assessment of a successful block is through motor or sensitive responses but a sympathetic fi

Hemorrhagic gastritis and duodenitis following celiac plexus neurolysis.

INTRODUCTION: Neurolytic celiac plexus block is a well established intervention to palliate pain, and it potentially improves quality of life in patients suffering from an upper abdominal malignancy, specifically pancreatic cancer. METHODS: We describe a 61-year-old female with a history of pancreatic cancer, unexplained transfusion dependent anemia with a normal recent upper endoscopy, and abdominal pain, who had previously undergone gastrojejunostomy and a Roux-en-Y hepaticojejunostomy as well as chemotherapy and radiation therapy. She suffered from intractable abdominal pain and elected to

Psychoneurological applications of endoscopic sympathetic blocks (ESB).

In addition to more widely and longer known indications of ETS, various neurological disorders and psychologically stressful situations in their worst expressions might be alleviated by the reversible ESB procedure. The patients with social phobia, especially those who have also blushing and/or stage fright type of heart racing, benefit from the ESB. The disturbances of the sympathetic nervous system, e. g. in Parkinson's disease and multiple system atrophy might be alleviated with sympathetic block, especially the extrapyramidal symptoms in these diseases. In migraine, sympathetic surgery has
